Sickle cell disease (SCD) is a hereditary red cell disorder with clinical manifestations secondary to sickling or crescent-shaped distortion of the red blood cells. Musculoskeletal complications of SCD are often the main causes for acute and chronic morbidities in children with manifestations including osteomyelitis, osteoporosis and osteonecrosis. This article aims to familiarise the paediatric radiologist with appendicular skeletal complications of SCD in the paediatric population and their imaging appearance.

Lower extremity soft-tissue lesions are frequently encountered in clinical practice. Cellulitis, osteomyelitis, and diabetic foot ulcerations remain the top differentials. The acuity of illness, imaging studies, and deep wound cultures are helpful in most cases. Malignancies are rare but need immediate attention. Hereby, we report a case of a chronic heel wound which on workup was confirmed as cutaneous squamous cell carcinoma (SCC).

INTRODUCTION: Wohlfahrtiimonas chitiniclastica has been associated with open wound infections, cellulitis, osteomyelitis, and bacteremia.
CASE PRESENTATION: We report the case of a 41 year old woman with history of congenital myelomeningocele, paraplegia and extensive decubitus ulcers that developed W. chitiniclastica bacteremia secondary to an infected ulcer.
CONCLUSION: W. chitiniclastica is an emerging human pathogen that can be readily identified by MALDI-TOF or sequencing.

CASE: We report a case of melioidotic septic arthritis of the ankle and secondary osteomyelitis of the talus in a 64-year-old farmer with diabetes mellitus. Arthroscopic drainage and debridement, followed by 6 months of appropriate antibiotic therapy, resulted in a good short-term outcome.
CONCLUSION: Melioidotic septic arthritis of the ankle is extremely rare. This case report highlights the possibility of this disease occurring on the Indian subcontinent.

This report describes an 11-year-old girl with systemic lupus erythematosus (SLE) with long-standing low levels of complement proteins. A disease period with lupus nephritis (class IIIa) was complicated by Staphylococcus aureus bacteraemia and osteomyelitis. She was treated with high-dose immunosuppressants and 6 weeks of high-dose intravenous antibiotics. The clinician should be aware of bacteraemia in SLE with secondary complement deficiency.

Musculoskeletal infections are commonly encountered in clinical practice. This review will discuss the ultrasound appearances of a variety of musculoskeletal infections such as cellulitis, infective tenosynovitis, pyomyositis, soft-tissue abscesses, septic arthritis, acute and chronic osteomyelitis, and post-operative infection. The peculiar sonographic features of less common musculoskeletal infections, such as necrotizing fasciitis, and rice body formation in atypical mycobacterial tenosynovitis, and bursitis will also be presented.

"Toddler's fracture" can be difficult to diagnose but should be suspected whenever a child presents to the accident and emergency department with a limp or fails to bear weight on the leg. Irritable hip and subacute osteomyelitis must feature in the differential diagnosis. The history may or may not include an obvious traumatic episode. Rather than fracture, elastic bowing of the bone and consequent periosteal stripping may explain symptoms in some cases.

Secondary hyperparathyroidism is ubiquitous in patients with end-stage renal disease who are on dialysis. We report a dialysis patient with an unusual manifestation of hyperparathyroidism. Initially, the patient was misdiagnosed to have clavicular osteomyelitis. The underlying hyperparathyroidism was not recognized, only to be discovered later, when the patient presented with spastic paraplegia and massive ectopic calcification in the cervical spine. The patient made a complete recovery following surgical resection and parathyroidectomy.

The authors reported here a case of dominant form of osteopetrosis. The patient was a 28 years old man hospitalized for a mandibular osteitis consecutive to a tooth extraction. The clinical examination detected a splenomegaly. The laboratory tests showed anemia and thrombopenia. A radiographic skeletal survey detected a typical generalized osteocondensation. The authors pointed out the radiologic signs of the disease and underscored the difficulties in managing osteomyelitis of the jaw in such patients.

The work analyses 55 cases of traumatic osteomyelitis of the leg bones, open fractures of the leg bones, and medial fractures of the femoral neck. Patients with medial fractures of the neck of the femur were treated by endoprosthesis after Mur-Kaplan. Endolymphatic antibiotic therapy was included in the complex of therapeutic measures in each group of patients. This method allowed suppuration of the postoperative wounds to be avoided and promoted in this manner rehabilitation of the patients.

Septic arthritis is a recognised complication of rheumatoid arthritis but has not been well described in sero-negative spondarthritis. We report 3 patients with sero-negative inflammatory joint disease who developed joint sepsis early in the course of the disease. In none was there a recognisable source of infection. Two cases were complicated by osteomyelitis and in one the sepsis has been recurrent over thirty years.

The efficacy of sodium dicloxacillin was studied in the treatment of pyo-inflammatory diseases, such as pneumonia, pyothorax, pulmonary abscess, osteomyelitis, burn disease, infections of the skin and soft tissues and others, caused by multiresistant staphylococci and other organisms sensitive to the antibiotic. The drug was effective both orally or parenterally. Satisfactory results were observed in more than 80 per cent of the patients (children and adults). It was well tolerated by the patients.

50 children with bone and joint infections (acute osteomyelitis, suppurative acute arthritis, osteoarthritis) were treated with pristinamycin. The responsible bacterium was most often but not exclusively Staphylococcus aureus; its sensitivity to various antibiotics is discussed. The value of biochemical tests for diagnosis and follow-up is emphasized; the potency of this drug avoids the need for intravenous therapy. A therapeutic protocol according to age, type of and sensitivity of the germs is proposed.

Radionuclide bone imaging of the skeleton, now well established as the most important diagnostic procedure in detecting bone metastases, is also a reliable method for the evaluation of the progression or regression of metastatic bone disease. The article concentrates on the technetium-99m agents and the value of these agents in the widespread application of low-dose radioisotope scanning in such bone diseases as metastasis, osteomyelitis, trauma, osteonecrosis, and other abnormal skeletal conditions.
