Pathology of Rare Umbilical Lesions: A Case Series and Literature Review.

The remnants of the omphalomesenteric duct (OMD) can lead to various congenital anomalies, including Meckel's diverticulum, cysts, and fistulas, which may require surgical intervention. The OMD remnants usually occur in pediatric age groups. Clinical presentation of the patients varies widely. Some patients may remain asymptomatic throughout their lives. Some others may have complications necessitating surgical treatment. Symptoms include abdominal pain, gastrointestinal bleeding, or signs of infection. Imaging techniques such as ultrasound or CT scans are required to evaluate the presence of …

Minimally invasive management of patent vitello-intestinal duct in an adult.

The patent vitello-intestinal duct (PVID) is a persistent embryological remnant of the omphalomesenteric duct, which normally connects the yolk sac to the primitive midgut, and it is obliterated between the 8 th and 9 th weeks of gestation. Failure of this involution results in a spectrum of anomalies, including Meckel's diverticulum, umbilical sinus, fistula, and cyst. Although such remnants are present in approximately 2% of the population, symptomatic presentation in adulthood is exceedingly rare. We report the case of a 30-year-old male who presented with foul-smelling umbilical discharge …

Concurrent Amyand and Littre Hernias With Incidental Hodgkin Lymphoma: A Diagnostic and Surgical Challenge.

We report a rare case of a 62-year-old man with a right giant inguinoscrotal hernia and a concurrent retroperitoneal mass. Preoperative imaging was inconclusive regarding the hernia sac contents and the nature of the pelvic mass. Intraoperatively, the hernia sac contained the caecum, appendix, Meckel's diverticulum (MD) and terminal ileum-confirming the simultaneous presence of Amyand and Littre hernias. Additionally, a left-sided retroperitoneal solid-cystic mass was identified and excised. This case underscores the diagnostic difficulties posed by complex hernias and highlighted the surgical…

A rare case of right aortic arch with mirror-image branching and vascular ring in an adult patient.

A right-sided aortic arch with mirror-image branching is a rare vascular anomaly, often associated with congenital heart defects. However, its occurrence with a vascular ring in the absence of cardiac abnormalities is extremely rare. We report the case of a 62-year-old male presenting with acute dyspnea, in whom CT angiography revealed a right-sided aortic arch with a vascular ring formed by an aortic diverticulum and the left brachiocephalic artery, causing mild tracheal and esophageal compression. Echocardiography confirmed the absence of cardiac anomalies. This case highlights a rare presen…

Postmortem CT and MRI for detecting bowel obstruction in cases of pediatric sepsis deaths.

This article describes the utility of postmortem CT and MRI in diagnosing pediatric bowel obstruction (BO) as the cause of septic shock-induced death. Six pediatric cases with confirmed septic shock were retrospectively analyzed. Postmortem CT scans revealed clear signs of BO in all cases, with volvulus, intussusception, diaphragmatic hernia, or Meckel's diverticulum identified. MRI scans, performed in three cases, did not provide additional diagnostic information. The presented case series highlights the potential of postmortem CT for diagnosing BO in children, potentially aiding in understan…

Complex balanced intrachromosomal rearrangement involving PITX2 identified as a cause of Axenfeld-Rieger Syndrome.

Axenfeld-Rieger Syndrome (ARS) type 1 is a rare autosomal dominant condition characterized by anterior chamber anomalies, umbilical defects, dental hypoplasia, and craniofacial anomalies, with Meckel's diverticulum in some individuals. Here, we describe a clinically ascertained female of childbearing age with ARS for whom clinical targeted sequencing and deletion/duplication analysis followed by clinical exome and genome sequencing resulted in no pathogenic variants or variants of unknown significance in PITX2 or FOXC1. Advanced bioinformatic analysis of the genome data identified a complex, b…

Duodenal Diverticular Perforation Treated Conservatively: Reassessing Indications for Treatment.

A 59-year-old woman presented with a chief complaint of melena. She had no abdominal findings, such as tenderness or tapping pain. Laboratory tests revealed a white blood cell count of 5,300 cells/μL and C-reactive protein level of 0.07 mg/dL. Inflammation and anemia (hemoglobin 12.4 g/dL) were denied. Contrast-enhanced computed tomography (CT) revealed multiple duodenal diverticula and air surrounding a descending duodenal diverticulum. Based on these findings, duodenal diverticular perforation (DDP) was suspected. Oral food intake was stopped, and nasogastric tube feeding and conservative tr…

A rare case of duodenal angiodysplasia-a case report.

Gastrointestinal bleeding can be manifested as a variety of symptoms and, often, it is difficult to classify as upper or lower gastrointestinal bleeding on mere symptomatology. This is a case report of a similar kind of patient who initially was diagnosed with fresh per rectum bleeding, subsequently diagnosed as bleeding angiodysplasia in duodenal diverticulum by a series of investigations and management including urgent oesophageal-gastroduodenoscopy (OGD), laparotomy, followed by computerized tomography-angiogram. As diagnosis was established after laparotomy, the patient was kept intubated …

Submucosal Tunneling Techniques for Tumor Resection.

The concept of third space endoscopy is based on the principle that the deeper layers of the gastrointestinal tract can be accessed by tunneling in the submucosal space and maintaining the integrity of the overlying mucosa. The mucosal flap safety valve enabled endoscopists to use submucosal space securely. The era of third space endoscopy started with peroral endoscopic myotomy for treatment of achalasia and has expanded to treat various other gastrointestinal disorders, such as mucosal lesions, submucosal tumors, extraluminal tumors, and refractory gastroparesis, Zenker diverticulum, and res…

A case of perforated Meckel's diverticulitis tethered to the umbilicus associated with a urachal remnant.

Meckel's diverticulum (MD) occurs in 2% of the population and is often asymptomatic. It is an embryological remnant of the oomphalomesenteric duct and can be associated with another embryonic structure-the urachus. A 23-year-old male presented with generalized abdominal pain and fever on a background of chronic abdominal pain and recurrent urinary infections. A CT scan of the abdomen and pelvis revealed an inflamed MD. Next day, the patient deteriorated and was taken to theatre. The MD was found to be both perforated and tethered to the umbilicus, which itself was directly related to an abnorm…

Jejunal Ectopic Pancreas: A Rare Cause of Small Intestinal Mass.

Ectopic pancreas (EP) is defined as the presence of pancreatic tissue outside the pancreas with no anatomical connection to the pancreas. It is also known as pancreatic heterotopia, accessory pancreas, aberrant pancreas, or pancreatic rest. The first case of EP was described in 1727 when pancreatic tissue was identified in the ileal diverticulum. Abdominal pain and lower gastrointestinal bleeding are the most common symptoms. On histopathological examination, EP can be classified into four subtypes. Once identified and diagnosed, the treatment is surgical resection and the post-operative cours…

An Unusual Cause of Biliary Tract Obstruction: Lemmel Syndrome.

BACKGROUND: Lemmel syndrome is a rare and misdiagnosed etiology of obstructive jaundice due to a periampullary duodenal diverticulum causing a mechanical obstruction of the common bile duct. It represents an obstructive jaundice with the absence of choledocholithiasis or pancreaticobiliary tumors. It is an underreported entity due to the absence of specific pathognomonic signs. CASE PRESENTATION: A 77-year-old-woman admitted for sepsis, due to an ascending cholangitis, underwent a MRCP and a gastroduodenoscopy revealing Lemmel's syndrome. Due to failure of ERCP, the patient underwent surgical…

Percutaneous endoscopic gastrostomy: a dislodgement complication due to a moving hiatal hernia.

Objectives: Frail patients with malnourishment due to an esophageal condition and dysphagia are often unamenable to safe surgery. Thus, in high-risk patients, less invasive techniques such as percutaneous endoscopic gastrostomy (PEG) are regarded as a safer choice. Nevertheless, as described here, PEG insertion can have rare serious complications.Materials and methods: We report the case of a frail patient with dysphagia due to a large Zenker's diverticulum and concomitant giant hiatal hernia. To improve her nutrinitional status before surgery she received a PEG after endoscopic repositioning …

Bowel Perforation During Haemophagocytic Lymphohistiocytosis Treatment with Corticosteroids and Anakinra.

UNLABELLED: We report the use of anakinra to treat a case of a 64-year-old man diagnosed with haemophagocytic lymphohistiocytosis (HLH) with neurological involvement. After the administration of intravenous pulse corticosteroid therapy, immunoglobulin and anakinra the patient showed neurological recovery. However, the recovery was complicated by the perforation of a pre-existing bowel diverticulum. The effect of anakinra on bowel inflammation has not yet been clearly established. It can potentially augment bowel inflammation and contribute to the risk of bowel perforation associated with the c…

Transcholecystic approach for distal common bile duct stricture in a non-dilated biliary system: an alternative route.

A 63-year-old patient was admitted to intensive treatment unit with biliary sepsis due to a small distal common bile duct stone. Endoscopic retrograde cholangiopancreatography was initially attempted for insertion of a biliary stent but failed due to the presence of a periampullary diverticulum. Referral to interventional radiology for percutaneous drainage was considered the next alternative even though there was no dilatation of intrahepatic ducts. Due to complete absence of intrahepatic duct dilatation, the traditional percutaneous transhepatic route was considered rather challenging. An al…
