[Results of plate osteosynthesis in comminuted fracture of the olecranon].

Comminuted fractures of the olecranon are an absolute indication for operative treatment. Adequate therapy is only possible when open reduction and plating are performed. In the last 20 years, 31 of our patients, i.e. 0.0001% of all patients treated for injuries, had this type of olecranon fracture, and in 25 follow-up examination was possible. The fracture type was classified according to the ASIF system. The range of motion, i.e., extension, flexion, pro- and supination, and pain, physical strength and rehabilitation were classified according to our own system. In cases with concomitant frac

Clinical Analysis of the Frosch Approach in the Treatment of Posterolateral Tibial Plateau Fractures Combined with Lateral Tibial Plateau Fractures.

OBJECTIVE: The treatment of posterolateral tibial plateau fractures is difficult, and providing sufficient exposure and effective fixation is a challenge. There is great controversy regarding the surgical approach for posterolateral tibial plateau fractures. The purpose of the study was to investigate the clinical effects of open reduction and internal fixation using the Frosch approach for the treatment of posterolateral tibial plateau fractures combined with lateral tibial plateau fractures. METHODS: Data from 19 patients with posterolateral tibial plateau fractures combined with lateral ti

Distal radius plate: correlation between the length of distal epiphyseal screws and the width of the watershed line.

BACKGROUND: Accurate distal screw length selection during volar plating of distal radius fractures is important to provide subchondral support while reducing the risk of dorsal cortical penetration. The width of the watershed line may provide an anatomical guide for screw selection. METHODS: Fifty-one wrist CT scans (35 men, 16 women) were retrospectively analyzed. Watershed line width and distal epiphyseal screw lengths (B1-B5) were measured after virtual placement of fixed-angle volar plates. The relationship between watershed line width and screw length was assessed using linear regression…

Outcomes of Free Vascularized Fibular Grafts in Treating Massive Forearm Skeletal Defects.

INTRODUCTION: Reconstructing long bone defects in the upper limbs, particularly the radius and ulna, poses significant challenges. These defects, resulting from trauma, tumors, infections, or congenital anomalies, require precise surgical intervention for functional restoration. Traditional non-vascularized autogenous bone grafts have limitations, such as resorption and limited biological activity. To address these challenges, free vascularized fibular grafts (FVFGs) have been developed, offering enhanced recovery by supplying nutrients and structural support, particularly in large defects or

Damage control orthopedics applied in an 8-year-old child with life-threatening multiple injuries: A CARE-compliant case report.

RATIONALE: Damage control is a staged surgical approach to manage polytraumatized patients. The damage control approach comprises three steps. First, bleeding is controlled and fractures are stabilized temporarily; second, vital parameters are stabilized and the child is rewarmed in the intensive care unit; and third, the child is reoperated for definitive repair of injuries. We aimed to describe the feasibility of the damage control orthopedic approach in a child. PATIENT CONCERNS: An 8-year-old girl fell from the balcony of the 5th floor onto concrete pavement and was admitted to our accide

Serum Levels of a Cathepsin-K Generated Periostin Fragment Predict Incident Low-Trauma Fractures in Postmenopausal Women Independently of BMD and FRAX.

Periostin is a matricellular protein involved in bone formation and bone matrix organization, but it is also produced by other tissues. Its circulating levels have been weakly associated with bone microstructure and prevalent fractures, possibly because periostin measured by the current commercial assays does not specifically reflect bone metabolism. In this context, we developed a new ELISA for a periostin fragment resulting from cathepsin K digestion (K-Postn). We hypothesized that circulating K-Postn levels could be associated with bone fragility. A total of 695 women (age 65.0 ± 1.5 years)

Skeletal Effect of Parathyroidectomy on Patients With Primary Hyperparathyroidism: A Systematic Review and Meta-Analysis.

CONTEXT: Parathyroidectomy (PTX) is recommended for curing primary hyperparathyroidism (PHPT), although uncertainty remains regarding the extent of fracture risk reduction following surgery. OBJECTIVE: This work aimed to compare fracture risk and bone mineral density (BMD) changes in patients with PHPT undergoing PTX vs observation (OBS). METHODS: We systematically searched PubMed, Embase, and the Cochrane Library until September 2022, including randomized controlled trials (RCTs) and cohort studies, and reviewed citations from previous reviews. Among 1260 initial records, 48 eligible articl

Photodynamic bone stabilization for traumatic and pathologic fractures: a systematic review of utilization, complications, and patient-reported outcomes.

INTRODUCTION: The photodynamic bone stabilization system (PBSS) was was developed in 2010, and in 2018 gained FDA approval in the United States. Given its relative novelty, our analysis sought to analyze the available literature exploring the indications, outcomes, and complications of the PBSS. METHODS: We performed a systematic review (PROSPERO registration of study protocol: CRD42022363065, October 8th, 2022). PubMed, EBSCOHost, and Google Scholar electronic databases were queried to identify articles evaluating PBSS in the treatment of pathologic or traumatic fractures between January 1 2

Elbow Dislocation with Irretrievable Rotating-Rope Injury of the Forearm: Case Report and Literature Review.

BACKGROUND: Simultaneous dislocation of the elbow, radioulnar joint and proximal radius fracture with rotary noose injury to the medial ulna tubercle is extremely rare. An emergency surgery was performed to reduce it. The radial head with the backbone was reset after two hammers were fixed, then the radial capitulum safety was fixed with a locking plate. After the ulnar instability was examined, two Kirschner wires were drilled percutaneously to fix the elbow flexion at 100° under closed reduction, and two Kirschner wires were drilled percutaneously to fix the ulnar joint. Good follow-up resul

Discrete Element Simulation of Fracture Evolution around a Borehole for Gas Extraction in Coal Seams.

The stability analysis of underground caverns, tunnels, and boreholes is of great significance to underground engineering. In order to study the stress change of the coal around a hole and the evolution law of fractures during the coal seam drilling process, the discrete element simulation of the coal seam drilling process was carried out by using the particle flow code (PFC2D). The results show that during the drilling process, under the influence of confining pressure and drilling disturbance, the coal stress field around the hole and the development of fractures around the hole have the cha

A new fracture model for "terrible triad" injuries of the elbow: influence of forearm rotation on injury patterns.

OBJECTIVE: The purpose of this study was to evaluate the influence of forearm rotation on failure patterns of the elbow under axial loads. METHODS: Fourteen upper extremities were resected mid-humerus and mounted on a custom apparatus, which allowed rotation of the ulna, radius, and humerus about a fixed wrist while loading in axial compression. Seven specimens were loaded to failure with the forearm in pronation and 7 in supination. RESULTS: Six of the 7 elbows axially loaded in pronation resulted in fractures of the radial head and coronoid with posterior dislocation (terrible triad). Six

The volar anatomy of the distal radius--an MRI study of the FCR approach.

INTRODUCTION: Fractures of the distal radius are one the commonest orthopaedic injuries. Recent advances in implant technology have seen a dramatic rise in the number of fractures treated with volar locked plates, as they permit accurate peri-articular reconstruction. The surgical approach along the bed of flexor carpii radialis (FCR) tendon encounters a number of key soft tissue and neurovascular structures during the dissection to the fracture plane. The aim of this study was to describe the exact position of such structures involved (and hence at risk) during the FCR approach. METHODS: 100

ChronOS inject in children with benign bone lesions--does it increase the healing rate?

OBJECTIVE: Benign bone lesions in children although rare, can result in a pathological fracture. Although their etiology and pathogenesis are not yet entirely clear, the phenomenon of spontaneous healing is well known. Nevertheless, some benign bone lesions are unlikely to heal spontaneously due to the patient's age or high risk of fracture and deformity due to the lesion's location or size. The following study presents our results after treatment of these bone cysts with chronOS Inject. METHODS: From June 2004 to May 2007 23 patients with 24 benign bone cysts were treated with chronOS Inject

[Injuries in the elderly patients].

More and more higher development of civilisation causes constant lengthening of life in humans. Changes, which occur during growing old of organism predispose to increased risk of trauma. Financial cost of medical treatment of injuries in elderly are higher and higher. Degenerative disease of joints, osteoporosis, earlier body injuries and co-existing other diseases are important risk factors of trauma. Deficiencies of eyesight, hearing and prolonged time reaction are other strengthening risk of trauma. Falls and motor-vehicle accidents are the most frequent causes of trauma in elderly. Distal

[Role of the bone mineral density investigation through measuring of peripheral dual-energetic x-ray absorption in prevention of fractures in children with osteogenesis imperfecta].

The Osteogenesis imperfecta is hereditary disturbance of collagen biosynthesis. Illness is characterized by frequent fractures that consequently cause functional disturbance of locomotor system as well as deformation of long tubular bones. The distinguishing feature of this illness is the secondary osteoporosis. The latter in its turn worsens clinical course and the outcome of the illness. The purpose of the present work was the study of bone mineral density in children with osteogenesis imperfecta through application of dual energetic x-ray absorption measurements. In total 32 patients 1-15 y

Load-displacement behavior in a distal radial fracture model. The effect of simulated healing on motion.

External fixation of fractures of the distal end of the radius neutralizes external forces and maintains axial alignment during healing. As far as we know, there have been no biomechanical studies of the effects of early removal of the fixator in a partially healed fracture model. The purpose of the present study was to observe the load-displacement behavior of a distal radial fracture model in which we had simulated partial healing by injection of butyl-rubber caulk and augmented this simulated healing with Kirschner-wire fixation. Sixteen fresh-frozen hand-wrist-forearm specimens from cadave

Cyclic Fatigue of Rotary Versus Reciprocating Endodontic Files: An In Vitro Study of Engine-Driven Endodontic Files.

Background/Objectives: Instrument fracture remains a significant complication in endodontics. This study compared the resistance to cyclic fatigue failure between rotary and reciprocating nickel-titanium file systems, as well as differences related to file size and taper. Methods: Nineteen rotary and reciprocating file types (n = 10 per group) were evaluated in three independent test series, harmonized according to file size and system. Cyclic fatigue testing was conducted using a static model with a stainless-steel artificial canal, with an internal diameter of 0.9 mm, a 75° curvature angle,
