Generalized vestibular hyporeflexia and chronic upbeat nystagmus due to thiamine deficiency.

BACKGROUND: Ocular motor and vestibular manifestations of Wernicke's thiamine deficiency (WTD) are frequent and heterogeneous. Previous neur … lami, but normal brainstem and cerebellum. Vestibular evaluation showed significant vestibular hyporreflexia affecting all six canals, and a chronic upbeat nystagmus, now for 14 months after WTD onset. METHODS: Serial clinical, video head impulse, nystagmus analysis, cervical and ocular vestibular evoked responses. She is undergoing treatment with Meman …

[A case of the abdominal form of Wilson's disease with atypical clinical symptoms in a 19-year-old man].

A case of the abdominal form of Wilson's disease is described in a male patient aged 19 years. The case progressed rapidly and presented rarely observed signs of central nervous system damage. Morphological examination of the brain demonstrated well developed picture of tissue damage characteristic of this disease entity.

Near-Fatal Mastocytosis Mimicking as Malignant Vasovagal Syncope.

BACKGROUND: Severe apparent reflex syncope may have other causes. CASE SUMMARY: A young girl was investigated for recurrent faints expected to be caused by reflex syncope, but ultimately complicated by cerebral ischemic lesions. A diagnosis of systemic mastocytosis (bone marrow mastocytosis) was finally made, after documentation of short-living abnormal coagulation tests and transient elevated tryptase levels. DISCUSSION: Severe mastocytosis-related vasoplegia can be the underlying cause of syncope and prolonged cerebral ischemia. Mastocytosis is a clonal disorder stemming from an acquired s…

[Epilepsy definitions, classifications, and epidemiology].

EPILEPSY DEFINITIONS, CLASSIFICATIONS AND EPIDEMIOLOGY. Epilepsy is the most common neurological disease in the world and the second most common in France. It is defined by the occurrence of 2 unprovoked (or reflex) epileptic seizures more than 24 hours apart, or by the occurrence of an unprovoked (or reflex) epileptic seizure with a recurrence probability of at least 60%. Anti-epileptic treatment can now be started immediately after a first unprovoked epileptic seizure, if risk factors for recurrence are identified, principally the presence of epileptic abnormalities on the electroencephalogr…

Baroreflex Curve Fitting Using a WYSIWYG Boltzmann Sigmoidal Equation.

Arterial baroreflex assessment using vasoactive substances enables investigators to collect data pairs over a wide range of blood pressures and reflex reactions. These data pairs relate intervals between heartbeats or sympathetic neural activity to blood pressure values. In an X-Y plot the data points scatter around a sigmoidal curve. After fitting the parameters of a sigmoidal function to the data, the graph's characteristics represent a rather comprehensive quantitative reflex description. Variants of the 4-parameter Boltzmann sigmoidal equation are widely used for curve fitting. Unfortunate…

Neural Activation of Molecular Circuitry in Intermittent Hypoxia.

People living at sea level experience intermittent hypoxia (IH) as a consequence of sleep apnea, which is a highly prevalent respiratory disorder. Sleep apnea patients and rodents exposed to IH exhibit autonomic dysfunction manifested as increased sympathetic nerve activity and hypertension. This article highlights physiologic basis of autonomic disturbances by IH, which involves abnormal activation of the carotid body (CB) chemo reflex by reactive oxygen species (ROS).We further evaluate major molecular mechanisms underlying IH-induced ROS generation including transcriptional activation of ge…

Mechanisms and Therapeutic Relevance of Neuro-immune Communication.

Active research at the frontiers of immunology and neuroscience has identified multiple points of interaction and communication between the immune system and the nervous system. Immune cell activation stimulates neuronal circuits that regulate innate and adaptive immunity. Molecular mechanistic insights into the inflammatory reflex and other neuro-immune interactions have greatly advanced our understanding of immunity and identified new therapeutic possibilities in inflammatory and autoimmune diseases. Recent successful clinical trials using bioelectronic devices that modulate the inflammatory…

Rub epilepsy in an infant with Turner syndrome.

We report a case of infantile refractory epilepsy associated with Turner syndrome (TS), showing very frequent, focal clonic seizures of the left upper extremity. Characteristically, in addition to spontaneous fits, her seizure was inducible by rubbing her left hand and forearm for a few seconds. Accordingly, she was diagnosed with a rare form of reflex epilepsy, "rub epilepsy". Neuroradiological investigation indicated the existence of cortical abnormalities, such as focal cortical dysplasia of the right parietal lobe. Patients with TS are reported to have neuroanatomical abnormalities, especi…

Wideband detection of middle ear muscle activation using swept-tone distortion product otoacoustic emissions.

The measurement of efferent-induced suppression of otoacoustic emissions (OAEs) using contralateral acoustic stimulation (CAS) is complicated by potential contamination by the middle ear muscle reflex (MEMR), particularly at moderate to high CAS levels. When logarithmically sweeping primaries are used to measure distortion product otoacoustic emissions, the level and phase of the primaries at the entrance of the ear canal may be monitored simultaneously along with the OAEs elicited by the swept-tones. A method of detecting MEMR activation using swept-tones is presented in which the differences…

Evaluation of three medetomidine-based protocols for chemical restraint and sedation for non-painful procedures in companion rats (Rattus norvegicus).

Three medetomidine-based protocols were evaluated for sedation in companion rats undergoing diagnostic procedures. Group Me received medetomidine at 150 μg/kg intramuscularly (IM); group Me-Bu received medetomidine 100 μg/kg IM and butorphanol 2 mg/kg IM, and group Me-Bu-Mi received medetomidine 50 μg/kg IM, butorphanol 2 mg/kg IM and midazolam 1 mg/kg IM. The righting reflex disappeared more quickly in the Me-Bu-Mi group, but recovery after atipamezole was longer. In group Me, a palpebral reflex was present throughout sedation in more rats than in the other two groups. Pulse and respiratory r…

Migraine with cranial autonomic features following surgically induced post-ganglionic sympathetic lesion.

BACKGROUND: Although not typically considered as part of the clinical phenotype of migraine, cranial autonomic symptoms, such as lacrimation … onomic dysfunction. At the time of the pain, recruitment and activation of the trigeminal autonomic reflex were demonstrated by lacrimation. CONCLUSION: The clinical picture suggests peripheral unmasking of the underlying central trigeminal autonomic reflex that is active in migraine. Recognition of cranial autonomic symptoms in migraine is a key to con …

Associations of retinal microvascular signs and intracranial large artery disease.

BACKGROUND AND PURPOSE: Intracranial large artery disease (ICLAD) is a major cause of ischemic stroke. Retinal microvascular changes are ass … lar caliber, focal arteriolar narrowing, or arteriovenous nicking. Severe enhanced arteriolar light reflex was independently associated with any ICLAD (P=0.006) and severe ICLAD (P<0.001). CONCLUSIONS: Enhanced arteriolar light reflex, but not retinal vessel caliber, was related to ICLAD. These data suggest that retinal microvascular signs have specific …

Orgasm-induced seizures: male studied with ictal electroencephalography.

Reflex seizures can occur in response to a variety of stimuli, both sensory and emotional. Common triggers include light and music; however, in a growing number of case reports, the phenomenon of sexual activity triggering epileptic seizures is described. The majority of these case reports have been in women so far, and most have been found to localise to the right cerebral hemisphere on interictal electroencephalography (EEG). We report the case of a 34-year-old male with orgasm-induced seizures, recorded on ictal EEG. This gentleman's electrophysiology localised his seizure focus to the left…
