Conservative Management of Omental Infarction Using Only Analgesia.

BACKGROUND: Omental infarction (OI) is an uncommon and often misdiagnosed cause of acute abdominal pain. Through increasing use of computed tomography (CT) early identification has improved, avoiding unnecessary surgical intervention. CASE PRESENTATION: A 49-year-old woman presented with right-sided abdominal pain radiating to the epigastrium. Laboratory tests revealed elevated inflammatory markers while abdominal ultrasound was inconclusive. CT revealed a localized area of fat stranding in the right hypochondrium, consistent with OI. The patient was managed conservatively with analgesia and

Case Report: Necrotizing myositis secondary to a pararectal fistula: diagnostic challenges and multidisciplinary management.

BACKGROUND: Necrotizing soft tissue infections (NSTIs) are rare but life-threatening conditions requiring early diagnosis and aggressive multidisciplinary management. Necrotizing myositis (NM) is an uncommon and particularly severe manifestation, often associated with diagnostic delay and poor outcomes. CASE PRESENTATION: We report the case of a 57-year-old man presenting with mild pelvic pain radiating to the posterior thigh. Contrast-enhanced computed tomography revealed a presacral abscess with an extrasphincteric fistulous tract extending into the posterior thigh musculature. Surgical exp

Internal mammary artery perforator flap in the management of tracheoesophageal fistulae.

BACKGROUND: A pathological communication between the trachea and oesophagus - a tracheoesophageal fistula - may be congenital or acquired, benign or malignant, necessitating a multidisciplinary approach. Conservative attempts at closure of this abnormal connection are ineffective; the interposition of healthy vascular tissue offers the least chance of recurrence. METHODS: Outcomes of an islanded fasciocutaneous internal mammary artery perforator flap applied for tracheoesophageal fistula management were assessed in four radiated patients with laryngeal carcinoma using retrospective records.

What Should We Focus on When Managing Breast Cancer-Related Lymphedema to Improve Quality of Life?

Background: Breast cancer-related lymphedema (BCRL) is correlated with poor quality of life (QOL). The effects of the management of lymphedema on QOL remain controversial. We analyzed the changes in QOL and identified factors associated with its improvement to improve the care of patients with BCRL. Methods and Results: A total of 194 patients with BCRL were recruited, and their medical records were reviewed regarding type of surgery, chemotherapy, and radiation. Patients completed the European Organization for Research and Treatment of Cancer Quality of Life Questionnaires, and the volume and

A mechanistic mathematical model of initiation and malignant transformation in sporadic vestibular schwannoma.

BACKGROUND: A vestibular schwannoma (VS) is a relatively rare, benign tumour of the eighth cranial nerve, often involving alterations to the gene NF2. Previous mathematical models of schwannoma incidence have not attempted to account for alterations in specific genes, and could not distinguish between nonsense mutations and loss of heterozygosity (LOH). METHODS: Here, we present a mechanistic approach to modelling initiation and malignant transformation in schwannoma. Each parameter is associated with a specific gene or mechanism operative in Schwann cells, and can be determined by combining

Clinical Outcome of Pulsed-Radiofrequency Combined With Transforaminal Epidural Steroid Injection for Lumbosacral Radicular Pain Caused by Distinct Etiology.

BACKGROUND: Lumbosacral radicular pain (LSRP) can be caused by disc herniation, spinal stenosis, and failed back surgery syndrome. The clinical effect of pulsed-radiofrequency (PRF) combined with transforaminal epidural steroid injection (TESI) for radiating pain in different population remains unclear. METHODS: We retrospectively reviewed the medical recordings of patients with LSRP caused by different etiologies, who underwent PRF and TESI treatment. The primary clinical outcome was assessed by a 10-point Visual Analog Scale (VAS) pre- and post-treatment. RESULTS: A total of 34 LSRP patien

Seeing C. diff Differently: A Case of Clostridioides difficile Bacteremia in Metastatic Melanoma.

BACKGROUND Clostridioides difficile infection (CDI) is a common community-acquired and nosocomial infection that usually presents as colitis. C. difficile bacteremia (CDB) is a rare blood infection, with only a few cases recorded in the literature. We seek to expound on the current literature by detailing the clinical course of a patient with metastatic melanoma who developed CDB. CASE REPORT This case highlights the hospital course of a 51-year-old man admitted for a new onset of arrhythmia during the evaluation and management of a malignancy. The patient experienced hemodynamic collapse and

Minimally Invasive Repair of a Prostatorectal Fistula with an Over-the-Scope Rectal Clip.

Background: Fistulae between the prostatic urethra and the rectum are rare. They may result from prostatic or rectal surgery. Predisposing factors are previous radiation or immunosuppression. The repair of such fistulae usually involves major surgery. Recently, clips that can be deployed over an endoscope have been developed to close gastrointestinal fistulae or access points for natural orifice surgery. We report the first case of effective treatment of a prostatorectal fistula with a rectal "over-the-scope" clip. Case Presentation: A 64-year-old man under chronic immunosuppression presented

Myoepithelial carcinoma with RB1 mutation: remarkable chemosensitivity to carcinoma of unknown origin therapy.

BACKGROUND: Myoepithelial carcinoma of soft tissue is a rare, malignant neoplasm that is morphologically and immunophenotypically similar to its counterpart in salivary gland. It demonstrates myoepithelial differentiation, possessing both epithelial and myogenic characteristics. Thought to be chemotherapy insensitive, the optimal treatment regimen of this tumor has yet to be established and only a select few cases in the literature discuss treatment efficacy in detail. CASE PRESENTATION: Here we present a case of a young adult with metastatic myoepithelial carcinoma with an initial excellent

Cold excitation and determination of hydrogen sulfide by dielectric barrier discharge molecular emission spectrometry.

A low-temperature microplasma generated in a dielectric barrier discharge (DBD) was used as a radiation source for the excitation of hydrogen sulfide and its determination by molecular emission spectrometry (MES). The excitation/emission chamber was enclosed to eliminate spectral interference from ambient air. The spectral emission lines of hydrogen sulfide were clearly discriminated from the background spectrum, and the emission line at 365.06 nm was selected for parameter optimization and quantitative analysis. The S(2-) ions in aqueous samples were reacted with acid to generate hydrogen sul

Primary squamous cell carcinoma of the breast after cured bilateral breast cancer.

BACKGROUND: Primary squamous cell carcinoma (SCC) of the breast is a rare and aggressive neoplasm that constitutes approximately 0.1% of all breast carcinomas. Before the tumor can be classified as a true SCC of the breast, certain criteria need to be fulfilled. These are: i) more than 90% of the malignant cells must be of squamous cells origin; ii) tumor is independent from the overlying skin and nipple; iii) other sites of primary SCC have been excluded. CASE REPORT: We describe a case of pure SCC of the breast that arose 15 years after local radiation for a primary adenocarcinoma of the br

Radiofrequency ablation for chronic radiation proctitis: our initial experience with four cases.

BACKGROUND: Chronic radiation proctitis (CRP) occurs up to 20 % of patients after pelvic radiotherapy, with rectal bleeding as the main presenting complaint. Radiofrequency ablation (RFA) has recently been used in the management of Barrett's esophagus, but its efficacy in CRP has to be studied. The aim of this case series was to describe four cases of patients with CRP treated with RFA that demonstrate the efficacy and safety of the technique. METHODS: All the procedures were performed with HALO 90 or HALO 90 Ultra ablation catheter fitted on the distal end of a standard flexible endoscope. F

Clonal evolution of a case of treatment refractory maxillary sinus carcinoma.

BACKGROUND: Maxillary sinus carcinoma (MSC) is a rare cancer of the head and neck region. Patients are treated with surgery, radiation therapy, and chemotherapy and the treatment regimen is based on patient's age, general health condition, disease stage, and its extent of spread. There is very little information available on the genetics of this disease. DNA content based flow sorting of tumor cells followed by array comparative genomic hybridization allows for high definition global assessment of distinct clonal changes within tumor populations. METHODS: We applied this technique to primary

[Radical conjunctivectomy in therapy of malignant melanoma of the conjunctiva].

BACKGROUND: In tumor excision of large malignant melanomas of the conjunctiva or in the excision of widespread precancerous melanosis the development of symblepharon is a threatening complication after conjunctivectomy, especially if the conjunctival fornix is afflicted. PATIENTS: Three patients with suspected nodular melanoma and surrounding precancerous melanosis of the conjunctiva were treated by extended excision of the conjunctiva. Instead of a conjunctival graft, spontaneous epithelization under the protection of an 'Illig' contact lens was achieved. After this, two patients with histol
