Diaphyseal forearm fractures, 20 years after surgical treatment. Is there still an indication for percutaneous fixation?

OBJECTIVE: This is a retrospective study of 98 diaphyseal forearm fractures in adults, treated by a percutaneous technique with intramedullar Kirchner wires. MATERIALS AND METHOD: We reviewed 64 patients with 98 forearm fractures with a radiographic follow-up, assessing the presence of pseudarthrosis or delayed bone union and evaluating functional outcomes with the Anderson and the Disability of the Arm, Shoulder and Hand scale. RESULTS: Clinical and radiological bone union was achieved in an average of 12 weeks. We obtained 77% of excellent and good results following Anderson's scale. There

Arthroscopic assisted tendon reconstruction for triangular fibrocartilage complex irreparable tears.

UNLABELLED: We report our 11-year experience of performing arthroscopically assisted triangular fibrocartilage complex reconstruction in the treatment of chronic distal radio-ulnar joint instability resulting from irreparable triangular fibrocartilage complex injuries. Eleven patients were treated. Three skin incisions were made in order to create radial and ulna tunnels for passage of the tendon graft, which is used to reconstruct the dorsal and palmar radio-ulnar ligaments, under fluoroscopic and arthroscopic guidance. At a mean follow-up of 68 months all but one had a stable distal radio-ul

EXTRA-ARTICULAR FRACTURE OF THE MEDIAL END OF THE CLAVICLE ASSOCIATED WITH TYPE IV ACROMIOCLAVICULAR DISLOCATION: CAAE REPORT.

Fractures of the clavicle and acromioclavicular dislocations are very common injuries when they occur separately. The combination of an acromioclavicular dislocation and a fracture of the lateral third of the clavicle is not rare. However, there are very few reported cases of acromioclavicular dislocations associated with fractures of the middle third of the clavicle; those associated with fractures of the medial third are even rarer. We report the case of an adult male who suffered an acromioclavicular dislocation (type IV) associated with a displaced extra-articular fracture of the medial en

Total Joint Replacement for Severe Bilateral Condylar Resorption--Gorham-Stout Syndrome.

Gorham-Stout syndrome is an extremely rare bone disorder characterized by osteolytic bony resorption. It is of unknown etiology and thought to result from localized endothelial proliferation of lymphatic vessels that cause the bony resorption. Diagnosis of Gorham-Stout can only be made after excluding other common causes of bony resorption such as those caused by infection, inflammation, malignancy, and endocrine. The clinical symptoms vary from minor pain and swelling to non-healing fractures from minor injuries. Due to the rarity of the disease, the overlap of symptoms with other diseases, a

Traumatic peripheral neuropraxias in neonates: a case series.

Traumatic peripheral nerve palsies in the newborn are uncommon but are a cause of severe anxiety in parents. Erb's palsy, brachial plexus, radial nerve and facial nerve are the common nerves affected. Perinatal injuries are the most frequent cause of traumatic peripheral neuropraxias / nerve palsies. Usually these neuropraxias are self-limited with good recovery with conservative management in majority of cases.Ten neonates with peripheral neuropraxias were included in this retrospective study based on a review of these cases over a period of three and a half years. Their clinical profile, pre

Arthroscopic rotator cuff repair: suture anchor properties, modes of failure and technical considerations.

Rotator cuff injury and tears are a common source of shoulder pain, particularly among the elderly. Arthroscopic repair has now become the mainstay in the treatment of significant injuries that have failed conservative therapy. Compared with the traditional open technique, arthroscopic repair offers patients smaller incisions and less soft-tissue trauma, which result in improved postoperative pain and rehabilitation. The advances that have made arthroscopic repairs a reality includes improvement in arthroscopic rotator cuff instrumentation, particularly suture anchors. Suture anchors are used

Chondrogenic differentiation and lubricin expression of caprine infraspinatus tendon cells.

Reparative strategies for the treatment of injuries to tendons, including those of the rotator cuff of the shoulder, need to address the formation of the cartilage which serves as the attachment apparatus to bone and which forms at regions undergoing compressive loading. Moreover, recent work indicates that cells employed for rotator cuff repair may need to synthesize a lubricating glycoprotein, lubricin, which has recently been found to play a role in tendon tribology. The objective of the present study was to investigate the chondrogenic differentiation and lubricin expression of caprine inf

[Multiple epiphyseal separations in a child with cerebral palsy and scurvy].

Scurvy is a dietary disease due to Vitamin C deficiency. Vitamin C is related to collagen synthesis and metabolism. Malnutrition, problems in bowel absorption, alcoholism and cerebral palsy are clearly often linked with scurvy, even it is no more common in the industrialized countries. Its clinical features are: asthenia, weight loss, appetite decrease, irritability, gingival or mucous lesions, porpora, follicular hyperkeratosis, musculoskeletal pain due to multiple fractures and subperiosteal bleedings, pseudoparalisis (frog-like position of legs) and refuse to walk. Authors report on a nine

Operative versus nonoperative treatment of acute shoulder dislocation in the athlete.

The purpose of this manuscript is to review evaluation and treatment techniques in patients with glenohumeral instability with specific attention to treatment of anterior shoulder dislocation. Scrutiny of the best available evidence reveals that: 1) there is little quality evidence to advocate one reduction technique over another, 2) premedication with intra-articular lidocaine is preferred over intravenous narcotics and benzodiazepine, 3) recurrence rates are age related and may be associated with higher energy injuries, but other predictors of recurrence are not clear, 4) post-reduction immo

Clinical application of ipsilateral C7 nerve root transfer for treatment of C5 and C6 avulsion of brachial plexus.

We applied a nerve transfer, using the ipsilateral C7 nerve root to treat the C5 and C6 root avulsion of the brachial plexus. Four patients with C5 and C6 preganglionic injury were operated on with this new technique from 1998-2000. Transfer of the spinal accessory nerve to the suprascapular nerve was simultaneously done in 2 these patients. After a follow-up of 1-2.5 years, the muscle strength of elbow flexors recovered to M4 (Lovett) in all cases, shoulder abduction of >90 degrees with external rotation of 30-40 degrees was gained in two cases, and that of 15-45 degrees with no external rota

Scalene regional anesthesia for shoulder surgery in a community setting: an assessment of risk.

BACKGROUND: A retrospective review of shoulder procedures using scalene block anesthesia was performed. METHODS: The records of all 218 patients who had undergone scalene block anesthesia over a three-year period at two facilities were retrospectively reviewed. All blocks were performed with use of a standard blunt-needle technique with the patient awake and with use of preoperative nerve stimulation to localize the brachial plexus. RESULTS: Adjunctive general anesthesia was used for 179 (82%) of the 218 patients. Seventy-two patients (33%) required intravenous pain medication immediately on

Longitudinal brainstem laceration associated with complex basilar skull fractures due to a fall: an autopsy case.

This report describes an autopsy case of a rare longitudinal brainstem laceration associated with complex basilar skull fractures. The victim was a 40-year-old male who died immediately after falling from a roof (9.2m in height) of a factory onto a concrete floor. The postmortem examination revealed an incomplete ring fracture of the base of the skull with longitudinal fractures of the sphenoid (clivus of the dorsum sellae turcicae) and occipital bones, cerebral contusions in the frontal and temporal poles, a longitudinal brainstem laceration at the posterior median sulcus of the pons accompan

[Fractures of the upper extremity in restrained automobile front seat passengers. Injury pattern and incidence in relation to accident mechanism].

During 1985 and 1995, 3,260 restrained car drivers (1,228 front seat passengers) were injured in 9,380 crashes involving cars. 179 (5.5%) (front seat passengers: 53, 4.3%) of those sustained fractures, of the arm. Among the 384 single fractures the hand (25%), wrist (23%) and forearm (23%) were affected most often, elbow (9%), upper arm (10%) and shoulder (10%) rather seldom. Half of the evaluated fractures of the long tubular bones were type A-fractures due to the AO classification and the other two quarters, respectively type B- and C-injuries in each case. 16% were open fractures. Humeral shaft fracture sustained during arm wrestling: report on 30 cases and review of the literature.

Thirty cases of humeral shaft fracture sustained during arm wrestling were analyzed. Twenty eight were men and two were women. Eighteen were fractures of the right arm and 12 were of the left. The patients ranged in age from 15 to 60 years old, averaging 30 years. Although there was no prominent tendency for the injuries to occur at a certain phase of the match, they happened in all cases while full force was being exerted in an effort to win or to change the tide of the contest. All cases were spiral fractures, of which 23% presented with a medial butterfly fragment, and also 23% of which had

Isolated infraspinatus atrophy in a collegiate volleyball player.

PURPOSE: To describe a case of isolated infraspinatus atrophy due to suprascapular nerve entrapment by a labral ganglion cyst in a volleyball player and to propose a mechanism for its formation. CASE SUMMARY: A female volleyball player was evaluated for shoulder pain and found to have atrophy of the infraspinatus muscle. Magnetic resonance imaging (MRI) showed a loculated ganglion cyst impinging on the suprascapular nerve. DISCUSSION: Several etiologies for isolated infraspinatus atrophy in volleyball players are cited in the literature. It is suggested here that the rapid deceleration in th

Intramedullary nailing of humeral shaft fractures.

Twenty-one patients with 22 acute traumatic fractures of the humeral diaphysis were treated either with the True/Flex intramedullary rod or the Russell-Taylor rod. Follow-up is provided on 20 fractures. Indications for fixation on all but 3 patients was either multiple fractures, associated chest or abdominal trauma, vascular interruption, or open fracture. The series included 4 open fractures. Time to union averaged 7.6 weeks, with two nonunions, both in open fractures. Two isolated preoperative radial nerve palsies resolved fully; four brachial plexus injuries had a more complex recovery. No

[Acute myoglobinuric kidney failure as a consequence of autoaggressive behavior in mental retardation].

A 49-year-old female with mental retardation was admitted with suspected renal insufficiency with a raised creatinine (5.1 mg/dl), hyperkalaemia (5.6 mmol/l), and a 12-hour history of diffuse abdominal pain and persistent vomiting. On admission, she had a haematoma around the right shoulder and arm-pit, swelling of the right upper-arm, and severe limitation of movement of the right hand. These injuries were the result of trauma some 5 days previously. She was a long-term inpatient in a psychiatric clinic, with a history of autoaggressive behaviour, which had led to several fractures in the pas

[Traumatic lesions caused by the "air-bag" system].

Hand injuries have occurred due to inflation of an air bag security system during a high velocity car accident. A driver and a passenger retained by three-point lap-shoulder belts crashed on the central railing of a motorway during their vehicle lost of control due to aquaplaning. The air bags inflated during the collision. The driver presented a superficial second degree burn with a 12 cm long blister on the radial side of the hand and distal forearm. The passenger suffered an intra-articular palmar avulsion fracture of the middle phalanx (PIP joint) and a mallet finger of the left fifth digi

[Remarkable findings in the criminal dismemberment of a corpse].

The objects of examination were two legs and two arms that were recovered at different times from waters traveled on by motor ships. The right leg was exarticulated at the hip joint, as was the right arm at the shoulder joint. Identification was complicated since both thumbs were exarticulated at the basal joints; the fingers II to V, respectively, were partially amputated. Identity (57-year-old vocational school teacher) could be established nevertheless by a number of anatomic findings (such as Dupuytren's contracture, coxitis, hallux rigidus), the blood group "O" was tatooed on the inner si
