Laparoscopic Transduodenal Ampullectomy: How We Have Standardized the Technique (with Video).

BACKGROUND: The procedure of choice for the resection of ampullary tumors comprises transduodenal ampullectomy (TDA), endoscopic papillectomy (EP), and pancreaticoduodenectomy (PD).1 For neoplasms with low-grade dysplasia, TDA and EP have equivalent efficacies and lower morbidities than PD1. Compared with EP, also as an organ-preserving procedure, TDA could be applicable for tumors involving the pancreatic ducts or common bile ducts.2 Because TDA has a lower incidence of postoperative gastrointestinal bleeding and a higher R0 resection rate, its use could avoid the need to use multiple endosco

Pediatric pleural empyema - decision making beyond imaging - a retrospective cohort study.

BACKGROUND/OBJECTIVES: In children and infants, thoracic empyema most often develops as a complication of parapneumonic pleural effusions progressing to purulent collections. With an estimated incidence of approximately 0.6% among pediatric pneumonia cases, empyema remains associated with significant morbidity. This study aimed to characterize pediatric parapneumonic empyema by analyzing clinical, biochemical, and radiological parameters and their relationship with treatment decisions and outcomes. MATERIALS AND METHODS: We conducted a retrospective, single-center study including children dia

Thoracoscopic right S1 + S3 segmentectomy for lung cancer with a displaced B2 bronchus: a case report.

BACKGROUND: Tracheobronchial branching anomalies are uncommon, but increase the risk of intraoperative vascular or bronchial injury if not recognized preoperatively. An independent origin of the right B2 bronchus from the bronchus intermedius is particularly rare and may be accompanied by complex pulmonary venous and arterial variations. Careful preoperative anatomical assessment is crucial when planning anatomical segmentectomy in such cases. CASE PRESENTATION: A 73-year-old woman was referred to our hospital after a 12-mm partially solid nodule with a cavity at the border between segments S

Forty years between contralateral bullectomies for giant pulmonary bullae: a case report.

BACKGROUND: Giant pulmonary bullae (GPB) are a rare manifestation of emphysema that often requires procedural intervention. Operative management of GPB has been shown to significantly improve quality of life and pulmonary function test (PFT) parameters. These improvements have been shown to persist for years, with a gradual decline associated with the progression of emphysema. For those with GPB who have had a pneumothorax, a future contralateral pneumothorax is more common in those with contralateral blebs. We present the unique case of a patient with GPB who required contralateral bullectomy

Long-term changes in psoas muscle mass after lobectomy and segmentectomy for early-stage lung cancer.

BACKGROUND: Segmentectomy is considered a less invasive procedure than lobectomy for patients with non-small cell lung cancer (NSCLC); however, little is known about the physiological mechanism underlying the lower invasiveness of segmentectomy. This study is aimed to compare the differences in the long-term changes in the psoas muscle mass after segmentectomy and lobectomy in patients with NSCLC. METHODS: Overall 315 recurrence-free patients who underwent segmentectomy (n = 93) or lobectomy (n = 222) for clinical stage 0-I NSCLC between January 2016 and December 2018 and underwent computed t

Demographic, Clinical Features and Outcome Determinants of Thoracic Trauma in Sri Lanka: A Multicentre Prospective Cohort Study.

Prognostic determinants in thoracic trauma are of major public health interest. We intended to describe patterns of thoracic trauma, demographic factors, clinical course, and predictors of outcome in selected tertiary care hospitals in Sri Lanka. A multicentre prospective cohort study was conducted in five leading teaching hospitals from June to September 2017. Patients with thoracic trauma were followed up during the hospital stay. A logistic regression analysis was conducted using in-hospital morbidity as the dichotomous outcome variable. One hundred seventy-one patients were included in the

Perioperative and oncological outcomes of single-port versus two-port thoracoscopic lobectomy for non-small cell lung cancer: a propensity score-matched retrospective study.

Although single-port video-assisted thoracoscopic surgery (VATS) lobectomy is increasingly adopted for non-small cell lung cancer (NSCLC), evidence comparing its long-term oncological efficacy with that of the two-port approach remains scarce. This propensity score-matched study evaluated perioperative outcomes, postoperative recovery, and 3-year survival following single-port versus two-port thoracoscopic lobectomy in patients with NSCLC. We retrospectively analyzed 167 consecutive patients (single-port, n = 82; two-port, n = 85); propensity score matching (1:1, caliper = 0.2) yielded 72 matc

Perioperative analgesic strategies for video-assisted thoracoscopic lobectomy: a systematic review and meta-analysis.

BACKGROUND: Video-assisted thoracoscopic surgery (VATS) lobectomy is the standard of care for early-stage lung cancer, yet 20-30% of patients still experience moderate-to-severe acute postoperative pain. A wide range of perioperative analgesic strategies-regional blocks, systemic adjuncts, cryoanalgesia, and multimodal combinations-have been evaluated, but their comparative effects on clinically meaningful endpoints such as postoperative complications remain uncertain. The aim of this systematic review and meta-analysis was to evaluate the comparative efficacy and safety of perioperative analg

Surgical outcomes and perioperative risk factors of patients with interstitial lung disease after pulmonary resection.

BACKGROUND: Patients of interstitial lung disease (ILD) combined with pulmonary lesions are increasingly common in clinical practice. Patients with ILD are at significantly higher risk for complications after pulmonary resection (including lobectomy and sublobar resection), especially acute exacerbations of ILD (AE-ILD). The purpose of this study is to summarize the short-term and long-term outcomes after pulmonary resection in ILD patients and to analyze the clinical factors affecting surgical safety. METHODS: From January 2004 to January 2022, a total of 78 patients who were diagnosed with

Role of routine computed tomography in paediatric pleural empyema.

BACKGROUND: The incidence of empyema in children is increasing worldwide. While there are emerging data for the best treatment options, there is little evidence to support the imaging modalities used to guide treatment, particularly with regard to the role of routine CT scanning. The aims of this study were to develop a radiological scoring system for paediatric empyema and to assess the utility of routine CT scanning in this disease. METHODS: Children with empyema were prospectively enrolled over a 3-year period into a randomised clinical trial of video-assisted thoracoscopic surgery versus

Primary lung cancer surgery after curative chemoradiotherapy for esophageal cancer patients.

The safety and perioperative problems of primary lung cancer surgery after curative chemoradiotherapy (CRT) for thoracic esophageal cancer (EC) are controversial. We retrospectively evaluated six patients who had received curative CRT for EC from 2003 to 2009, in whom the lung nodule was identified as a primary lung cancer and who subsequently underwent pulmonary resection. The treatment for EC consisted of chemotherapy with cisplatin and 5-fluorouracil with concurrent curative thoracic radiotherapy (60 Gy). The median age at the surgery was 75 years (range 69-80 years). The median time from r
