Volar plating of distal radius fractures does not restore the anatomy.

INTRODUCTION: The objective of this study was to evaluate whether volar locking plating can restore the anatomical volar tilt of 12° and the anatomical length of the radius relative to the ulna. We retrospectively reviewed the radio-graphic outcome of distal radius fractures treated with two different first-generation locking plate systems; VariAx (Stryker) and Acu-Loc (Acumed). METHODS: Three independent observers evaluated radial volar tilt and ulnar variance (> 2 mm) on radiographs taken preoperatively, immediately after the operation and five weeks later. In addition, the radial inclinati

Pulmonary contusion in the pan-scan era.

BACKGROUND: Although pulmonary contusion (PC) is traditionally considered a major injury requiring intensive monitoring, more frequent detection by chest CT in blunt trauma evaluation may diagnose clinically irrelevant PC. OBJECTIVES: We sought to determine (1) the frequency of PC diagnosis by chest CT versus chest X-ray (CXR), (2) the frequency of PC-associated thoracic injuries, and (3) PC patient clinical outcomes (mortality, length of stay [LOS], and need for mechanical ventilation), considering patients with PC seen on chest CT only (SOCTO) and isolated PC (PC without other thoracic inju

CT-detected traumatic small artery extremity injuries: surgery, embolize, or watch? A 10-year experience.

Advances in computed tomography (CT) angiography have increased the sensitivity and specificity of detecting small branch arterial injuries in the extremities of trauma patients. However, it is unclear whether these patients should undergo surgery, angioembolization, or conservative watchful waiting. We hypothesized that uncomplicated small arterial branch injuries can be managed successfully with watchful waiting. A 10-year retrospective review of extremity CT angiograms with search findings of arterial "active extravasation" or "pseudoaneurysm" was performed at a level 1 county trauma center

Vacuum-assisted closure device as a split-thickness skin graft bolster in the burn population.

The vacuum-assisted closure device (VAC) is associated with improved wound healing outcomes. Its use as a bolster device to secure a split-thickness skin graft has been previously demonstrated; however, there is little published evidence demonstrating its benefits specifically in the burn population. With use of the VAC becoming more commonplace, its effect on skin graft take and overall time to healing in burn patients deserves further investigation. Retrospective review of burn registry database at a high-volume level I trauma center and regional burn center during a 16-month period was perf

Mortality increases with recurrent episodes of nonaccidental trauma in children.

BACKGROUND: Nonaccidental trauma (NAT) is a leading cause of childhood traumatic injury and death. Our objectives were to compare the mortality rates of children who experience recurrent episodes of NAT (rNAT) with children who experience a single episode of NAT and to identify factors associated with rNAT and increased mortality from rNAT. METHODS: Patients of NAT and rNAT in the Ohio State Trauma Registry were identified by matching date of birth, race, and sex between records of patients younger than 16 years between 2000 and 2010 with an DRG International Classification of Diseases--9th R

Blunt cerebrovascular injuries in acute trauma care: a screening protocol.

INTRODUCTION: Blunt cerebrovascular injuries (BCVI) of the extra- or intracerebral vessels are frequently observed lesions which may lead to thrombembolic events with focal neurological deficits, stroke or death particularly in patients <60 years. However, a comprehensive standardised clinical algorithm for screening and management of these secondary injuries is still lacking. MATERIALS AND METHODS: We developed a standardised screening protocol applicable for mild as well as severely injured patients. In this prospective cohort study, we evaluated the feasibility of this diagnostic algorithm

High mortality after emergency room laparotomy in haemodynamically unstable trauma patients.

INTRODUCTION: Hypovolaemic shock is a major course of death in trauma patients. The mortality in patients in profound shock at the time of arrival is extremely high and we wanted to investigate the outcome of patients undergoing laparotomy at the Trauma Care Unit (TCU). MATERIAL AND METHODS: Forty-four emergency laparotomies performed at the TCU at Rigshospitalet between January 2003 and December 2009 were registered. The indication for surgical intervention was based on persisting, unstable haemodynamics and either positive findings at focused abdominal sonography in trauma (FAST) or penetra

Battlefield trauma, traumatic shock and consequences: war-related advances in critical care.

Over the course of history, while the underlying causes for wars have remained few, mechanisms of inflicting injury and our ability to treat the consequent wounds have dramatically changed. Success rates in treating war-related injuries have improved greatly, although the course of progress has not proceeded linearly. From Homer's Iliad to the Civil War to Vietnam, there have been significant improvements in mortality, despite a concurrent increase in the lethality of weapons. These improvements have occurred primarily as a result of progress in three key areas: management of wounds, treatment

[Special features and possible pitfalls in paediatric trauma care].

Paediatric trauma care represents treatment of a dynamic and growing system. Following evaluation of fracture stability, the individual spontaneous correction can be included. Possible pitfalls can occur with diagnosis, indication, choice of the treatment modality, surgical technique and follow-up. Treatment in a specialised paediatric trauma centre is not always possible and is not necessary in all childhood injuries. But the paediatric trauma centres have to set standards, to show problems, to investigate alternatives, to present solutions and to secure a child-appropriate treatment by compe

Bedside laparotomy for trauma: are there risks?

BACKGROUND: Critically ill trauma patients are often too unstable for safe transfer to the operating room. Damage control laparotomy patients frequently require early reoperation and have a reported mortality of 50-60%. As a result, many of these patients must undergo laparotomy in the intensive care unit. We hypothesized that patients undergoing bedside laparotomy (BSL) and managed with the abdomen left open would have an unacceptably high mortality or intra-abdominal complications. METHODS: We performed a retrospective chart review of our Trauma Registry. Of the 11,096 consecutive trauma ad

Host Susceptibility Phenotypes and Construction of an Inflammation-Vascular-Metabolic (IVM) Integrated Score for Hard-to-Heal Wounds After Low-Energy Foot Trauma in Older Adults.

BACKGROUND: Older adults with low-energy trauma-initiated foot wounds often become hard-to-heal, but prognostic tools tailored to this scenario are scarce. METHODS: In this single-center retrospective cohort, we included consecutive patients aged ≥65 years with below-malleoli low-energy trauma-initiated foot wounds. Baseline predictors were the first laboratory values within 48 h and an ordinal perfusion/ischemia category derived from available ABI, pedal Doppler waveform findings, and toe-based measures. Hard-to-heal was failure of complete epithelialization without drainage by 12 weeks (maj

Complication profile and surgical trends in hemiarthoplasty for femoral neck fractures: analysis from a national referral hospital in Ethiopia.

BACKGROUND: Hemiarthroplasty is one of the standard treatment options for displaced femoral neck fractures in the elderly. While surgical approaches, implant choices, and complication profiles are well-documented in high-income countries, there is a critical lack of data from low-resource settings like Ethiopia, where system constraints and resource availability may significantly influence practice and outcomes. This study aimed to analyze the surgical trends, implant choices, and complication profiles of hemiarthroplasty at a major Ethiopian trauma center. METHODS: A retrospective cross-secti

MAP-Derived Shock Index for Point-of-Care Physiological Risk Stratification After CT-Confirmed Cervical Spine Fracture: Development and Internal Validation of a Prognostic Model for In-Hospital Mortality.

Background/Objectives: Early mortality risk stratification after CT-confirmed cervical spine fractures remains clinically challenging in patients with major trauma. Imaging establishes a structural diagnosis, but it does not fully capture early physiological deterioration. This study aimed to develop and internally validate a prognostic model for in-hospital mortality and evaluate whether a MAP-derived Shock Index provides incremental prognostic information beyond routinely available clinical variables and the Injury Severity Score (ISS). Methods: This retrospective single-center cohort study

The Dirty Truth About Resuscitative Endovascular Balloon Occlusion of the Aorta: A Descriptive Analysis of Infection Rates in a High-Risk Population.

Introduction: Resuscitative endovascular balloon occlusion of the aorta (REBOA) has become a viable alternative to open aortic occlusion for hemorrhage control. It is often performed without maximal barrier precautions, and sterility is not consistently ensured. As REBOA usage increases, a knowledge gap exists in its infectious risks. We sought to characterize the type and incidence of infectious complications in patients undergoing REBOA. Patients and Methods: A retrospective review of all REBOA patients at an urban, American College of Surgeons-verified Level I Trauma Center was conducted fr

Accuracy of National Early Warning Score 2 (NEWS2) in Prehospital Triage on In-Hospital Early Mortality: A Multi-Center Observational Prospective Cohort Study.

INTRODUCTION: In cases of mass-casualty incidents (MCIs), triage represents a fundamental tool for the management of and assistance to the wounded, which helps discriminate not only the priority of attention, but also the priority of referral to the most suitable center. HYPOTHESIS/PROBLEM: The objective of this study was to evaluate the capacity of different prehospital triage systems based on physiological parameters (Shock Index [SI], Glasgow-Age-Pressure Score [GAP], Revised Trauma Score [RTS], and National Early Warning Score 2 [NEWS2]) to predict early mortality (within 48 hours) from t
