When a brain scan is abnormal, one can often describe the abnormality in terms of its location, shape, sharpness, brightness, and other descriptive parameters. Certain types of abnormality have been linked to certain specific diagnoses: for example, a crescent-shaped lesion would suggest subdural hematoma to many people, a wedge-shaped lesion would suggest cerebral vascular accident, etc. Some features thought to be characteristic of certain diseases are actually quite nonspecific. For example, the "doughnut" sign--at first believed to be characteristic of brain abscess--has also been found in…

Seven patients with "locked-in" syndrome were studied by prolonged polygraphic recordings. Severe alterations in the sleep pattern were observed in five patients who had bilateral extensive pontine lesions resulting in tetraplegia, facial and pseudobulbar paralysis, and absence of conjugate horizontal gaze. Rapid eye movement (REM) sleep was entirely absent while non-rapid eye movement (NREM) sleep was absent, reduced, or altered. The remaining two patients, both of whom had relative sparing of horizontal gaze and apparently either no or minimal pontine tegmental involvement, showed both REM a

The results and conclusions of 43 surgical interventions performed on account of oto-rhinogenous brain abscess are reported. In these patients the infection of brain substance occurred in two ways: 1 by contact propagation and 2. per venous reflux, associated with thrombosinusitis. In the 1. the brain abscess was accompanied by purulent meningitis and the majority of the patients died during the postoperative period. In the 2, the patients operated on account of cerebral abscess developed per venous reflux, improved and became health. In order to get more exact localisation of the abscess ther…

Sequential brain scans were performed at various time intervals of up to four hours after injection of 99mTc pertechnetate in 108 patients who had previously undergone "routine" one-hour brain scans which were considered normal or equivocal. Forty-six patients were proved to have a discrete morphologic brain lesion. Pathologic processes were demonstrated by the sequential scanning technique in all but 6 cases. In nearly a third of these 46 patients (28%), careful observation of the progressive and subtle changes in radioactivity in the whole sequential scan series was necessary for detection o…

'Pure motor hemiplegia' is a common stroke syndrome defined by Fisher as paralysis of face, arm, and leg on one side, unaccompanied by sensory signs, visual field defect, aphasia, or apractognosia. It occurs almost exclusively in hypertensive patients and carried a good prognosis. We report a case of a normotensive patient in whom pure motor hemiplegia was the presenting feature, not of a cerebrovascular syndrome, but of a pontine glioblastoma. We note that brain-stem tumours may masquerade as brain-stem strokes.

As the most frequent solid tumor occurring in childhood, brain tumors constitute an important segment of pediatric oncology. Neurologic manifestations may be deceptively mild and easily overlooked or misinterpreted, particularly in the very young, because of the remarkable resiliency of the immature central nervous system and the skull's ability to expand throughout the pre-adolescent years. The majority of childhood tumors produce increased intracranial pressure, usually the consequence of obstructive hydrocephalus. Specific neurologic deficits correspond to the tumor's location. The posterio…

Late after-effects of cerebral trauma are difficult to study because patients tend to be seen for persisting symptoms, and not simply for their lesions. We have tried to avoid this bias by recalling periodically, over the years, 520 men with known brain injuries incurred in World War II or in Korea or Vietnam. These men are seen irrespective of clinical need and all undergo intensive behavioural and neurological assessment, which still continues. For such groups, recovery is impressive, though one third shows persistent intellectual loss. In addition, some tasks reveal specific deficits enduri…

Three different slow potential (SP) changes resulting from focal brain injury are described. The first is an immediate, high amplitude (in excess of 25 mV)negative shift at the site of injury. The second is a biphasic negative-positive SP wave which spreads throughout the cortex ipsilateral to injury and is similar to spreading depression (SD). The third SP change, called here the injury-induced diffuse slow potential (IDSP)is a prolonged (lasting approx. 2h) negative shift occurring simultaneously in many brain areas, also in those far removed from the injured focus. The SD can be separated f…

A malignant fibrous mesothelioma in a 52 year old white man arising from the left parietal pleura associated with lupus erythematosus with metastases to brain and liver is reported. Asbestos bodies were found in digested pulmonary tissue but none in the primary or metastatic lesions. Light microscopic and ultrastructural studies suggest that this tumor contains mesothelial or endothelial cells, some of which revealed fibroblastic features while others disclosed epitheloid characteristics.

Organic psychoses are caused by a variety of disorders. In general, they are due to diffuse dysfunction of the brain without any specific anatomical basis, but they may be modified by disorders of some distinct neuronal systems as a result of local accentuation of a dissuse morbid process or localized brain damage. Acute organic psychoses are usually caused by disorders of the blood-brain barrier (cerebral oedema), acute neuronal dysfunction of disorders of synaptic transmission. Acute lesions may be reversible or terminate in stationary defective states or progressive neuronal degeneration. I…

A man of 22 with Whipple's disease suffered from a acute febrile state and unconsciousness. Focal inflammatory lesions accompanied by large numbers of peculiar macrophages occurred in thalamus and basal ganglia. The electron microscopic findings conform to previous reports on brain involvement in Whipple's disease.

A study of 100 subjects has confirmed that brief voluntary hyperventilation commonly causes apnoea in patients with supramedullary lesions, but not in healthy people. Apnoea was related to drowsiness rather than to the extent of the lesion; it was unrelated to the measured reduction in end-tidal carbon dioxide tension.
