A patient with Hunter's Syndrome (systemic mucopolysaccharidosis type II) was evaluated for bilateral uveal effusion syndrome. The right eye showed a circular peripheral choroidal detachment for 360 degrees. The left eye showed a larger circular peripheral choroidal detachment and an exudative retinal detachment. Evaluation of the sclera during combined sclerectomies and sclerostomies of the left eye revealed markedly thickening sclera and a reduced number of vortex veins. The surgery resulted in complete resolution of the choroidal detachment and exudative retinal detachment of the left eye.

Patients with macular disciform degeneration may undergo spontaneous improvement in visual acuity of the first affected eye when the second eye later undergoes a decrease in vision. Three hundred twenty-nine cases of age-related macular degeneration (AMD) and presumed ocular histoplasmosis syndrome (POH) were examined. During vitreoretinal procedure, 26 aphakic eyes with proliferative vitreoretinopathy and diabetic traction retinal detachment were treated with a new technique for surgical dilatation of a miotic immobile pupil. Unilateral macular infarction occurred in a 23-year-old black man with sickle cell hemoglobinopathy (SS), who was already receiving oxygen and hydration treatment for a painful crisis. Visual acuity of 20/200-1 in the affected eye did not improve with 36 hours of "conservative" treatment. Fifteen hours after an exchange transfusion, however, visual acuity started to improve. A series of 24 consecutive patients presenting with a fundus picture characterized by a predominance of cotton-wool spots, or a single cotton-wool spot, is reported. The authors evaluated the limitation of iridocyclectomy in primate eyes. Six-clock-hour resections were tolerated and the intraocular pressure was reestablished to preoperative levels (17 mm Hg) within 1 month after surgery. Seven- to eight-clock-hour resections were also tolerated by the eyes, but the intraocular pressure remained low (7-10 mm Hg) for the extent of the study (5 months). Eyes in which the vitreous fluid was replaced with Healon showed a trend toward faster recovery of the intraocular pressure than those having a physiologic saline solution.

A live, subretinal fly larva was observed in a 16-year-old Caucasian female. Argon-laser photocoagulation killed the larva without a destructive inflammatory response. The patient's vision improved from 20/200 to 20/20 as the eye recovered. Clinical photographs demonstrate the larva and the acute and later healed pathopneumonic subretinal tracks. The laser photocoagulation technique is described.

A 52-year-old Japanese woman had retinal crystalline deposits with atrophy of the pigment epithelium and choriocapillaris and pigment clumps in the periphery. The patient did not have corneal crystals. The crystals were seen biomicroscopically in all layers of the retina and on the retinal vessels. The diagnosis of crystalline retinopathy was made. The progressive nature of the disease was confirmed, based on a 9-year follow-up. The authors reviewed this condition in the Japanese literature and believe that crystalline retinopathy may not be as rare in the Orient as previously thought.

A 60-year-old white man with recurrent nondeforming arthritis and no intestinal symptoms developed bilateral vitreous opacities strongly suggestive of Whipple's disease. He was successfully treated with tetracycline and erythromycin. Thirteen years later, a presumed diagnosis of Whipple's disease was established by histopathologic observation of macrophages that were distended by periodic acid-Schiff-positive material and the finding of bacilli by electron microscopy in a vitreous aspirate. The characteristics of ocular involvement in Whipple's disease are discussed.

Ophthalmologists do not consider pars planitis to occur on a familial basis. The development of pars planitis in nine members of four different families is reported. Affected family members include twin sisters, a mother and daughter, two brothers, and a sister and two brothers from the same family. This suggests that a common hereditary and/or environmental factor contributed to the disease in these patients.

The cases of 23 patients with untreated cilioretinal artery obstruction were reviewed. Three distinct groups were found: (1) isolated cilioretinal artery obstruction, (2) cilioretinal artery obstruction associated with central retinal venous obstruction, and (3) cilioretinal artery obstruction in conjunction with ischemic optic neuropathy. In the first group 90% of eyes achieved 6/12 or better vision and there was a high incidence of associated atherosclerotic carotid disease. Spontaneous release of a taut, fibrotic subretinal strand was observed in a patient with rhegmatogenous retinal detachment. This strand had been elevating the retina, and its release led to partial reattachment. Whereas spontaneous resolution of preretinal membrane contraction in retinal detachment has been reported, this is the first report of spontaneous release of such a subretinal fibrotic strand. The manner in which the strand released raised the question of whether efforts should be made to rupture such strands with transcleral pressure prior to considering retinotomy.

A combination of various electrophysiologic tests--simultaneous recording of VER and ERG, EER, and single-flash ERG--is proposed to assess visual function in eyes being considered for vitrectomy. It is hypothesized that the primary underlying cause of the idiopathic uveal effusion syndrome is a congenital anomaly of the sclera, and in some cases, the vortex veins. Superimposed aging and hormonal changes in the sclera and its emmissary channels impair its permeability to protein and predispose the eye to vortex vein obstruction. The inability of the eye to transport extravascular protein across the abnormal sclera is probably the cause of prolonged exudative detachment of the uvea and retina in these patients.
