Sleep in patients with restrictive lung disease.

Restrictive lung disease leads to ventilatory defects and diffusion impairments. These changes may contribute to abnormal nocturnal pathophysiology, including sleep architecture disruption and impaired ventilation and oxygenation. Patients with restrictive lung disease may suffer significant daytime fatigue and dysfunction. Hypercarbia and hypoxemia during sleep may impact progression of lung disease and related symptoms. Little is known about the impact of treatment of sleep disruption on sleep quality and overall prognosis in restrictive lung disease. This review discusses the pathophysiolog

A prospective questionnaire study in 100 healthy sleepers: non-bothersome forms of recognizable sleep disorders are still present.

STUDY OBJECTIVES: Despite several polysomnographic normative studies and multiple surveys of sleep disorders in the general population, few data have been collected on healthy sleepers. We aimed to survey the characteristics of healthy sleep. METHODS: We prospectively investigated the sleep history of 100 subjects of a representative population sample who reported undisturbed sleep and in whom relevant sleep disorders were ruled out by a two-step screening procedure. Approximately four subjects had to be contacted for identifying 1 eligible subject who participated. RESULTS: The median repor

Does self-reported sleep quality predict poor cognitive performance among elderly living in elderly homes?

OBJECTIVES: Sleep complaints are common among elderly, especially institutionalized elderly, as they experience poorer sleep quality and higher use of sedative hypnotics, when compared to community-dwelling elderly. Recent findings suggest that there may be a relationship between poor quality of sleep and cognitive deficits. This study aimed at studying the relation between sleep quality and cognitive performance in older adults living in elderly homes. METHOD: 100 elderly living in an elderly home in El Mansoura, Egypt, were recruited in this study, 50 cases with subjective poor quality of s

Ornithine ingestion improved sleep disturbances but was not associated with correction of blood tryptophan ratio in Japanese Antarctica expedition members during summer.

Members of expeditions to Antarctica may show changes in biological and physiological parameters involved in lipid, glucose, and thyroid hormone metabolism as they adapt to the environment; however, alterations in amino acid (AA) levels and sleep among expedition members in Antarctica have yet to be fully elucidated. We hypothesized that there would be alterations of blood AA levels, and ornithine (Orn) ingestion would affect biological parameters and sleep in Japanese expedition members during the summer in Antarctica. Japanese Antarctica Research Expedition members (22 people) who stayed in

State-trait arousal and daytime sleepiness after sleep restriction.

The importance of an arousal system in the regulation of sleepiness has been widely recognised in contexts of insomnia theory and research. Arousal is also incorporated in some general models of sleepiness and is considered one of the principal factors regulating sleepiness in a model by De Valck and Cluydts (2003), in which arousal has both state and trait components. In the present experimental study, we explored the effects of state and trait components of arousal on subjective sleepiness and sleep latency during daytime. On a day after partial sleep deprivation, 28 good sleepers aged 18-26

Association of incident cardiovascular disease with periodic limb movements during sleep in older men: outcomes of sleep disorders in older men (MrOS) study.

BACKGROUND: Periodic limb movements during sleep (PLMS) cause repetitive sympathetic activation and may be associated with increased cardiovascular risk. We hypothesized that PLMS frequency (periodic limb movement index [PLMI]) and PLMS arousal frequency (periodic limb movement arousal index [PLMAI]) are predictive of incident cardiovascular disease, including coronary heart disease, peripheral arterial disease, and cerebrovascular disease, in an elderly male cohort. METHODS AND RESULTS: A total of 2911 men in the observational Outcomes of Sleep Disorders in Older Men (MrOS) Sleep Study cohor

Sleep duration is an independent factor in nocturia: analysis of bladder diaries.

OBJECTIVE To analyse the relationship between the number of nocturia episodes and other variables obtained from a bladder diary, with special attention to the duration of sleep, as nocturia is closely related to sleep disorders. PATIENTS, SUBJECTS AND METHODS Between April 2005 and December 2006, 532 participants, including both community-dwelling subjects who underwent a mass screening programme and outpatients who consulted the department of urology, completed a 3-day bladder diary. Clinical variables, i.e. the number of daytime and night-time voids, diurnal and nocturnal urine volume, dayti

Use of computerized visual performance test in assessing day-time vigilance in patients with sleep apneas and restless sleep.

We have studied day-time vigilance in 31 patients (median age 49 years) with suspected sleep disorders using a new visual reaction time and performance test. The findings in the day-time vigilance test were compared with the number of desaturation events and movement arousals measured with a sensitive movement detector in the night-time. In our statistical model the high number of desaturations correlated with a high dispersion in reaction-times. The squared multiple r was 0.465 in a model where the dispersion of reaction times was the dependent variable and the number of desaturations, durati

A multicentre hospital study to compare the hypnotic efficacy of loprazolam and nitrazepam.

A multicentre, parallel group hospital study was carried out in 190 subjects with insomnia to compare the efficacy, incidence of hangover and the side-effects of loprazolam and nitrazepam. Following 2 nights single-blind phase on placebo, loprazolam (1.0 mg), nitrazepam (5.0 mg) or placebo was administered double-blind for 7 consecutive nights. Visual analogue scales and questions were used to rate efficacy. There was no statistically significant difference between loprazolam and nitrazepam for 'ease of getting to sleep', 'restfulness of sleep' and 'depth of sleep'. Like nitrazepam, loprazolam

Findings of the Maintenance of Wakefulness Test and its relationship with response to modafinil therapy for residual excessive daytime sleepiness in obstructive sleep apnea patients adequately treated with nasal continuous positive airway pressure.

OBJECTIVE: We aimed to examine the relationship between subjective and objective sleepiness in obstructive sleep apnea syndrome (OSAS) patients with residual sleepiness, and to determine whether baseline objective sleepiness severity predicts the response to modafinil therapy. METHODS: Data were obtained from a randomized, placebo-controlled modafinil (200 mg/day) study in Japanese OSAS patients with residual sleepiness receiving nasal continuous positive pressure (n-CPAP) treatment. We analyzed 50 participants whose subjective (Epworth Sleepiness Scale [ESS] total score) and objective (Maint

Characteristics and circadian distribution of cardiac arrhythmias in patients with heart failure and sleep-disordered breathing.

BACKGROUND: Cardiac arrhythmias and sleep-disordered breathing (SDB) are common comorbidities in heart failure with reduced ejection fraction (HFrEF). However, understanding of the association between arrhythmias and SDB is poor. This study assessed the occurrence and circadian distribution of ventricular arrhythmias in HFrEF patients with and without SDB. METHODS: This retrospective analysis included HFrEF patients admitted for unattended overnight cardiorespiratory polygraphy and 24-h Holter-ECG recording. Holter-ECG data (events/h) were categorized by time of day: morning, 06:00-13:59; aft

Abnormalities of social interactions and home-cage behavior in a mouse model of Rett syndrome.

Rett syndrome (RTT) is an autistic spectrum disorder with a known genetic basis. RTT is caused by loss of function mutations in the X-linked gene MECP2 and is characterized by loss of acquired motor, social and language skills in females beginning at 6-18 months of age. MECP2 mutations also cause non-syndromic mental retardation in males and females, and abnormalities of MeCP2 expression in the brain have been found in autistic spectrum disorders. We studied home-cage behavior and social interactions in a mouse model of RTT (Mecp2(308/Y)) carrying a mutation similar to common RTT causing allel

Unraveling the pathophysiology of narcolepsy type 1 through hypothesis-driven and hypothesis-generating approaches.

Narcolepsy type 1 (NT1) is a chronic orphan neurological sleep disorder characterized by the loss of hypocretin-producing neurons in the lateral hypothalamus, which play a crucial role in wakefulness. Given the genetic association with the HLA-DQB1 * 06:02 allele and environmental links with the 2009 influenza pandemic, many lines of evidence point towards an immune mechanism, notably autoimmunity, underlying the disease pathophysiology. Autoreactive T cells are found in the blood of NT1 patients, and mouse models demonstrate their migratory capacity and contribution in the selective destructi

Frequent epileptic apnoea in a patient with Pitt-Hopkins syndrome.

Pitt-Hopkins syndrome is a rare genetic disease, characterised by severe intellectual disability, distinctive dysmorphic features, epilepsy and distinctive breathing abnormalities during wakefulness. Here, we describe the case of a 22-year-old woman with Pitt-Hopkins syndrome who presented with intractable generalised tonic seizures from the age of 11 years, which increased in frequency with age and onset of menstruation despite usage of some anticonvulsant drugs. From the age of 16 years, polysomnography and video-EEG led to the detection of frequent epileptic apnoea during sleep. Although th

Treatment of behavioral and psychological symptoms of dementias with psychopharmaceuticals: a review.

Behavioral and psychological symptoms represent common complications in patients with different types of dementia. Predominantly, they comprise psychosis, agitation and mood disorders, disinhibited behavior, impairment of the sleep and wakefulness rhythm, wandering, perseveration, pathological collecting, or shouting. Their appearance is related to more rapid progression of the disease, earlier institutionalization, use of physical restraints, and higher risk of mortality. Consequently, appearance of behavioral and psychological symptoms of dementia leads to higher costs of care provided and g

[EEG changes in the course of progressive cerebral disorders in children (author's transl)].

This survey presents EEG-data on the following disorders: Globoid-cell leucodystrophy (Krabbe), Metachromatic Leukodystrophy, GM2 Gangliosidosis I (Tay-Sachs), Neuronal Ceroidlipofuscinosis. During Krabbe's disease the background activity of the EEG slows down, multifocal and generalized hypersynchroneous activity (HSA) appears, in the final stage hypsarrythmia is found occasionally. In contrast HSA during Sulfatide lipidosis appears only in the final stages. The EEG changes in Tay Sachs disease are minimal in the beginning; later only high amplitude slowing and especially focal abnormalities

Comprehensive Health Predictors of Injury, Illness, and Separation From Service After Enrollment in Special Forces Assessment and Selection: A Two-Year Longitudinal Study.

Individuals in physically and cognitively demanding professions face increased risk of injury, illness, and attrition due to repeated exposure to physical and psychological stressors. Identifying modifiable factors associated with these outcomes can guide risk mitigation strategies. This longitudinal study examined the 2-year cumulative incidence of injuries, illnesses, and attrition following enrollment in the United States Army Special Forces Assessment and Selection (SFAS) course (n = 792) and assessed how physical, nutritional, cognitive, sleep, and social support at the time of enrollment

Neurodevelopment as a shared genetic etiology linking sleep-related phenotypes and psychiatric disorders: a genome-wide pleiotropic analysis.

Comorbidity and mutual transformation between psychiatric disorders (including autism spectrum disorder, attention-deficit/hyperactivity disorder, bipolar disorder, post-traumatic stress disorder, major depressive disorder, obsessive-compulsive disorder, schizophrenia, and anxiety disorders) and sleep disorders are common, with circadian rhythm disruption considered a potential biological basis, and neurodevelopmental abnormalities seemingly playing a key role. However, the extent to which shared genetic determinants contribute to these associations remains unclear. Extensive genetic correlati
