Recent reports have suggested that flow volume curve abnormalities may be of interest in the diagnosis of obstructive sleep apnea syndromes by showing either extrathoracic airway obstruction (ratio of expiratory flow to inspiratory flow at 50 percent of forced vital capacity [FEF50/FIF50] exceeding 1) or upper airway fluttering (indicated by a sawtooth aspect on the mid-half of the inspiratory part of the curve) or both. In our study, 57 patients referred for a suspected sleep apnea syndrome (SAS) underwent conventional spirometry, assessment of flow-volume curves, ENT examination, and polysom…

This study investigated the prolonged effects on state behavior of theophylline administered to infants for apnea of prematurity. There were three groups: Four premature infants who had received theophylline in the preterm period, five premature infants who had not received theophylline, and twenty-eight normal fullterm infants. The Theophylline infants had been off the drug for at least one month prior to the beginning of the study. Sleep-wake states were observed in the home for seven-hour periods when all infants were the same corrected ages: two, three, four and five weeks post-term. Data …

Multiple Sleep Latency Test (MSLT) findings in a case of Kleine-Levin syndrome are reported for the first time. MSLT data indicate sleepiness as severe as in narcolepsy or obstructive sleep apnea and the occurrence of four sleep onset rapid eye movement (REM) periods, with a greater REM propensity at 2:00 p.m. and 4:00 p.m. than at 10:00 a.m. and 12:00 noon. The replication of such findings might suggest that Kleine-Levin syndrome could be considered a form of periodic REM sleep disinhibition. Therefore, the traditional hypothesis of diencephalic dysfunction may require modification to include…

A patient is described who developed apnea, hypertention, and tachycardia following the excision of bilateral carotid body tumors. Hypertension and tachycardia resulted, most probably, from bilateral ablation of the carotid sinus and/or the nerve of Hering. These phenomena have been described in the literature. The pathogenesis of the apneic attacks may be related to the ablation of the peripheral sensory organs (both carotid bodies), interference with the function of the respiratory automatic control system, and impaired control of ventilation during non-REM sleep. Temporary unilateral paraly…

To assess the relationship of abnormal flow-volume curves during awake periods to the clinical severity of sleep-disordered breathing and the need for surgical treatment of obstructive sleep apnoea, flow-volume curves were measured in 72 adults with obstructive apnoea. Patients in whom surgery was recommended for standard clinical indications had significantly lower inspiratory flow rates (p less than 0.01) and a higher incidence of flow-volume curves indicating extrathoracic airway obstruction (p less than 0.01) than did non-surgical patients. These abnormal flow-volume curves correlated with…

Midazolam in doses of 0.21-0.46 mg/kg was given to four insomniacs for 27 days, and to a fifth insomniac for an additional 124 days, to assess its short, intermediate, long-term and long-standing efficacy. Automatic analysis of the polygraphic sleep recordings was carried out employing real-time signal processing (based on linear prediction), and an off-line sequential fuzzy clustering. Total sleep length did not vary significantly during medication, whereas other efficacy parameters such as sleep onset latency, wake time after sleep onset and number of nightly awakenings decreased significant…

Employment which requires frequent shift rotation may lead to the development of specific sleep disorders. Delayed sleep phase (DSP) insomnia, a syndrome identified recently, can greatly impair an individual's circadian rhythm-dependent functions; it can occur when shift work disrupts normal sleep-waking schedules. Disorders of excessive daytime sleepiness, such as narcolepsy, occur in some subjects after they have been subjected to frequently rotating shifts. Understanding the problems associated with circadian rhythm disturbances and their interaction with sleep disorders is particularly imp…

In 3 patients with the syndrome of Unverricht-Lundborg's myoclonus-epilepsy electropolygraphic examinations were carried out during night sleep. A substantial disorganization of the sleep was revealed, the degree of which correlated with the gravity of the clinical picture. This disorganization manifested itself in pronounced changes of the electroencephalographic phenomenology of the sleep and disturbance of its structure with a prolongation of the 1st stage and the delta-sleep and a shortening of the 2nd stage and the quick sleep phase. An insufficient EEG desynchronization was noted in the …

Body movements during sleep were observed in 14 full-term sick newborn infants such as perinatal asphyxia, purulent meningitis, meconium aspiration syndrome, melena, porencephaly, and hydranencephaly. Five newborn infants who had recovered from transient vomiting were examined as control. Infants with minimally depressed background EEG showed an increase of generalized movement (GM) and localized tonic movement (LTM) in quiet sleep. Hydranencephalic infants revealed an increase of generalized phasic movement (GPM) in active and quiet sleep. On the other hand, a decrease or an absence of body m…

Body movements (BMs) during night sleep of 25 neurologically normal infants, 11 premature and 14 full-term, whose ages ranged from 30 conceptional weeks to 18 months post-term, were examined to evaluate the changes of their features with age. The examinations performed during sleep periods totaled 65 times, 1 to 60 times on each subject. Through visual observation and EEG recordings, the BMs were classified into 3 types: (1) Gross movements (GM), (2) localized movements (LM), both of the above two lasting more than 0.5 second, and (3) twitch movements (TM) lasting less than 0.5 second. Total G…

Improved case identification of children with upper airway obstruction during sleep should result if physicians are aware of such signs and symptoms as excessive daytime sleepiness, loud snoring, restless sleep, recurrent nocturnal enuresis, systemic and pulmonary hypertension, undergrowth or obesity, and cor pulmonale. Furthermore, partial airway obstruction during wakefulness may be a risk factor for the development of sleep apneas or hypopneas. In suspected cases, polysomnography is a useful method for confirming and quantitating the type (central, obstructive, or mixed) and extent of venti…

Sleep-wake cycles are closely tied with the circadian period of other bodily functions. Optimal synchronization is essential for troublefree alterations of sleep and wake periods. The neurophysiologic control areas are in various of the brain are far apart from each other for which reason a great variety of possible causes for sleep difficulties do exist. It has been shown recently that spezific mono-aminergic and cholinergic systems in the brain stem do control sleep. A case study will show that it is possible to improve sleeping difficulties with the serotonin precursor L-Troyptophan.

A 67-year-old woman with acquired micrognathia developed severe daytime hypersomnia, loud snoring, nocturnal enuresis, encopresis, and hypertension. A polysomnogram demonstrated 564 sleep apneas, primarily obstructive, recurrent hypoxia, a bradytachycardia, and absent stages III, IV, and REM sleep. Endoscopy during sleep revealed recurrent active closure of the upper pharynx associated with loud snoring. A tracheoplasty was done because of severity of symptoms and failure of conservative therapy. Dramatic improvement in sleepiness and hypertension occurred within 48 hours. On postoperative nig…

This report is based on a study of 53 infants, 28 of whom had clinically observed prolonged apnea (greater than or equal to 20 seconds) during sleep whereas the remaining did not. In addition to the clinical observations, each infant was studied in a sleep laboratory during a complete nap, and a continuous recording was made of respiratory activity and rapid eye movements. Measurements were made of all apneic pauses (greater than or equal to 2 seconds) observed in the laboratory and the two groups of infants were compared in terms of the frequency and average duration of apneic pauses, the lon…

Respiratory pattern and heart rate were studied in 28 pre-term infants and four full-term infants during the first weeks of life. Sleep state was identified by standard neurological and EEG criteria. Apneic spells lasting 10 seconds or more were most frequent in the least mature infants, and their frequency tended to diminish with increasing maturity. When these apneic attacks were related to sleep state they were found to occur more frequently during non-REM sleep in infants of 30 to 33 weeks gestation. This was the only statistically significant difference found. No over-all pattern relating…
