Heavy alcohol consumption among marginalised African refugee young people in Melbourne, Australia: motivations for drinking, experiences of alcohol-related problems and strategies for managing drinking.

OBJECTIVE: Little is known about substance use among resettled refugee populations. This study aimed to describe motivations for drinking, experiences of alcohol-related problems and strategies for managing drinking among marginalised African refugee young people in Melbourne, Australia. DESIGN: Face-to-face interviews were conducted with 16 self-identified African refugees recruited from street-based settings in 2012-2013. Interview transcripts were analysed inductively to identify key themes. RESULTS: Participants gathered in public spaces to consume alcohol on a daily or near-daily basis.

A mosque-based methadone maintenance treatment strategy: implementation and pilot results.

BACKGROUND: This paper describes the rationale, implementation and operation of a "world first" Islamic inspired methadone maintenance treatment project delivered in a mosque setting and presents the outcome for the first group of participants. The project explored the viability of expanding addiction recovery services through the network of mosques in Muslim communities. METHODS: The project combined methadone maintenance with peer and religious counseling. Participants consisted of 36 male Muslim heroin users who went through the project. Urine tests and self-reported measures on various di

Application of a nurse-managed inpatient smoking cessation program.

This study reports on the effectiveness of a nurse case-managed smoking cessation program for general hospitalized patients that was continued for 3 years after clinical trials were completed. Patients admitted to the hospital who smoked were offered a smoking cessation program during their hospitalization. The program included physician advice, bedside education and counseling with a nurse specially trained in smoking cessation techniques, take-home materials (videotape, workbook, and relaxation audiotape), nicotine replacement therapy if requested or indicated, and four nurse-initiated post-

N-of-1 Analytics Makerspace: A Prototype for Cystic Fibrosis Self-Care and Co-Care.

We have designed a prototype N-of-1 analytics makerspace, which is a collaborative work environment that provides a space for different stakeholders in healthcare to learn new skills and work together on projects that can improve individual patient care and the effectiveness of healthcare systems. Our prototype was designed to study the use of antibiotics in self-management for children with cystic fibrosis in Sweden but is intended to be disease agnostic and potentially include other complex medical conditions in the future.

European Academies Advise on Gain-of-Function Studies in Influenza Virus Research.

Gain-of-function (GoF) studies to understand factors affecting transmissibility of potentially pandemic pathogens are controversial. The European Academies Science Advisory Council (EASAC) recently published consensus recommendations relating to GoF research review and management on self-regulation and harmonization; bioethical considerations; benefit-risk assessment; biosafety, and biosecurity advisory options; and publication of sensitive information. A layered approach to integration of responsibilities must include conforming to the stringent rules and guidance already existing. Further co

Impacted stapler pin in fractured maxillary central incisor with open apex: Advanced endodontic management using biodentine as innovative apical matrix.

The presence of foreign objects in the pulp chamber of fractured permanent teeth is a rare phenomenon and often diagnosed accidently .These foreign bodies are most commonly self inflicted by young patients and remain impacted within the pulp canal thereby acting as potential source of infection and painful conditions. In the present case report, we present successful endodontic management of stapler pin lodged in fractured maxillary central incisor with challenge of open apex in young patient using biodentine as a novel apical matrix.

Potassium hydroxide 5% for the treatment of molluscum contagiosum.

Molluscum contagiosum is a common reason for consultation in primary care. The condition is normally benign and self-limiting1 and the standard advice is to wait for the lesions to resolve spontaneously.2 Recently, potassium hydroxide 5% (MolluDab-Alliance Pharmaceuticals Limited) has been marketed in the UK for the treatment of the condition.3 It is sold as a medical device rather than a licensed medicinal product. Here we consider the evidence for potassium hydroxide 5% in the management of molluscum contagiosum.

Norovirus infection in immunocompromised hosts.

Acute gastroenteritis caused by noroviruses often has a duration of 2-3 days and is characteristically self-limiting. In contrast, chronic infection caused by noroviruses in immunocompromised individuals can last from weeks to years, making clinical management difficult. The mechanisms by which noroviruses establish persistent infection, and the role of immunocompromised hosts as a reservoir for noroviruses in the general human population, are not known. However, study of this patient cohort may lead to new insights into norovirus biology and approaches to treatment.

Prepubertal Hypertransfusion in Thalassemia Intermedia: A Case Report of Sustained Positive Effects on Growth, Splenic Function and Endocrine Parameters.

We report a known case of thalassemia intermedia (Hb Dhofar) who in spite of mild thalassemic facies, attained his normal genetic height, pubertal maturity and improved self-image with minimal splenomegaly through a hypertransfusion/chelation regimen that was started just before puberty and maintained for 5 years. As there are no clear guidelines in the management of patients with thalassemia intermedia, the option of hypertransfusion/chelation during the pubertal growth spurt may alleviate some of the complications associated with thalassemia intermedia.

Acute symptomatic intervertebral disk calcification in a child with retropharyngeal edema: computed tomography and magnetic resonance findings.

Intervertebral disk calcification in children is an uncommon self-limiting disease, which can cause symptoms like neck pain or torticollis, and can be treated with conservative management. The calcified disk material can herniate anteriorly, inducing dysphagia, or herniate posteriorly, causing neurologic symptoms secondary to spinal cord compression. We report computed tomography and magnetic resonance findings of a symptomatic intervertebral disk calcification at the C2-3 level with retropharyngeal edema caused by anterior herniation of calcified material in a 7-year-old boy.

What is Central Toxic Keratopathy Syndrome if it is not Diffuse lamellar Keratitis Grade IV?

The Central Toxic Keratopathy (CTK) syndrome describes a rare, acute, self-limited, non-inflammatory process that yields central corneal opacification and significant hyperopic shift after refractive surgery. Despite being exceedingly rare, certain clinical features of CTK give the condition a striking resemblance to other more serious inflammatory conditions, including diffuse lamellar keratitis (DLK). As the authors demonstrate in this article, despite the overlapping clinical features, CTK is a disease process that is distinct from DLK and, therefore, in need of distinct management interven

Cryptosporidiosis: environmental, therapeutic, and preventive challenges.

Cryptosporidium spp. are responsible for endemic and epidemic disease worldwide. Clinical manifestations may include acute, persistent, or chronic diarrhea, biliary, and pulmonary disease. Disease severity ranges from asymptomatic or mild to severe, intractable diarrhea with wasting depending on immune status, nutrition, and age. Transmission is fecal-oral with both human and animal reservoirs. Disease is often self limited in healthy individuals, but therapy remains a challenge in the immune-compromised. Prevention currently depends on appropriate hygiene and proper water management and treat

Natural remedies recommended for the management of oral health.

This paper discusses various practices recommended by homeopaths, herbalists, spiritual and natural healers for the management and maintenance of oral health. It is intended as a partial guide to educate the dental professional on self-administered, over-the-counter remedies that are easily available to the general population. Since few if any clinical studies exist assessing the efficacy or side effects of these ingested products, little is known about the beneficial or potentially harmful course these remedies may take on the human body.

Denial of hostility and psychopathology in the evaluation of child molestation.

We administered the Buss-Durkee Hostility Inventory (Buss & Durkee, 1957) and the Minnesota Multiphasic Personality Inventory (Hathaway & McKinley, 1967) to 82 alleged child molesters. Denial of hostility was significantly associated both with minimization of psychopathology and with denial of charges of sex offense. No subjects who denied charges acknowledged psychopathology, whereas one third of admitters acknowledged psychopathology. Our results suggest that alleged sex offenders may show complex patterns of impression management and that self-report hostility inventories are highly vulnera

[Immune granulocytopenia in children].

Immune neutropenia in infancy comprises alloimmune neutropenia and autoimmune neutropenia. Both diseases are caused by granulocyte-specific antibodies. Patients are newborns and children, often being younger than three years. Neutropenia usually leads to recurrent mild bacterial infections. Beside the blood count and the bone-marrow smear demonstration of granulocyte-specific antibodies is typical. As neutropenia is self-limiting, most patients require only conservative management. In more severe cases interventions with high-doses of intravenous immunoglobulin or corticoid-therapy are indicat

Diabetes mellitus in elderly persons.

The person who develops diabetes late in life needs carefull assessment of his abilities to assume the new self-care activities required by the advent of a chronic disease. Knowledge of both the aging process and the complications and management of diabetes is needed to assist him toward this goal. His commitment may have to be to a part of the whole plan, but to the elderly person, any participation will provide a measure of independence and control over his life.

Clinical Translation of the 2024 AO/AAP Consensus on Prevention and Management of Peri-implant Diseases and Conditions.

As the number of placed implants increases exponentially worldwide, the prevalence of peri-implant diseases, namely mucositis and peri-implantitis, is also in an ascending trajectory. Focusing on prevention and identifying and decreasing risk factors are important aspects of managing the epidemic of peri-implant diseases. However, many clinicians are faced with patients who already have signs of inflammatory disease in varying degrees of severity. Treatment of peri-implant mucositis involves addressing the etiologic factors causing local inflammation, as well as improving self-performed oral h
