Cochlear implantation and management of chronic suppurative otitis media: single stage procedure?

In a series of 360 patients who underwent cochlear implantation at our center, four patients (five procedures) had cochlear implantation with obliteration of the mastoid cavity and management of cholesteatoma as a single-staged procedure. Three patients were bilaterally deaf secondary to CSOM and had bilateral mastoid cavities, and in one patient congenital cholesteatoma was identified during cochlear implantation. A mastoidectomy or revision mastoidectomy with obliteration of the mastoid cavity and cochlear implantation was performed as a single stage procedure. Cholesteatoma reoccurred in on

Anaesthetic management of labour in two patients with Klippel-Feil syndrome.

Two patients with Type I Klippel-Feil syndrome presented at the antenatal clinic. The first patient, who suffered from sleep apnoea, was delivered of a healthy infant by vacuum extraction. The second, who was profoundly deaf and had marked kyphoscoliosis, developed pregnancy-induced hypertension and urinary tract infection and was delivered at 38 weeks by vacuum extraction. In both cases epidural analgesia was employed to allow pain relief during labour. Anaesthetic management of Klippel-Feil syndrome is discussed and the benefits of early anaesthetic assessment and continued involvement of se

Argon laser in otosclerosis surgery.

Data are reported from 323 otosclerotic ears in which primary surgery was carried out by using a large fenestra stapedectomy technique with fascial sealing of the window. In 129 ears the footplate was perforated with an argon laser. In the whole series, one ear became deaf (0.3%) as a result of granulomatous reaction in the operated area. Results in the argon laser group were slightly better than those in the group with mechanical footplate perforation and reconstruction with wire and fascia. Use of argon laser facilitates stapes surgery and, when used judiciously, it is a very helpful tool in

[Direct optic nerve and retinal injury caused by retrobulbar injections].

The authors report on 2 patients with direct damage to the optic nerve and retina: In a 25-year-old deaf-mute patient a peripheral iridectomy was performed after lid and retrobulbar anesthesia. The amaurosis noticed immediately after surgery was caused by direct injury to the optic nerve. A 69-year-old patient with high myopia was treated with retrobulbar steroid injections. The globe was perforated and a series of 4 retinal defects from the periphery to close to the optic nerve head were created. An unhealed retinal detachment developed. The importance of informing the patient preoperatively

Mondini dysplasia--late complications.

Mondini's deformity is the second most common malformation seen in congenital genetic deafness. It is characterized by bony and membranous abnormalities of the inner ear, with a wide range of morphological and functional abnormalities. The importance of the Mondini malformation is that the patient is at an increased risk of developing meningitis or bilateral total hearing loss (or both) at an early age. Six cases are presented which illustrate the various sequelae of the Mondini malformation including meningitis, vertigo, and auditory deterioration. Decompression of the endolymphatic sac was u…

[Selected problems in the rehabilitation of young children with severe hearing loss].

The aim of the work was an analysis of the causes of difficulties in the rehabilitation of children with profound hearing loss. A theoretical basis of this analysis is the assumption that rehabilitation effects are determined by factors related to the system of care of the deaf child, diagnostic possibilities, and psychophysical properties of the child. The reported analysis was carried out in a material of 232 children aged up to 3 years, treated at the Audiology Outpatient Clinic of the Institute. The following factors were found of great importance in the analysis of difficulties in rehabil

The primary ultrastructural defect caused by anophthalmic white (Wh) in the Syrian hamster.

Anophthalmic white (Wh) of the Syrian hamster is a highly pleiotropic dominant spotting color mutation causing homozygotes to be deaf, blind, and white. An ultrastructural analysis of embryonic tissue reveals that Wh causes the retention of cilia by cells of opposing layers of the embryonic retina and by the lens fiber cells. Previous ultrastructural analyses indicate that Wh also causes the retention of cilia by secretory cells of the anterior lobe of adult pituitaries. We propose that the primary ultrastructural defects caused by Wh is the retention of cilia by embryonic cells. These retaine

Adenotonsillectomy; indications and application of surgical techniques.

As a result of audiometric surveys in public schools, many children are being referred to physicians for further examination and definitive treatment. Therefore, the indications for adenotonsillectomy are reviewed. It is believed that the operation should not be done in infants with congenital deafness until full study of the problem has been completed, and that it should not be done routinely in children in whom there is no positive indication for it. When the operation is done, as much normal mucosa as possible should be saved. Closing the superior aspect of the incision with chromic suture …

Case Report: Rapid cataract development preceding diabetes mellitus in WFS1 spectrum disorder.

WFS1 spectrum disorder is a rare condition, characterized by diabetes insipidus, diabetes mellitus, optic atrophy, and deafness (DIDMOAD). A 2-year-old female patient with a history of sensorineural hearing loss presented with rapid, sequential cataract development. Diabetes mellitus was not manifested at the time but developed 4 years later. While cataracts have been described in this syndrome, rapid acquisition of cataracts in the setting of mild hyperglycemia was unique considering they could not be definitively attributed to diabetes mellitus alone. This case provides real-world evidence t…

A rare lymphoplasmacyte-rich meningioma involving the dura of the skull base and cervical spinal cord: A case report.

Lymphoplasmacyte-rich meningioma (LPRM) is a rare subtype of meningioma, the specific pathogenesis of which remains unclear. Herein, we report the case of a 48-year-old Asian man who experienced progressive deafness and limb weakness. Magnetic resonance imaging revealed extramedullary masses diffusely growing, wrapping, and compressing the cervical spinal cord. The dural lesion was partially excised by surgery, and postoperative pathological examination confirmed the diagnosis of LPRM. Diffuse LPRM is extremely rare, and its treatment is challenging owing to difficulties associated with surger…

Prevention of the foreign body response to implantable medical devices by inflammasome inhibition.

SignificanceImplantable electronic medical devices (IEMDs) are used for some clinical applications, representing an exciting prospect for the transformative treatment of intractable conditions such Parkinson's disease, deafness, and paralysis. The use of IEMDs is limited at the moment because, over time, a foreign body reaction (FBR) develops at the device-neural interface such that ultimately the IEMD fails and needs to be removed. Here, we show that macrophage nucleotide-binding oligomerization domain-like receptor family pyrin domain containing 3 (NLRP3) inflammasome activity drives the FBR…

Lyme neuroborreliosis as a cause of sudden sensorineural hearing loss and facial palsy.

We present a case of sudden sensorineural hearing loss and rapidly progressive facial palsy in a female patient in her 40s with no, apparently, notable past medical or surgical history. Investigations revealed a positive serology for B. burgdoferi and the MRI allowed us to identify suggestive signs of Lyme meningitis with multiple cranial nerve involvement. After diagnosis, the patient was treated with intravenous ceftriaxone with a full recovery of sensorineural deafness and facial palsy. This case report highlights the importance of collecting a complete medical history in all cases of facia…

Navigating Hereditary Hearing Loss: Pathology of the Inner Ear.

Inherited forms of deafness account for a sizable portion of hearing loss among children and adult populations. Many patients with sensorineural deficits have pathological manifestations in the peripheral auditory system, the inner ear. Within the hearing organ, the cochlea, most of the genetic forms of hearing loss involve defects in sensory detection and to some extent, signaling to the brain via the auditory cranial nerve. This review focuses on peripheral forms of hereditary hearing loss and how these impairments can be studied in diverse animal models or patient-derived cells with the ult…
