[Total carpometacarpal dislocations. Two case reports].

We report two cases of carpometacarpal dislocation of the four long fingers. They were diagnosed in emergency and treated by open reduction and stabilization by intramedullary pinning. They were followed for an average of 30 months. The results were rated good, and the two patients had an excellent grip strength, no deformity, no instability, no disabling or limited prehension. Dislocation or fracture-dislocation of the carpometacarpal joints are uncommon injuries. The diagnosis can be easily missed. The authors recommend closed or open reduction and fixation by pins and immobilisation in a pl

Drop episodes in Coffin-Lowry syndrome: exaggerated startle responses treated with clonazepam.

A 16-year-old girl had fully manifested Coffin-Lowry syndrome and drop episodes. Her drop episodes were precipitated by sudden unexpected tactile or auditory stimuli associated with the electrostatic circumstances in her leg muscles immediately after the stimuli. Studies revealed that her drop episode symptom was an unusual type of startle response and that it may be associated with Coffin-Lowry syndrome.

[ROC comparison of visualization of hand fractures using digital and conventional techniques].

PURPOSE: To compare the accuracy of digital luminescence radiography (DLR) and conventional film-screen radiography (FSR) in diagnosing fractures. MATERIAL AND METHOD: Both conventional and digital radiographs were acquired from a consecutive series of 57 patients with suspected wrist or hand fractures. The digital images were obtained with a 30% dose reduction. A ROC-analysis (receiver-operating characteristics) was performed. RESULTS: The area under the curve was 0.89 for conventional FSR, 0.93 for DLR, "gray scale" and 0.94 for DLR, "edge enhanced". CONCLUSIONS: Although its spatial reso

[Hand injury in polytrauma. A retrospective study of 782 cases].

The incidence of hand injuries in multiple trauma is not well investigated. Between 1980 and 1986, 782 multiple trauma patients received primary treatment at the University Hospital of Erlangen. At the time of the study, 22% (n = 173) of these patients had died. 93% of the patients met with a traffic accident. In 20% of the multiple trauma patients, additional hand injuries were seen. 75% of these injuries were closed fractures of hand and wrist. The severity of multiple trauma had no influence on the incidence of hand injury. Young people between twenty and fourty years of age were mostly aff

Reconstruction of the upper extremity with multiple microvascular transplants: analysis of method, cost, and complications.

During an 8-year period, 131 multiple microvascular transplants were performed on 62 patients for upper extremity reconstruction of complex wounds. Patients were classified into two groups: In group I 35 patients received paired simultaneous transplantation of 70 flaps performed in 35 operations. Group II consisted of 27 patients who underwent reconstruction with 61 sequentially transferred flaps; each procedure consisted of a single transplant. No statistical differences in flap failure or complications were seen between groups, except that group I required emergency reexploration more freque

Fractures of the phalanges of the hand in children.

Fractures of the phalanges of the hand were studied in 203 children. Fingertip fractures caused disability for three weeks but usually had good results. Comminuted fractures often took the form of longitudinal splitting. Epiphyseal fractures were mostly of the Salter type 2 and metaphyseal types and usually had good results: remodelling occurred in both planes at the base of the proximal phalanx but only in the sagittal plane at the base of the middle phalanx. Epiphyseal fractures at the base of the distal phalanx often had unsatisfactory results. Fractures of the neck of phalanx showed no rem

Open reduction for perilunate injuries--clinical outcome and patient satisfaction.

PURPOSE: Perilunate injuries cause severe carpal malalignment. Open reduction and internal fixation of these injuries has become the treatment of choice. This study evaluated clinical outcome and the patients' perception of disability in activities of daily living after open reduction, ligament reconstruction, and/or internal fixation of the scaphoid. In addition, potential prognostic factors for functional outcome and individual perceptions of disability were analyzed and compared with radiologic findings. METHODS: This study consisted of a retrospective analysis of patients with perilunate

Wrist arthrodesis using a Synthes wrist fusion plate.

Thirty-nine patients were retrospectively reviewed after a wrist arthrodesis using a Synthes wrist fusion plate and iliac crest bone graft. Information was obtained from review of patient files, a questionnaire to assess pain, function and work status, and clinical assessment of grip strength, forearm rotation and fingers motion. All wrist fusions united except that the index carpometacarpal joint failed to unite in one patient. Thirty-seven patients were satisfied with the procedure, noting a reduction in wrist pain after fusion, but all reported some limitation of function. The wrist fusion

Perilunate dislocation and fracture dislocation: a critical analysis of the volar-dorsal approach.

A combined volar-dorsal approach was used to treat 11 perilunate dislocations and fracture dislocations between 1989 and 1994. The mean average age of the patients was 38 years, and the mean average time between injury and surgery was 13 hours. Outcome was assessed after an average of 30 months. Results were based on measurements of grip strength, range of motion, radiographs, and patient satisfaction. Patient satisfaction was high in 9 of 11 patients. Seven had satisfactory pain relief, and 5 had returned to their previous occupation without limitation. The wrist flexion-extension arc and gri

A novel percutaneous achievement and maintenance of reduction and screw fixation for acute displaced scaphoid fractures: minimum two-year follow-up.

PURPOSE: The purpose of this study was to introduce a novel method of percutaneous achievement and maintenance of reduction for acute displaced scaphoid fractures and evaluate the feasibility of this method in treating acute displaced scaphoid fractures as well as explore its indications. METHODS: From February 2012 to March 2014, 15 patients with acute displaced scaphoid fractures were treated with our technique. Two Kirschner wires were used to achieve and maintain the reduction of the scaphoid fractures throughout the entire process of the traditional percutaneous screw fixation process. T

Volar radioscapholunate arthrodesis for malunited distal radius fracture with unsalvageable wrist articular degeneration: case report.

Fractures of the distal radius occur commonly, and volar plating has emerged as a current, successful, and popular treatment for this injury. When this technology fails, salvage procedures must be selected that aim to maximize wrist motion and minimize pain. We present a case of volar radioscapholunate wrist arthrodesis in the setting of unfeasible intraoperative volar osteotomy to treat a malunited distal radius fracture.

Dorsal intercarpal ligament capsulodesis for chronic, static scapholunate dissociation: clinical results.

The purpose of this study was to review and report the results of using a new procedure, the dorsal intercarpal ligament capsulodesis (DILC), to treat a group of patients with chronic flexible scapholunate dissociation. This was a prospective study of 21 patients (22 wrists), ranging in age from 16 to 62 years followed up for 1 to 4 years. For this study all patients returned to complete a questionnaire and have a physical examination performed by physicians and therapists independent of the treating surgeons and to obtain standardized radiographs of the wrists. Patient demographics, mechanism

Limited discriminatory value of RAMRIS findings between RA and hand osteoarthritis: a cross-sectional MRI study.

To evaluate the capacity of the RA MRI Score (RAMRIS) in distinguishing rheumatoid arthritis (RA) and hand osteoarthritis (HOA). Patients with RA and HOA were retrospectively matched in pairs. Inflammatory and degenerative changes in MRI scans of the hands were quantified and compared using RAMRIS, focusing on bone marrow edema (BME), erosion, synovitis, and tenosynovitis. CRP, ESR, and autoantibodies (RF and ACPA) and clinical scores (DAS-28, FFbH, and pain level), smoking status, and medication use were assessed additionally. Overall, 100 RA and 100 HOA patients with confirmed diagnosis were

Age-Associated Senescence of Decidual Macrophages: A Key Mediator of Adverse Pregnancy Outcomes in Advanced Maternal Age.

Senescence of immune cells can drive senescence of solid organs, and reversing immune cell senescence ameliorates organismal senescence phenotypes. As the second largest subset of decidual immune cells, macrophages play a critical role in pregnancy. However, whether decidual macrophages (DM) exhibit senescence in advanced maternal age (AMA) pregnancies (≥ 35 years old) and their involvement in AMA-related adverse outcomes remain undefined. This study elucidates the phenotypic and functional characteristics of DM in AMA pregnancies through clinical samples, animal models, and in vitro experimen
