Postoperative endophthalmitis.

Postoperative endophthalmitis remains a serious clinical problem in ophthalmology, with an incidence of approximately 0.5%. Prognosis is largely determined by the virulence of the offending organism. The Endophthalmitis Vitrectomy Study (EVS) was a prospective, randomized trial comparing various diagnostic and treatment modalities in cases of endophthalmitis that followed cataract surgery. The EVS found that vitrectomy was only beneficial for patients presenting with very poor visual acuity and that intravenous antibiotic treatments had no additional benefit, compared with intravitreal antibio…

Obstruction of the lacrimal drainage system.

This review covers evolving concepts in lacrimal outflow obstruction. Recent studies have increased our understanding of the surgical anatomy and pathophysiology of the lacrimal drainage system through radiologic, clinical, and microbiologic techniques. While external dacryocystorhinostomy remains an important treatment for nasolacrimal duct obstruction, there have been a number of therapeutic developments worthy of review, including advances in endoscopic and transcanalicular dacryocystorhinostomy, conjunctivodacryocystorhinostomy, and the use of mitomycin C in these procedures. In addition,

Management of ectopia lentis in children.

Marfan syndrome, homocystinuria, trauma, and simple ectopia lentis are the most common causes of pediatric lens subluxation. Many patients are best treated with a careful refraction of the phakic or aphakic pupillary axis. For those patients not achieving satisfactory refractive results, endocapsular lensectomy using modern automated vitreous cutting devices is relatively safe and successful. Given the lack of postoperative capsular support and our current level of intraocular lens technology, postoperative optical correction is most safely achieved with aphakic contact lenses or spectacles.

Recent developments in vitreoretinal surgery.

In the past three decades, a great improvement has occurred in microsurgical techniques used in the management of various eye diseases involving retina and vitreous. Advances in instrumentation has made the surgery easier and refined. The instruments used are narrated widely in this article. Various vitreous substitutes have been developed and are required in vitreoretinal surgery. These are gases, silicone oil, perfluorocarbon liquids and fluorosilicone oil. Anterior segment indications for vitreous surgery are: Vitreous loss during cataract surgery, thick after cataracts that can't be manage

Surgical management of seventh nerve paralysis and floppy eyelid syndrome.

A seven-stage approach to the management of the paretic eyelid complex has been described. These stages include supportive care, planning and execution of general facial reanimation, lower eyelid and canthal resuspension or support, passive upper eyelid animation, dynamic lid animation, and soft tissue repositioning. A final stage, the epilogue, is described for the treatment of the synkinesis and hypertonicity that often develop. Recent developments in these areas are discussed. Floppy eyelid syndrome, first described by Culbertson and Ostler, is a syndrome of chronic papillary conjunctivitis

The ocular complications of periocular laser surgery.

Laser periocular surgery has achieved an increased popularity, especially since recent technical developments in the carbon dioxide and erbium:yttrium-aluminum-garnet lasers. Despite the relative safety of these procedures in experienced hands, postoperative complications affecting the periocular region, and the eye itself, may follow laser surgery. Common complications include persistent erythema, hyper- and hypopigmentation, and hypertrophic scarring. Skin infection (viral, bacterial, or fungal) may also jeopardize the postoperative period after periocular laser treatment. Severe burns, tran

Paralytic strabismus.

In the past 12 months there have been many interesting publications on paralytic strabismus. Many investigators have reported on various imaging techniques helping both the clinician and the basic scientist better evaluate the causes and effects of paralytic strabismus on the extraocular muscle environment. Some interesting cases of neurologic and systemic associations have also been reported, including a unilateral pupil-sparing third nerve paralysis developing after the use of sildenafil citrate (Viagra; Pfizer, New York, New York). Advances in genetics are now helping us further understand

Outcome criteria in strabismus surgery.

Criteria for success in strabismus surgery should include more than just the alignment result in primary position. It is important to consider as well the extent of the field of BSV gained: restoring a useful range of BSV will maximize the patient's ability to carry on daily activities. Psychometric tests can also be used to provide objective measures of improvement following strabismus surgery. Irrespective of the patient's age, the realigning of an eye is reconstructive, not cosmetic, surgery. Surgery in children and adults can restore fusion and normalize the field of BSV in most cases. The

Measurement of globe position in complex orbital fractures. I. A modification of Hertel's exophthalmometer, using the external auditory canal as a reference point.

Where there has been displacement or removal of the lateral orbital rim or zygomaticomaxillary complex secondary to trauma or surgery, the measure of globe position by the Hertel exophthalmometer is problematic and inaccurate. A modification of Hertel's instrument that uses the external auditory canal (EAC) rather than the orbital rim as reference point has been developed. Measurement of globe position of 120 normal volunteers was obtained with the Hertel and the modified EAC-fixated exophthalmometer. Although the mean absolute difference between right and left measurements for both methods (H

A clinical evaluation of the potential acuity meter in 210 cases.

The potential acuity meter (PAM) has been reported to be a useful instrument for predicting postoperative visual acuity after cataract extraction surgery. In this study, 210 eyes were tested with the PAM before surgery, and the best postoperative visual acuity was obtained. The PAM accurately predicted the postoperative visual acuity to within three lines in 85.2% of the cases. Further, the PAM was able to predict whether or not an operation would be successful (success defined as a final visual acuity of 20/40 or better) in 91.6% of the cases. It was found that senile macular degeneration may

Pars plana vitrectomy. Microsurgical pars plana lensectomy.

Pars plana lensectomy, wherein the entire lens within the pupillary area (the anterior capsule, anterior cortex, nucleus, posterior cortex, and posterior capsule) is removed, is now a well-established technique. It is to be considered as a method of choice for the removal of presenile cataracts in patients under the age of 40, developmental cataracts, cataracta complicata secondary to uveitis, subluxated and luxated cataracts. Pars plana lensectomy of the senile cataract is limited to a grade 2 nuclear sclerosis on a scale of 4. The use of ultrasonic instrumentation via the pars plana in conju
