[Doppler sonography of arteriovenous fistulas for haemodialysis].

BACKGROUND: Permanent and reliable vascular access is of utmost importance for patients (with chronic renal failure) in need of haemodialysis. Autogenous arteriovenous fistula (AVF) on the forearm are robust and have the benefits of providing long-term access with relatively few complications. The main problem is insufficient blood flow due to stenoses requiring early intervention. The AVF therefore needs surveillance with regular clinical examination supplemented with imaging. In recent years Doppler sonography has gained an increasingly important role in the assessment of AVFs. This paper di

Salient components of a comprehensive service for eating disorders.

Eating disorders are challenging and difficult to treat, because of the necessity of a multidisciplinary treatment team for effective outcomes and the high mortality rate of anorexia nervosa. An adequate initial assessment and evaluation requires a psychiatric assessment, a medical history and medical examination, a social history and an interview of family members or collateral informants. A comprehensive eating disorder treatment team includes a psychiatrist coordinating the treatment and appropriate medical physician specialists, nutritionists, and psychotherapists. An adequate outpatient e

[Patient orientation and reference to the ICF as challenges in outcome assessment in rehabilitation].

Measuring the results of rehabilitation interventions presents a number of issues regarding content and method, two of which have been selected for discussion in view of the findings of current research--the significance of patient orientation and the relation to the International Classification of Functioning, Disability and Health, ICF. Compilation of patients' treatment goals, expectations and preferences, patient participation in the development of assessment instruments, compilation of patients' evaluations of treatment results and consideration of the subjectivity of the physician as wel

Endometrial protection from tamoxifen-stimulated changes by a levonorgestrel-releasing intrauterine system: a randomised controlled trial.

INTRODUCTION: Tamoxifen is currently the most commonly used adjuvant treatment for breast cancer, however, it frequently causes episodes of unscheduled uterine bleeding, which could be associated with proliferative changes of the endometrium, or even endometrial cancer. We aimed to assess whether a levonorgestrel intrauterine system could modulate the uterine responses to tamoxifen. We also aimed to assess women's tolerance of the screening procedures, the insertion, removal, and potential side-effects of the device. METHODS: We did a randomised controlled trial, in which postmenopausal women

Validity of the PAS-ADD for detecting psychiatric symptoms in adults with learning disability (mental retardation).

The Psychiatric Assessment Schedule for Adults with Developmental Disability (PAS-ADD) is a semi-structured interview for use with respondents who have learning disability and for key informants. This report investigates the ability of the instrument to detect symptoms that had been found to exist during routine clinical assessment of the patients. Field trials involved 95 referred patients with learning disability and a key informant for each sample member. Clinical opinions of the referring psychiatrists were sought using a symptom checklist. Referrer checklist symptoms and PAS-ADD data were

Oral health-related quality of life and pain after periodontal regenerative therapy.

PURPOSE: The use of patient-centered assessments, including oral health-related quality of life (OHRQoL), alongside conventional clinical assessments, such as attachment level, has been recommended in periodontal treatment. This study surveyed OHRQoL to evaluate postoperative patient burden and pain after periodontal regenerative therapy. METHODS: The participants were 104 patients who underwent periodontal regenerative therapy using enamel matrix derivative (EMD), fibroblast growth factor-2 (FGF-2), or open flap debridement. OHRQoL was assessed with the General Oral Health Assessment Index (

[Communication Support for Neurological Disorders].

Communication support for patients with neurological disorders extends beyond high-technology communication aids. It also includes non-aided and low-technology methods and requires flexible selection and the combined use of these methods depending on the situation. Gaining experience with various communication strategies in a stepwise manner from an early stage enables the smoother introduction of advanced communication devices when necessary. Effective support must be tailored to the disease stage, as communication abilities and needs change over time. In this context, collaboration among mul

Negative pressure wound therapy made simple: expert panel recommendations.

OBJECTIVE: Wound management poses a substantial economic burden, and the prevalence of hard-to-heal (chronic) wounds is a growing health concern. Negative pressure wound therapy (NPWT), with and without instillation, is an adjunctive therapy commonly used to manage acute and hard-to-heal wounds and has demonstrated positive clinical outcomes. The variety of NPWT modalities, patient needs, wound characteristics and care requirements can make selecting the optimal system challenging, leading to uncertainty and inconsistent practices among new users, increasing the risk of suboptimal outcomes. Antimicrobial resistance and the human gut microbiome-a food safety perspective.

The gastrointestinal environment is where the resident gut microbiome encounters foodborne microorganisms, antimicrobial resistance genes (ARGs), and bioactive substances from food, all of which may influence the acquisition and dissemination of antimicrobial resistance (AMR). Although resistant bacteria and ARGs are frequently detected in food and food production environments, their contribution to the gut resistome remains unclear. Most ingested microbes are transient and constrained by ecological barriers; however, the conditions that enable horizontal gene transfer in vivo are not well cha

Discrimination and Symptoms of Post-Traumatic Stress Among Black Transgender Women in the United States: The Moderating Effect of Sleep.

Background: Black transgender women experience high rates of intersectional discrimination contributing to post-traumatic stress disorder (PTSD) symptoms. While sleep typically buffers psychological distress among general populations, these relationships remain underexplored among Black transgender women, and existing protective sleep literature derives primarily from non-Hispanic White, cisgender, socioeconomically advantaged populations. Methods: This exploratory secondary cross-sectional analysis of 155 Black transgender women (aged 18+) examined whether sleep quality (Pittsburgh Sleep Qual

Pain management in the perioperative orthogeriatric setting: Recommendations of the special interest group Orthogeriatrics of the German Geriatrics Society.

Management of perioperative pain in older adults can be challenging. Multimorbidity, functional decline or diminished physiological reserves, the corresponding polypharmacy, age-related pharmacodynamic and pharmacokinetic changes need to be considered. Pain assessment can be complicated by cognitive impairment and communication barriers. Older adults are at high risk of adverse consequences from surgery and undertreated pain as well as pharmacological overtreatment. Effective pain control in the perioperative period needs to be balanced against potential pharmacological harm. The aim of the or

Multimodality Imaging for Left Atrial Appendage Occlusion Devices.

PURPOSE OF REVIEW: This review summarizes the current and evolving landscape of multimodality imaging for percutaneous endocardial left atrial appendage occlusion (LAAO) across three phases: preprocedural planning, periprocedural guidance, and postprocedural follow-up. RECENT FINDINGS: Transesophageal echocardiography (TEE) remains the primary imaging modality to assess the left atrial appendage (LAA) in all three phases of clinical care surrounding LAAO, though many innovations have led to the rise of cardiac computed tomography (CCT) and intracardiac echocardiography (ICE) as powerful adjunc

Exploring the core competencies of geriatric nursing among township nurses in China: a qualitative study using the iceberg model.

Rural regions of China are facing a more severe population aging problem than urban areas, and the diverse health and care needs of rural older adults bring great challenges to frontline nurses working at township hospitals. At present, a targeted core competency system for township nurses engaged in geriatric nursing has not been systematically constructed in China. This qualitative descriptive study adopted the Iceberg Model as the theoretical framework. Semi-structured interviews were conducted with six nurses and four physicians from nine township hospitals in Jiangsu Province. A total of

Clinical Features of Hypo- and Hypervitaminosis of Fat-Soluble Vitamins in Pediatric Patients.

Fat-soluble vitamins (A, D, E, and K) are crucial for pediatric health, contributing to normal cellular function, growth, immune defense, and development. Unlike water-soluble vitamins, they are absorbed with dietary fats and stored in the liver and adipose tissue, leading to risks of deficiency (hypovitaminosis) and toxicity (hypervitaminosis) in certain physiological and pathological conditions. This narrative review aimed to summarize the clinical manifestations, diagnostic considerations, and management of hypo- and hypervitaminosis of fat-soluble vitamins in pediatric populations. Broca's biases in modern aphasiology.

Broca's description of aphemia signals the emergence of aphasiology as a discipline. It also marks the beginning of a long tradition of studies of right-handed patients ("Nous parlons avec l'hémisphère gauche") with lesions in the inferior frontal gyrus (Broca's area) displaying production deficits (Broca's aphasia) in their single, mainly Romance or, as years went by, Germanic language. In modern-day aphasiology, language crosses hemispheric boundaries, production is no longer confined to the left inferior frontal gyrus, and lesions in Broca's area do not necessarily entail Broca's aphasia, w
