BACKGROUND: The safety and feasibility of sterile, acellular pulley allografts in reconstruction has been previously demonstrated. Comparisons with tendon-based techniques for pulley reconstruction have not been reported. We hypothesized that the use of allograft pulleys would result in reduced procedural time and equivalent clinical outcomes as compared with traditional tendon-based reconstructive techniques.
METHODS: All cases of pulley reconstruction using either allograft pulleys or tendon-based pulley reconstruction between November 2013 and November 2015 were reviewed. Patients who unde

Achilles tendon ruptures are common in the general population, especially among members of the older demographic occasionally active in sports. Operative treatments provide a lower incidence of rerupture than do nonoperative treatments, although surgical complications remain a concern. The use of a human acellular dermal matrix to augment Achilles tendon repair might reduce the incidence of complications. In the present case series, we describe the outcomes of 9 patients who underwent Achilles tendon repair with acellular dermal matrix augmentation. Functional outcomes were evaluated using the

BACKGROUND: Acute Achilles tendon rupture (AATR) is a frequent injury occurring dominantly in young to middle-aged males. Outcomes and complications between percutaneous and open repair are still controversial. Thus, the purpose of this meta-analysis is to evaluate the outcomes and complications of these two operative methods.
MATERIALS AND METHODS: We searched multiple databases: PubMed, Web of Science, EMBASE and the Cochrane Library up to October 2016. Two reviewers independently screened the studies for eligibility, evaluated the quality and extracted data from eligible studies, with conf

In tendon rupture repair, improvements such as higher primary repair strength, anti-adhesion and accelerated healing are needed. We developed a potential carrier system of an electrospun DegraPol tube, which was tightly implanted around a transected and conventionally sutured rabbit Achilles tendon. Histomorphometric analysis of the tendon tissue 12 weeks postoperation showed that the tenocyte density, tenocyte morphology and number of inflammation zones were statistically equivalent, whether or not DegraPol tube was implanted; only the collagen fibres were slightly less parallelly orientated

OBJECTIVE: To develop and validate a histologic scoring system for the assessment of tendon graft to bone tunnel healing in anterior cruciate ligament (ACL) reconstruction.
STUDY DESIGN: The scoring system, tendon-bone tunnel healing (TBTH) score, comprised 5 items on graft status, fiber type and interface connectivity, evaluated on either a 5- or 6-point scale. Two observers were trained to use the scoring system, examining 15 blinded histologic slides from an ongoing study. Afterward, an independent validation dataset consisting of 89 blinded histologic slides from the same study of differen

BACKGROUND: Tendo-Achilles lengthening (TAL) and gastrocnemius recession (GR) are components of the operative treatment for certain foot conditions. We investigated changes in rabbit gastrocsoleus weight, volume, and percent fat resulting from TAL, GR, and immobilization.
MATERIALS AND METHODS: Eighteen rabbits were randomized into three groups. Group I = gastrocnemius recession; Group II = (TAL) Z-plasty lengthening; Group III = immobilization only. Treatment limbs were randomized. Six randomly selected contralateral limbs were used for controls. At 12 weeks the gastrocnemius and soleus were

BACKGROUND: Clinical and experimental studies have demonstrated that restrictive adhesions and poor digital motion are common complications after extrasynovial tendon grafting in an intrasynovial environment. The purpose of this study was to test the hypothesis that surface modification of an extrasynovial tendon with use of a carbodiimide-derivatized hyaluronic acid-gelatin polymer (cd-HA) improves gliding ability and digital function after tendon grafting in a canine model in vivo.
METHODS: The peroneus longus tendons from both hindpaws of twenty-four dogs were harvested and transplanted to

A tendon subluxation test was performed on the unaffected side in 13 patients with traumatic extensor tendon dislocation of the middle finger and on both middle fingers in 800 healthy controls to identify extensor mechanism laxity at the metacarpophalangeal joint. Ten of the 13 patients with dislocation had laxity of middle finger extensor tendon, compared with 174 of the 800 (22%) controls. The difference in these rates of extensor tendon laxity is significant (P<0.0001), and suggests that extensor mechanism laxity at the metacarpophalangeal joint may predispose to traumatic extensor tendon d

BACKGROUND: By arthroscopy, we observed a phenomenon that, according to our knowledge not previously described, we call the "biceps tendon footprint" (BTF)--an area of chondromalacia beside the bicipital groove.
PATIENTS AND METHODS: We studied 118 shoulder arthroscopies prospectively. We documented whether a BTF could be observed and what the main pathology associated with it was. We used 3 grades of cartilage wear to describe BTF, and we analyzed pathological changes in associated structures (subscapularis, biceps tendon and humeral head).
RESULTS: We found a BTF in 16% of the cases. Assoc

The use of bone-patellar tendon-bone allograft or autograft in anterior cruciate ligament (ACL) reconstruction is an accepted method of repair of the ACL-deficient knee with few complications. We report an unusual complication associated with this technique, the development of synovial fistula and cyst associated with the use of nonabsorbable suture material.

We studied the vascular pattern of human peroneal tendons with injection techniques and immunohistochemically by using antibodies against laminin The main blood supply arises from the peroneal artery. The distal part of the peroneus longus tendon is supplied by branches of the medial tarsal artery. Blood vessels enter the peritenon of both tendons via a mesotenon from the posterior aspect. From the peritenon, the blood vessels penetrate the peroneus brevis and peroneus longus tendons and anastomose with a longitudinally-oriented intratendinous network. The amount of vessels in the tendon subst

The aim of this study was to assess long-term results of extensor indicis (EI) to extensor pollicis longus (EPL) transfers and to assess donor site morbidity. A specific EI-EPL evaluation method (SEEM) was used to measure EPL function after transfer. The outcomes in 17 patients are presented. Results were assessed by the Geldmacher score, the SEEM, mobility and strength of thumb and index finger, pinch and grip strength, and a questionnaire, comparing the operated and non-operated hands. Based on the SEEM, the results were excellent to good in 11 of 17 patients. There was no marked loss of ind

Pain over the lateral aspect of the hip commonly is attributed to trochanteric bursitis. Typical findings include local tenderness and weakness of hip abduction. When conservative measures fail to relieve symptoms, surgical release of the iliotibial band over the greater trochanter has been recommended. In the management of seven such patients, an unusual finding was encountered: partial tear of the gluteus medius tendon at its attachment to the greater trochanter. Each patient presented with increasing hip pain of duration of months to years. There were no diagnostic findings on physical exam

Abnormalities of the long tendon of the biceps brachii muscle commonly accompany other lesions about the shoulder, especially rotator cuff rupture, and are a frequent cause of a painful shoulder. The spectrum of abnormalities associated with medial displacement of the biceps tendon seen at magnetic resonance (MR) imaging was reviewed in six patients. MR imaging was performed on a 1.5-T system with use of a dedicated surface coil. The findings at MR imaging were correlated with those at surgery. Two types of medial displacement of the biceps tendon were identified. More frequently observed was

Ankle pain can be disabling for the geriatric patient. Unlike symptoms in younger individuals, symptoms in the elderly are often insidious in onset and result from chronic disease processes. Acute injuries, when they occur, are often managed differently than in younger patients. Accurate diagnosis is essential for appropriate treatment. An anatomic approach to diagnosis is presented along with guidelines for appropriate conservative and surgical treatment.

Based on 4 long time observations of syringomyelitic arthropathy and the literature, the clinical and roentgenological picture, the differential diagnosis, the course and the limited chances of treatment are discussed. As to diagnosis initial shoulder pain, pseudophlegmoneous swelling of the shoulder, rupture of the long tendon of the biceps muscle and bone appositions at the medialcaudal portion of the humeral neck-head area and at the lower margin of the acetabulum are emphasized as well as small destructions of the greater tubercle, prior to extensive bone resorption. We present an original technique for determining the tension of the extensor pollicis longus tendon after reconstruction. We treated 20 patients using this technique for an extensor pollicis longus tendon graft or extensor indicis tendon transfer and reviewed the results. The tension of the reconstructed extensor pollicis longus was adjusted so that the centre of the distal edge of the thumbnail was elevated 2 cm above the operation table. The mean retropositional distance of the treated thumbs was 1.2 cm less than the contralateral thumbs. The mean total active motion of the thumb was 90%. Th
