Results of distal metatarsal osteotomy using absorbable pin fixation.

BACKGROUND: Metatarsalgia with plantar hyperkeratosis can be a difficult problem to treat. Many different techniques have been reported with the Weil osteotomy very popular currently. Despite good results complications include metatarsophalangeal stiffness, hyperextension of the toe, prehensile deficit and a reduction in strength. We evaluated our results with a distal diaphyseal osteotomy. MATERIALS AND METHODS: From 1999 to 2005, we performed 66 distal midshaft subtraction osteotomies in 62 patients suffering from metatarsal overload without metatarsophalangeal subluxation. Fixation was obt

Comparison of short-term results of bone tunnel enlargement between EndoButton CL and cross-pin fixation systems after chronic anterior cruciate ligament reconstruction with autologous quadrupled hamstring tendons.

We performed arthroscopic anterior cruciate ligament (ACL) reconstructions of chronic ACL rupture with autologous hamstring tendon grafts in males (mean age 23.9 years). Thirty-two cross-pin fixations and 24 EndoButton CL fixations were performed on the femoral side. Distal fixations on the tibial side were maintained with interference screws plus a staple. Tunnel enlargement was evaluated using X-rays. Femoral and tibial tunnels were measured at three levels 5 mm apart, and average enlargement was calculated after correction for magnification. Tunnel enlargement 12 months after surgery was gr

Treatment of trochanteric fractures with the PFNA (proximal femoral nail antirotation) nail system - report of early results.

Currently, intramedullary devices are widely used for the treatment of trochanteric femoral fractures. A new device designed by AO/ASIF, the PFNA (proximal femoral nail antirotation), represents a unique intramedullary nail system for improved management, particularly in the elderly. The aim of the present study was to analyze the results of treatment with PFNA in 50 patients with trochanteric fractures. The operating time, intraoperative blood loss, and intraoperative and postoperative complications were recorded. The length of the surgical procedure averaged 20.3 min, and intraoperative bloo

Transarticular stabilization of the immature femoral head: assessment of a novel surgical approach to the dislocating pediatric hip in a porcine model.

BACKGROUND: Acetabular dysplasia and hip instability are common in neuromuscular diseases such as spina bifida and cerebral palsy due to deranged muscle function around the hip. Occasionally in developmental dysplasia of the hip, persistent instability may be difficult to manage by standard treatments. It is in these contexts that we wished to investigate whether a dislocatable hip could be stabilized by means of a transarticular suture acting as a reconstructed ligamentum teres. The aim of this study was to investigate and assess the iatrogenic damage caused by such a procedure on the immatur

How should one treat iatrogenic ulnar injury after closed reduction and percutaneous pinning of paediatric supracondylar humeral fractures?

OBJECTIVE: The purpose of this study was to assess iatrogenic ulnar nerve injuries after supracondylar humeral fractures treated with closed reduction and percutaneous pinning. METHODS: The series consisted of 473 children. All patients were treated with closed reduction and percutaneous pinning. Neurological examination performed immediately after the operation revealed 25 ulnar nerve injuries (5.2%) in patients who had completely normal neurological findings in the preoperative period. Electromyographic examinations were performed at 6 and 12 weeks postoperatively in patient with ulnar nerv

Analysis of patients with nonambulatory neuromuscular scoliosis surgically treated to the pelvis with intraoperative halo-femoral traction.

STUDY DESIGN: Retrospective case-control study. OBJECTIVES: To compare patients treated with and without intraoperative halo-femoral traction to assess neuromuscular spinal deformity correction as well as the safety of the technique. SUMMARY OF BACKGROUND DATA: Optimal sitting balance can be achieved in nonambulatory neuromuscular patients with pelvic obliquity by maneuvering a Galveston-type rod or inserting screws into the iliac wings; however, this is often clinically challenging because of the small, soft bone-stock in the pelvis of these patients. METHODS: A total of 40 patients with n

[Clinical results with A new retrograde femoral nail with a radiographical-free proximal locking device. A prospective study with 50 fractures].

INTRODUCTION: Conventional retrograde nailing of the femur causes two important disadvantages: the proximal locking of the nail is difficult because of the anatomic conditions and a chondral defect was left into the knee. MATERIAL AND METHODS: After the retrograde implantation the new nail was lead through the greater trochanter. An additional proximal aiming device for proximal interlocking can be fixed. The entrance portal will be sealed by an osteochondral cylinder. 50 cases of femur fractures were selected for the prospective study. We recorded all intraoperative complications and technic

Elastic intramedullary nailing: a minimally invasive concept in the treatment of unstable forearm fractures in children.

The standard treatment in forearm fractures in children is usually conservative. Unstable fractures of the proximal parts of the forearm often show poor results after nonoperative management so that these fractures usually require surgical intervention. We report 30 children (ages, 4-14 years) who were treated by elastic intramedullary nailing. Sixteen patients were treated by intramedullary splinting immediately after their accident; 14 children required intramedullary nailing after failure of conservative treatment and fracture redisplacement. At the time of follow-up 6 months later, functio

Clinical evaluation of crossed-pin versus lateral-pin fixation in displaced supracondylar humerus fractures.

The radiographs and patient charts of 47 children treated with closed reduction and percutaneous pin fixation of displaced supracondylar humerus fractures were reviewed. Twenty-seven fractures were fixed with crossed medial and lateral pins. Twenty fractures were treated with two parallel laterally placed pins. Baumann's angle on the anteroposterior elbow film and the humerocapitellar angle on the lateral elbow film were independently measured by the three authors on initial postoperative films and on films taken at the time of pin removal. No statistically significant differences regarding ma

Preliminary clinical experience with a new fluted humeral nail.

The early clinical results of humeral nailing using a fluted intramedullary nail with a locking screw are reported. The True-Flex nail has been designed with a proximal cancellous screw to prevent 'back-tracking' and with flutes to give rotational control. The design features have proved to be successful in the authors' brief clinical experience in 12 patients with complicated fractures, non-unions or pathological lesions. Eight of the 10 fractures healed uneventfully, one by delayed union; one had to be revised because of loosening due to use of a nail which was too narrow. There were no seri

Interspinous process segmental spinal instrumentation.

A method for interspinous segmental spinal instrumentation (ISSI) is described and the laboratory testing and early clinical results are reported. The method utilizes a button-wire implant that is passed through the thickest and strongest part of the base of the spinous process. Tension tests show the stress-relieving qualities of the implant improve pullout strength 47% over simple wire fixation of the spinous process. Tests on the scoliosis simulator show that the interspinous instrumented spine resists high compressive loads to failure, comparing favorably with other systems tested. Early c

Trisomy 8 syndrome.

Trisomy 8 syndrome, with or without mosaicism, is characterized by the following clinical signs and symptoms: mental retardation, deformed skull, prominent forehad, high-arched palate, low-set and/or dysplastic ears, long and slender trunk, reduced joint mobility, and deep plantar furrows. The deep furrow or longitudinal crease of the sole may be pathognomonic for the syndrome. It should be noted that it has been seen in other rare syndromes. It is recommended that a child with mental retardation and associated multiple anomalies should be subjected to a cytogenetic study in order to define th

Hexaxial external fixator versus intramedullary nail in treating segmental tibial fractures: a retrospective study.

BACKGROUND: It's difficult to treat segmental tibial fractures (STFs), which are intricate injuries associated with significant soft tissue damage. The aim of this study was to compare the clinical effect of hexaxial external fixator (HEF) and intramedullary nail (IMN) in treatment of STFs. METHODS: A total of 42 patients with STFs were finally recruited between January 2018 and June 2022. There were 25 males and 17 females with age range of 20 to 60 years. All fractures were classified as type 42C2 using the Arbeitsgemeinschaftfür Osteosythese/Orthopaedic Trauma Association (AO/OTA) classifi

[Our experience with AES total ankle replacement].

PURPOSE OF THE STUDY: The method of choice for the treatment of severe ankle arthritis is either arthrodesis or joint arthroplasty. Each has its advantages and disadvantages. Arthrodesis is the definitive therapy for severe ankle destruction and instability. Joint arthroplasty has an advantage in maintaining ankle mobility. However, its range of indications and its reliability and durability are more limited. The aim of this study is to present our experience with the AES prosthesis and draw attention to some drawbacks of this surgical treatment. MATERIAL: From September 2003 till June 2008,

Comparison of the cable pin system with conventional open surgery for transverse patella fractures.

BACKGROUND: The cable pin system is an effective device for fixation of transverse patella fractures. However, whether this device provides superior results using a minimally invasive technique instead of conventional open surgery using the K wire tension band method is unclear. QUESTIONS/PURPOSES: We asked whether a minimally invasive technique would be associated with (1) increased operative time; (2) reduced postoperative pain; (3) faster recovery of ROM; (4) higher knee scores; and (5) reduced complications. METHODS: Forty patients with displaced transverse fractures of the patella parti

Retrograde reamed femoral nailing.

Closed, reamed, antegrade nailing remains the standard of care for femoral shaft fractures. This technique however, may be less attractive in the management of femoral shaft fractures associated with (a) ipsilateral acetabular, pelvis, or femoral neck fractures; (b) polytrauma requiring multiple simultaneous surgical procedures; and (c) pregnancy. We now report on our experience with the retrograde femoral nailing as a treatment option in these situations. Between 4/88 and 10/90, 29 retrograde femoral nailing in 24 patients were attempted. Average age was 29.3 (16-74) years. Five fractures wer

Results of Subtrochanteric Femoral Fractures Fixation by Proximal Femoral Locking Compression Plate.

The treatments of subtrochanteric femoral fractures are a challenge. It accounts about 10.0% to 34.0% of all hip fractures with a high complication rate. This area consists of mostly cortical bone with high stress generation thus heal slowly. The fracture is too proximal to adequately control with implants for femoral shaft and too distal to control with implants for intertrochanteric fractures. The intrinsic insecurity of this fracture and forces of the muscles with comminuted medial calcar is giving the fracture a tendency to varus crumple. Extramedullary implants are associated with higher
