Association between the cumulative duration of abnormal cardiotocography patterns and fetal acidemia.

OBJECTIVE: To assess the association between the cumulative duration of abnormal cardiotocography (CTG) findings, classified using the Japan Society of Obstetrics and Gynecology (JSOG) five-tier cardiotocography (CTG) classification system, and fetal acidemia, with analyses performed separately for the first and second stages of labor. METHODS: We conducted a single-center retrospective cohort study at the National Center for Child Health and Development between 1 May 2015 and 1 May 1 2022, including women with planned vaginal delivery at ≥37 0/7 weeks of gestation. Cases with an umbilical ar

Trajectory of serial amniotic fluid index measurements is associated with fetal growth, birth weight and neonatal adiposity.

OBJECTIVE: Amniotic fluid volume, measured in terms of the amniotic fluid index (AFI), is used widely in prenatal care to assess fetal health and development. We investigated whether distinct longitudinal AFI trajectories exist during pregnancy and their association with fetal growth. METHODS: This secondary analysis of a randomized controlled trial included singleton pregnancies without pre-existing or gestational diabetes mellitus that received prenatal care at National Taiwan University Hospital in Taipei and its Hsin-Chu Branch in Hsinchu, Taiwan. Ultrasonography was performed at 22 + 0 t

Glucose control during pregnancy in patients with type 1 diabetes correlates with fetal hemodynamics: a prospective longitudinal study.

BACKGROUND: Maternal diabetes adversely affects fetal cardiovascular system development. Previous studies have reported that the fetuses of mothers with diabetes exhibit both structural and functional changes; nevertheless, prior studies have not examined the association between glucose control and fetal cardiac morphology and performance. Thus, the objective was to determine the association between fetal cardiac morphology and function and maternal glucose control in type 1 diabetes and to compare the differences in measured cardiac parameters between the fetuses of mothers with diabetes and

Chronic inflammation of the placenta: definition, classification, pathogenesis, and clinical significance.

Chronic inflammatory lesions of the placenta are characterized by the infiltration of the organ by lymphocytes, plasma cells, and/or macrophages and may result from infections (viral, bacterial, parasitic) or be of immune origin (maternal anti-fetal rejection). The 3 major lesions are villitis (when the inflammatory process affects the villous tree), chronic chorioamnionitis (which affects the chorioamniotic membranes), and chronic deciduitis (which involves the decidua basalis). Maternal cellular infiltration is a common feature of the lesions. Villitis of unknown etiology (VUE) is a destruct

Redo aortic valve replacement in twin pregnancy: navigating high-risk cardiac surgery with maternal and fetal success - A case report.

INTRODUCTION AND IMPORTANCE: Redo aortic valve replacement (AVR) in pregnancy presents significant challenges due to maternal hemodynamics, fetal well-being, and anticoagulation management. Prosthetic valve stenosis is a life-threatening condition and can significantly alter the maternal and fetal outcome without treatment. We report a case of a pregnant woman with twin gestation who developed prosthetic valve stenosis due to pannus formation and requiring redo AVR. CASE PRESENTATION: A 31-year-old woman, pregnant with dichorionic diamniotic twins at 14 weeks, presented with worsening dyspnea

Integrated Deep Learning and Supervised Machine Learning Model for Predictive Fetal Monitoring.

Asphyxiation associated with metabolic acidosis is one of the common causes of fetal deaths. The paper aims to develop a feature extraction and prediction algorithm capable of identifying most of the features in the SISPORTO software package and late and variable decelerations. The resulting features were used for classification based on umbilical cord pH data. The algorithms developed here were used to predict cord pH levels. The prediction system assists the obstetricians in assessing the state of the fetus better than the category methods, as only about 30% of the patients in the pathologic

The role of doppler waveforms in the fetal main pulmonary artery in the prediction of neonatal respiratory distress syndrome.

OBJECTIVE: To describe changes in the Doppler waveforms of the fetal main pulmonary artery (MPA) throughout gestation and to assess their predictive value of neonatal respiratory distress syndrome (RDS). STUDY DESIGN: In the first phase of this study, we performed Doppler measurement of MPA acceleration time (AT), ejection time (ET), peak systolic velocity, end-diastolic velocity, mean velocity, pulsatility index, and resistance index in 288 healthy fetuses. In the second phase, we carried out these measurements in a prospective cohort of 52 pregnant women with impending preterm birth. RESUL

Effectiveness of sotalol as first-line therapy for fetal supraventricular tachyarrhythmias.

Fetal supraventricular tachycardia (SVT) and atrial flutter (AF) can be associated with significant morbidity and mortality. Digoxin is often used as first-line therapy but can be ineffective and is poorly transferred to the fetus in the presence of fetal hydrops. As an alternative to digoxin monotherapy, we have been using sotalol at presentation in fetuses with SVT or AF with, or at risk of, developing hydrops to attempt to achieve more rapid control of the arrhythmia. The present study was a retrospective review of the clinical, echocardiographic, and electrocardiographic data from all preg

Effect of maternal autoantibodies on fetal cardiac conduction: an experimental murine model.

The pathogenesis of congenital heart block (CHB) remains unclear. The occurrence rate of neonatal CHB is low, even in murine models of lupus erythematosus. The assessment of heart block in murine maternal lupus models by measuring atrioventricular conduction in neonatal offspring is potentially confounded by fetal wastage. We therefore sought to develop a murine CHB model with a superior immune response and to use embryonic Doppler echocardiography to observe conduction system damage in the fetus. Mature 8-wk-old female C3H/HeJ mice (n=43) were immunized with 60 kD Ro, 48 kD La, or recombinant

Haematological and obstetric aspects of antenatal diagnosis of beta-thalassaemia: experience with 200 cases.

The results of 200 antenatal diagnoses in pregnancies at risk for homozygous beta-thalassaemia, carried out on fetal blood samples obtained by placental aspiration in the second trimester, are described. Globin chain synthesis in the fetuses was measured by means of 3H-leucine incorporation and separation of the chains on carboxy-methyl-cellulose columns. Fetal red cell enrichment was performed by NH4Cl-NH4HCO3 differential lysis of maternal cells or anti-i differential agglutination. Sufficient fetal blood for analysis was obtained in 97.5% of the cases. The overall fetal loss rate was 6.5%,

Fetal MRI: what's new? A short review.

Fetal magnetic resonance imaging (fetal MRI) is usually performed as a second-level examination following routine ultrasound examination, generally exploiting morphological and diffusion MRI sequences. The objective of this review is to describe the novelties and new applications of fetal MRI, focusing on three main aspects: the new sequences with their applications, the transition from 1.5-T to 3-T magnetic field, and the new applications of artificial intelligence software. This review was carried out by consulting the MEDLINE references (PubMed) and including only peer-reviewed articles wri

Detection of fetal arrhythmia by adaptive single channel electrocardiogram extraction.

Fetal arrhythmia, the abnormal heartbeat of a fetus is broadly classified as tachy arrhythmia (too fast > 160 beats/min) and brady arrhythmia (too slow < 120 beats/min). Detection of this irregular heart beat rhythm of the fetus during pregnancy is still a challenging task for the clinicians. Heart rate detection through electrocardiography has always been accurate for identifying cardiac defect in humans. Adult ECG has achieved several developments in the modern medicine whereas noninvasive fetal ECG (FECG) continues to be a big challenge. Automatic detection of fetal heart rate is vital for

Prenatal typing of fetal DNA in cases of potential alloimmune haemolytic disease of the newborn: clinical benefit outweighs disadvantage.

Haemolytic disease of the newborn (HDN) is caused by the action of maternal antibodies on paternally derived antigens present on fetal erythrocytes. In its most severe form it may lead to death in utero. Although the introduction of anti-D prophylaxis has greatly reduced the incidence of the disease, a significant number of cases appear each year. Current antenatal management aims to predict whether the fetus will be severely affected, correct the fetal anaemia if present and deliver the baby at the optimal time. Indirect methods include antibody quantitation performed on maternal blood sample

A rapid combined immunocytochemical and fluorescence in situ hybridisation method for the identification of human fetal nucleated red blood cells.

Fetal nucleated red blood cells are found in the maternal circulation during pregnancy. If a simple routine method of detection of these cells was developed, it could be used as the basis of non-invasive prenatal diagnosis of fetal genetic disorders. Fetal male and adult female blood were mixed to mimic maternal blood in pregnancy and used to establish a simple technique to unequivocally detect fetal nucleated red blood cells. These were identified by combined immunocytochemistry using a human fetal haemoglobin antibody and a rapid and simple-to-use fluorescence in situ hybridisation method us

Human fetal testis xenografts are resistant to phthalate-induced endocrine disruption.

BACKGROUND: In utero exposure to endocrine-disrupting chemicals may contribute to testicular dysgenesis syndrome (TDS), a proposed constellation of increasingly common male reproductive tract abnormalities (including hypospadias, cryptorchidism, hypospermatogenesis, and testicular cancer). Male rats exposed in utero to certain phthalate plasticizers exhibit multinucleated germ cell (MNG) induction and suppressed steroidogenic gene expression and testosterone production in the fetal testis, causing TDS-consistent effects of hypospadias and cryptorchidism. Mice exposed to phthalates in utero exh

Prenatal diagnosis of cerebral lesions in Tuberous sclerosis complex (TSC). Case report and review of the literature.

Tuberous sclerosis complex (TSC) is an autosomal-dominant neurocutaneous disorder with multi-organ involvement. The diagnosis is suspected at fetal ultrasound on the discovery of multiple cardiac rhabdomyomas (CRs). They typically develop in utero and undergo spontaneous regression during the first years of live. With developing neuroradiological methods more light is shed on antenatal cerebral lesions like cortical tubers or giant cell astrocytomas. Unfortunately these do not regress, but instead are in principle progressive in size and number, correlated with epilepsy, mental retardation and

Melatonin Receptor 1 and Melatonin Receptor 2 Expression During Human Kidney Development and Their Association with CAKUT.

BACKGROUND/OBJECTIVES: Growing evidence indicates that melatonin contributes to kidney development and function, while disruptions of fetal circadian signaling have been linked to congenital anomalies of the kidney and urinary tract (CAKUT). This study aimed to characterize the developmental and spatial expression patterns of melatonin receptors MTNR1A and MTNR1B in normal human fetal kidneys and in CAKUT phenotypes. METHODS: This study analyzed 40 human fetal kidney specimens, including healthy controls and CAKUT cases (horseshoe kidneys, duplex kidneys, and dysplastic kidneys), obtained fro

Differential disruption of gonadal development by DEHP and paracetamol in male and female Wistar rats.

Paracetamol and di(2-ethylhexyl) phthalate (DEHP) are endocrine-disrupting chemicals (EDCs) known to impair prenatal gonadal development and inhibit testosterone synthesis in experimental models. We hypothesized that these substances may interact with the endocannabinoid system (ECS), potentially contributing to testicular and ovarian developmental abnormalities. Pregnant Wistar rats were exposed to paracetamol (50 or 250 mg·kg⁻¹·day⁻¹) or DEHP (750 mg·kg⁻¹·day⁻¹) from gestational days 15-18. We assessed fetal testicular testosterone production, early postnatal ovarian follicle counts, and the

Effects of selective serotonin reuptake inhibitors on the placenta†.

The rate of depression in pregnant mothers has dramatically risen in the past few decades. One of the well-studied causes of depression is a deficiency of serotonin (5-HT) in the synaptic cleft of neurons in the central nervous system, called the serotonergic theory of depression. The serotonin transporter (SERT/SLC6A4) binds 5-HT such that it can no longer bind and activate its cognate receptors on the post-synaptic neurons. Greater number of pregnant women are prescribed a selective serotonin reuptake inhibitor (SSRI) drug to combat depression. These compounds act to bind to SERT, and thereb
