STUDY DESIGN: Atlas fractures are evaluated according to the fracture type and ligamentous injury. External immobilization may result in fracture nonunion.
OBJECTIVE: The ideal treatment method for non-stabilized atlas fractures is limited fixation without restricting the range of motion of the atlantoaxial and atlantooccipital joints.
SUMMARY OF BACKGROUND DATA: Such a result can be established by using either anterior fixation or posterior lateral mass fixation. However, none of these techniques can fully address anterior 1/2 atlas fractures such as in this case.

Hoffa fracture is a rare injury consisting of unicondylar tangential posterior fracture of the distal femur and only very few cases have been reported in the literature. These fractures are due to high energy trauma and conservative treatment generally yields poor results, but rigid internal fixation allows early functional rehabilitation and decreases the incidence of complications. The purpose of the study was to prospectively analyse the clinico-radiological and functional outcome following open surgical treatment. From July 2005 to July 2010, 22 patients (14 males and 8 females) were recru

BACKGROUND: MacFarland fracture is a joint fracture of the ankle in children involving the medial malleolus (Salter-Harris type III or IV). These fractures are acknowledged to have poor prognosis because of the risk of misalignment due to the development of an epiphysiodesis bridge. Current recommended treatment for a displacement of ≥ 2 mm is open reduction through an arthrotomy with screw fixation. This study aimed to evaluate functional and radiologic results of a less-invasive surgical technique consisting of closed reduction, arthrographic control of fracture reposition, and percutaneous

BACKGROUND CONTEXT: In contrast to the majority of outcome data, many consider C1 fractures to be benign injuries and so have advocated for conservative management, except in the case of concomitant transverse atlantal ligament (TAL) injury where C1-C2 or occiput-C2 fusions are recommended.
PURPOSE: Our goal was to evaluate a series of unstable C1 fractures treated with C1 open reduction and internal fixation (ORIF) to assess clinical and radiographic outcomes by determining the success of reduction and pain relief.
STUDY DESIGN/SETTING: This is a retrospective cohort review.

BACKGROUND: The risk of internal fixation failure remains relatively high in stable femoral neck fracture (FNF) (Garden I or II). Preoperative sagittal displacement of the femoral head has been proposed as a potential influencing factor. This study aimed to evaluate the impact of sagittal displacement on the outcomes of cannulated screw internal fixation (CSIF) in patients with stable FNF (Garden I or II) by reconstructing the axial sagittal oblique plane of the fracture using preoperative computed tomography (CT) imaging.
METHODS: This study included 167 patients with FNF who underwent CSIF.

INTRODUCTION: Mandibular fractures are common injuries during maxillofacial trauma, and currently, open reduction and internal fixation are considered gold-standard treatments. There is a wide discussion about which plates give the best outcomes. Hence, we are conducting a biomechanical comparison of two plates for mandibular symphysis and angle fracture with finite element analysis (FEA). The aim of this study was to do a comparative study of FEA between the conventional and our new modified three-dimensional (3D) strut miniplate in mandibular fractures at symphysis and angle regions.

The 5th metatarsal fracture is a common foot fracture which could exclude a player from competition for several months and significantly affect his or her career. This manuscript presents the treatment and rehabilitation of professional soccer players who had acute fractures of the 5th metatarsal bone and a cannulated screw fixation. The main purpose of the analysis was to determine the minimum time necessary for a permanent return to the sport after a 5th metatarsal fracture among professional soccer players. We followed the surgical and rehabilitation path of 21 professional soccer players f

Avulsion fractures of the calcaneus are relatively uncommon and are seen most frequently in elderly or osteoporotic patients. A surgical method that avoids displacement of the avulsed fragment after fixation has not been developed. We report the cases of 3 patients (a 73-year-old male, an 85-year-old male, and an 81-year-old female) treated by open reduction and internal fixation using titanium wire and cannulated cancellous screws. The posterior approach was used by way of a vertical midline incision. The fracture was fixed with 2 screws, and then a titanium wire was passed through the holes

PURPOSE: Operative fixation of femoral neck fractures (FNFs) remains challenging. Complications are not infrequent, especially in displaced patterns. Numerous fixation techniques have been previously described in the literature; however, there remains a paucity of data regarding outcomes of these injuries treated with the femoral neck system (FNS).
METHODS: Patients with a displaced FNF (OTA/AO 31B) treated with the FNS at a single level 1 academic trauma center between 1/1/2019 and 1/1/2023 were identified. Radiographs were reviewed to assess fracture displacement, location, and characterist

The surgical treatment for fibrous dysplasia (FD) of bone is problematic due to its variable clinical courses. And multifarious surgical treatment options have been reported while no consistent view can be reached. Therefore, we reviewed a series of 22 patients (11 males and 11 females; mean age 28.4 years, range 15-48 years) with FD between December 2011 and July 2015. Fourteen patients had monostotic fibrous dysplasia (MFD) and eight patients had polyostotic fibrous dysplasia (PFD) with nine lesions. All patients were followed up from 15 to 58 months with an average of 26.0 months. Functiona

BACKGROUND: As a congenital metabolic bone disease caused by defective osteoclastic resorption of immature bone, osteopetrosis is characterized by diffused sclerosis of bones, brittle bones, easy fracturing, narrow medullary canals, and a weak fracture healing ability. At present, clear standards and principles for the treatment of fractures in patients with osteopetrosis are lacking. Non-operative treatment can prevent fracture hematoma and preserve the blood supply to the bone fragments, while being associated with frequent failures and higher mortality rates. Meanwhile, closed reduction and

BACKGROUND: It has been assumed that outcome after open reduction and internal fixation of displaced intra-articular calcaneal fractures may be affected by the presence of institutional trauma care and the institution's fracture volume. The purpose of this systematic review was to investigate whether a relationship exists between institutional fracture load and the rates of serious infection and subtalar arthrodesis following the treatment of these fractures.
METHODS: With use of a systematic method, all studies published between 2000 and 2006 describing adult patients undergoing open reducti

Management of multiple hand fractures in a mangling injury requires an individualized approach that balances biomechanical stability with soft tissue preservation and surgical efficiency. While plate-and-screw constructs offer strength, less rigid methods like Kirschner wires may achieve comparable outcomes, particularly when soft tissues are intact. Evidence showed that rigid fixation does not consistently give better outcomes, and early mobilization remains key to preventing stiffness. A tailored strategy using both absolute and relative stability principles can optimize function while minim

CASE: Two septuagenarian women presented with chronic periprosthetic scapular spine fractures after reverse total shoulder arthroplasty (RTSA) placement. Preoperative radiographs revealed a displaced unstable scapular spine fracture with scapula notching and onlay humeral position. One case additionally demonstrated uptilted baseplate placement. Both patients underwent concurrent revision RTSA with fixation of the scapular spine to correct component malposition and reduce glenohumeral tension. At 27-month follow-up, both achieved radiographic union, pain reduction, and functional gains without

This study biomechanically evaluated three titanium locking plate constructs for stabilizing a segmental defect model that simulated a comminuted femoral shaft fracture in cats. Four groups were formed: G1 - single locking plate; G2 - plate-rod construct; G3 - dual plate in an orthogonal configuration; and G4 - intact bone. G1 used 16-hole locking plate with bicortical screws placed in holes 1, 3, 7, 10, 14, and 16. In G2, an intramedullary pin was included, with monocortical screws in the same positions. In G3, screws were placed as in G1, except that monocortical screws were used in holes 7
