The yield of tuberculosis contact investigation in low- and middle-income settings: a systematic review and meta-analysis.

BACKGROUND: Contact investigation, the systematic evaluation of individuals in close contact with an infectious tuberculosis (TB) patient, is a key active case-finding strategy for global TB control. Better estimates of the yield of contact investigation can guide strategies to reduce the number of underreported and underdiagnosed TB cases, approximately three million cases per year globally. A systematic review (Prospero ID # CRD42019133380) and meta-analysis was conducted to update and enhance the estimates of the yield of TB contact investigation in low- and middle-income countries (LMIC).

Effect of valve suture technique on incidence of paraprosthetic regurgitation and 10-year survival.

BACKGROUND: The primary objective was to estimate the risk of paraprosthetic regurgitation (PPR) after aortic (AVR) and mitral valve replacement (MVR) using interrupted (IN) or semicontinuous (SC) sutures. The secondary objective was to estimate the risk of redo valve surgery and 10-year survival after valve replacement performed using either suture technique. METHODS: Patients who underwent mechanical AVR or MVR using a St. Jude prosthesis between December 1991 and June 1997 were included. Eighteen patients had MVR and 43 had AVR using IN sutures; 49 and 83 patients received MVR and AVR, res

Use of beta blockers and the risk of death in hospitalised patients with acute exacerbations of COPD.

BACKGROUND: Cardiovascular disease is a major cause of death in patients with chronic obstructive pulmonary disease (COPD) and predicts hospitalisation for acute exacerbation, in-hospital death and post-discharge mortality. Although beta blockers improve cardiovascular outcomes, patients with COPD often do not receive them owing to concerns about possible adverse pulmonary effects. There are no published data about beta blocker use among inpatients with COPD exacerbations. A study was undertaken to identify factors associated with beta blocker use in this setting and to determine whether their

Real-world effectiveness of clozapine augmentation with antidepressants: within-subject data evidence from two European nationwide cohorts.

BACKGROUND: Only 40% of people with treatment-resistant schizophrenia will have an adequate response to clozapine. Data on the effect of antidepressant augmentation on these patients are scarce. We aimed to study the effectiveness and safety of antidepressant augmentations to clozapine in two nationwide cohorts. METHODS: For this within-subject data evidence study, data from two cohort studies were analysed then meta-analysed, including patients with schizophrenia using clozapine identified from Finnish (1996-2017) and Swedish (2006-23) nationwide registers, followed up from first clozapine u

Effect of Early Treatment of Spasticity After Stroke on Motor Recovery: Protocol for the Baclotox Multicenter, Double-Blind, Double-Dummy Randomized Controlled Trial.

BACKGROUND: Individuals with poststroke hemiplegia often develop spasticity, which increases disability. Antispastic treatments such as baclofen and botulinum toxin are commonly prescribed in poststroke recovery. However, their impact on motor recovery, especially when administered within the first 2 months after stroke, remains unclear. OBJECTIVE: This study aims to compare the motor recovery effects of botulinum toxin versus oral baclofen. The hypothesis is that botulinum toxin is more supportive of motor recovery than baclofen and enhances functional recovery. METHODS: The study is a mult

Sequence optimization of a DNA aptamer inhibiting COVID-19 infection guided by analysis of secondary structure distribution.

The post-SELEX optimization of nucleic acid aptamers is often hindered by conformational ambiguity, as the specific functional structure is typically unknown. This study presents a combined computational and experimental framework to address this challenge, using an aptamer that inhibits the SARS-CoV-2/ACE2 interaction as a model system. We analyzed the secondary structure ensembles of a pool of aptamer candidates from an initial screening, employing kinetic and structural clustering alongside a novel nucleotide-structure similarity score. By correlating structural similarity patterns with mea
