Anodic and symmetric biphasic pulses enlarge the therapeutic window in deep brain stimulation for essential tremor.

BACKGROUND: Since the inception of DBS, cathodic pulses have been used. OBJECTIVE: To investigate the effect of anodic and symmetric biphasic pulses on the therapeutic window (TW) in essential tremor (ET) patients. METHODS: A randomized, doubled-blinded, cross-over design was used to test the effect of cathodic, anodic and symmetric biphasic pulses (cathode-first and anode-first) on the TW in an acute clinical setting. TW was defined as the difference between the minimal stimulation amplitude provoking side effects and minimal stimulation amplitude inducing tremor arrest. RESULTS: 9 ET pati

Characterization of interlimb interaction via transcutaneous spinal stimulation of cervical and lumbar spinal enlargements.

The use of transcutaneous electrical spinal stimulation (TSS) to modulate sensorimotor networks after neurological insult has garnered much attention from both researchers and clinicians in recent years. Although many different stimulation paradigms have been reported, the interlimb effects of these neuromodulation techniques have been little studied. The effects of multisite TSS on interlimb sensorimotor function are of particular interest in the context of neurorehabilitation, as these networks have been shown to be important for functional recovery after neurological insult. The present stu

Transcutaneous auricular vagus nerve stimulation and heart rate variability: Analysis of parameters and targets.

OBJECTIVES: Transcutaneous auricular vagus nerve stimulation (taVNS) modulates central and peripheral neurophysiology. Specifically, taVNS increases heart rate variability (HRV) indicating a shift in autonomic function towards parasympathetic predominance. However, knowledge on the influence of stimulation parameters and targets is scarce. We hypothesized that the location and charge per phase of taVNS influences HRV. MATERIALS AND METHODS: In thirteen healthy subjects, six different stimulation targets were investigated, i.e., cymba conchae, cavum conchae, outer tragus, inner tragus, crus he

Auricular transcutaneous vagus nerve stimulation modulates the heart-evoked potential.

BACKGROUND: There is active interest in biomarker discovery for transcutaneous auricular vagus nerve stimulation (taVNS). However, greater understanding of the neurobiological mechanisms is needed to identify candidate markers. Accumulating evidence suggests that taVNS influences activity in solitary and parabrachial nuclei, the primary brainstem relays for the transmission of visceral sensory afferents to the insula. The insula mediates interoception, which concerns the representation and regulation of homeostatic bodily states. Consequently, interoceptive pathways may be relevant to taVNS me

Effect of posture and body weight loading on spinal posterior root reflex responses.

The posterior root muscle response (PRM) is a monosynaptic reflex that is evoked by single pulse transcutaneous spinal cord stimulation (tSCS). The main aim of this work was to analyse how body weight loading influences PRM reflex threshold measured from several lower limb muscles in healthy participants. PRM reflex responses were evoked with 1-ms rectangular monophasic pulses applied at an interval of 6 s via a self-adhesive electrode (9 × 5 cm) at the T11-T12 vertebral level. Surface electromyographic activity of lower limb muscles was recorded during four different conditions, one in decubi

Transcutaneous vagus nerve stimulation for the treatment of drug-resistant epilepsy: a meta-analysis and systematic review.

BACKGROUND: The transcutaneous vagus nerve stimulation (tVNS) is a newly developed non-invasive technique in the treatment of drug-resistant epilepsy and results in positive effects for patients who cannot tolerate invasive vagus nerve stimulation. In this study, we aim to define the relationship between tVNS and seizure control, quality of life (QOL) and some other factors. METHODS: We found articles by searching through PubMed and Web of Science, and a total of three articles with 280 patients overall were included. These eligible studies include two randomized double-blinded trials and one

Electrical Neuromodulation of the Respiratory System After Spinal Cord Injury.

Spinal cord injury (SCI) is a complex and devastating condition characterized by disruption of descending, ascending, and intrinsic spinal circuitry resulting in chronic neurologic deficits. In addition to limb and trunk sensorimotor deficits, SCI can impair autonomic neurocircuitry such as the motor networks that support respiration and cough. High cervical SCI can cause complete respiratory paralysis, and even lower cervical or thoracic lesions commonly result in partial respiratory impairment. Although electrophrenic respiration can restore ventilator-independent breathing in select candida

The relationship among distortion-product otoacoustic emissions, evoked potential thresholds, and outer hair cells following interrupted noise exposures.

Distortion-product otoacoustic emissions (DPOAEs) are gaining popularity as indicators of the status of the cochlea. The efficacy of DPOAEs as indicators of changes in thresholds and the status of outer hair cells (OHCs) were examined using an animal model. Monaural chinchillas were exposed to an octave band noise (OBN) centered at 0.5 kHz at 95 dB SPL for 6 hr/day for 10 days. DPOAEs and evoked potential thresholds were recorded before, during, and after the exposures. The animals were sacrificed 5 days after the last exposure, and the status of OHCs was assessed using scanning electron micro

Idiopathic trigeminal neuralgia: sensory features and pain mechanisms.

We present a case report of a patient with the typical sensory features of idiopathic trigeminal neuralgia (ITN). The pain was elicited by innocuous stimuli, summated with repeated stimulation, radiated outside the stimulus zone, referred to a distant site, persisted beyond the period of stimulation, and exhibited a variable refractory period. Unusual sensory features included multiple trigger zones that changed over time and involved all 3 trigeminal divisions. Our sensory evaluation indicated that the pain was evoked by repetitive activation of rapidly adapting, A beta, low-threshold mechano

Strategies for replacing non-invasive brain stimulation sessions: recommendations for designing neurostimulation clinical trials.

Despite the potential impact of missed visits on the outcomes of neuromodulation treatments, it is not clear how this issue has been addressed in clinical trials. Given this gap in the literature, we reviewed articles on non-invasive brain stimulation in participants with depression or chronic pain, and investigated how missed visits were handled. Areas covered: We performed a search on PUBMED/MEDLINE using the keywords: 'tDCS', 'transcranial direct current stimulation', 'transcranial magnetic stimulation', 'depression', and 'pain'. We included studies with a minimum of five participants who w

Phosphene thresholds correlate with paired-pulse suppression of visually evoked potentials.

BACKGROUND: Phosphene thresholds (PT) induced by transcranial magnetic stimulation (TMS) as well as paired-pulse suppression (PPS) of visually evoked potentials (VEP) are used to characterize visual cortex excitability, however, their relation remains unknown. METHODS: We measured PT after single TMS over the occipital lobe, and recorded VEPs after paired-pulse stimulation at short stimulus-onset-asynchronies in the same subject. PPS was expressed by the ratio second to first response. RESULTS: We found a negative correlation between PT and PPS (r = -0.36, P = 0.039) indicating that higher P

Transcranial Random Noise Stimulation for the Acute Treatment of Depression: A Randomized Controlled Trial.

BACKGROUND: Transcranial electrical stimulation has broad potential as a treatment for depression. Transcranial random noise stimulation, which delivers randomly fluctuating current intensities, may have greater cortical excitatory effects compared with other forms of transcranial electrical stimulation. We therefore aimed to investigate the antidepressant efficacy of transcranial random noise stimulation. METHODS: Depressed participants were randomly assigned by computer number generator to receive 20 sessions of either active or sham transcranial random noise stimulation over 4 weeks in a d
