Comparative study of open and arthroscopic coracoid transfer for shoulder anterior instability (Latarjet)-computed tomography evaluation at a short term follow-up. Part II.

PURPOSE AND HYPOTHESIS: The aim of this study was to evaluate and to compare the radiological parameters after arthroscopic and open Latarjet technique via evaluation of computed tomography (CT) scans. Our hypothesis was that the radiological results after arthroscopic stabilisation remained in the proximity of those results achieved after open stabilisation. MATERIAL AND METHODS: CT scan evaluation results of patients after primary Latarjet procedure were analysed. Patients operated on between 2006 and 2011 using an open technique composed the OPEN group and patients operated on arthroscopic

Machine Learning-driven Probability Calculators Can Accurately Predict 1-year Mortality After Proximal Humerus Fractures in Patients Over the Age of 65 Years.

BACKGROUND: Proximal humerus fractures (PHFs) in patients ≥ 65 years of age are associated with increased risk of death in the months after injury. Controversy exists regarding the preferred treatment strategy in these patients, and operative treatment is associated with high complication and reoperation rates. Machine learning (ML)-driven probability calculators for mortality prediction therefore may be valuable during shared decision-making for surgeons and patients. QUESTIONS/PURPOSES: (1) To develop ML algorithms to predict 1-year mortality in patients ≥ 65 years of age. (2) To externally

Arthroscopic, Implant-Free Bone-Grafting for Shoulder Instability with Glenoid Bone Loss.

UNLABELLED: Anterior shoulder instability is commonly associated with injuries to the labrocapsular structures of the glenohumeral joint and with osseous glenoid defects of varying degree. If the bone defect exceeds a clinically relevant threshold, mere soft-tissue repairs result in high rates of recurrent instability and the glenoid needs to be reconstructed with osseous augmentation. The most popular techniques for glenoid reconstruction include variations of coracoid transfer or bone-block procedures; however, these procedures are not free from risks and complications, which are often relat

Surgical Treatment for Osteochondral Lesions of the Talus.

Osteochondral lesions of the talus (OLT) are often associated with ankle pain and dysfunction. They can occur after ankle trauma, such as sprains or fractures, but they usually present as a continued ankle pain after the initial injury has resolved. Chronic ankle ligament instability and subsequent microtrauma may lead to insidious development of an OLT. Medial-sided lesions are more common (67%) than lateral-sided lesions. For acute lesions that are nondisplaced, nonoperative management is initially performed, with a 4-6 week period of immobilization and protected weight bearing. Symptomatic

Medium Term Reoperation Rates after Primary Total Elbow Arthroplasty at a Single Institution: 303 Cases.

INTRODUCTION: Total elbow arthroplasty (TEA) is a definitive treatment for a broad spectrum of elbow pathology; however, durability remains limited by infection, aseptic loosening, and mechanical failure. As the indications for TEA continue to shift away from auto-immune etiologies and toward traumatic indications, it is important to assess the modern state of TEA long-term outcomes. However, large-scale contemporary data examining mid to long-term outcomes on TEA remains limited. The purpose of this study was to evaluate indications, complications and reoperation patterns following TEA. METH

Optimizing periprosthetic fracture management and in-hospital outcome: insights from the PIPPAS multicentric study of 1387 cases in Spain.

BACKGROUND: The incidence of all periprosthetic fractures (PPF), which require complex surgical treatment associated with high morbidity and mortality, is predicted to increase. The evolving surgical management has created a knowledge gap regarding its impact on immediate outcomes. This study aimed to describe current management strategies for PPF and their repercussions for in-hospital outcomes as well as to evaluate their implications for the community. METHODS: PIPPAS (Peri-Implant PeriProsthetic Survival Analysis) was a prospective multicentre observational study of 1387 PPF performed dur

Functional Recovery and Safety of a Novel Hyaluronic Gel/Alginate Sheet for Complex Hand Injuries.

INTRODUCTION: Adhesion formation following hand and microsurgical procedures is a relatively common but often substantial complication, potentially limiting mobility and impairing recovery. A bioresorbable hyaluronic acid (HA)/alginate hydrogel was developed to reduce this risk. This study aimed to 1) evaluate functional recovery after use of this novel hydrogel with grip strength and range of motion (ROM), 2) determine its effect on patient-reported outcomes and pain, and 3) assess its safety and complication profile. METHODS: A prospective, single-center cohort study was conducted in 17 pat

Restoration of internal rotation after reverse shoulder arthroplasty may vary depending on etiology in patients younger than 60 years of age: a multicenter retrospective study.

BACKGROUND: Reverse shoulder arthroplasty (RSA) offers promising functional outcomes for young patients, yet challenges persist in restoring internal rotation (IR). This study aimed to assess the restoration of IR after RSA in patients younger than 60 years of age and analyze the factors affecting IR recovery. METHODS: A retrospective multicenter study was conducted, examining the functional outcome of patients who underwent RSA, with a minimum follow-up period of 2 years. Two subgroups of patients who underwent primary RSA were analyzed separately with respect to active internal rotation wit

Efficacy of a thermoplastic mask and pneumatic abdominal compression device for immobilization in stereotactic ablative radiotherapy of spine metastases.

Stereotactic ablative radiotherapy (SABR) has become a key technique in management of spine metastases. With improved control over treatment plan dosimetry, there is a greater need for accurate patient positioning to guarantee agreement between the treatment plan and delivered dose. With serious potential complications such as fracture and myelopathy, the margins of error in SABR of the spine are minimal. In this study, we assessed the performance of two patient immobilization setups in SABR for spinal metastases. First, a Type-S head and shoulders mask (CQ Medical, Avondale, PA), and second,

Free fillet flap: Outcomes in emergency microsurgical reconstruction of upper and lower limb.

INTRODUCTION: Despite its infrequency, 'spare-part surgery' represents a reconstructive strategy that uses segments of amputated limbs as donor tissue to preserve function in other injured anatomical regions. The purpose of this study was to review our 14-year experience in emergency microsurgical reconstruction of traumatised limbs using free fillet flaps obtained from non-salvageable amputated parts. MATERIALS AND METHODS: A retrospective observational study was conducted on patients who underwent urgent free fillet flap transfer for upper or lower limb reconstruction between 2007 and 2021.

Open Foveal Repair of the Triangular Fibrocartilage Complex Tears Associated with Symptomatic Ulnar Styloid Non-union.

Background: Symptomatic ulnar styloid non-union can be treated by excision of the ulnar styloid fragment. For combined triangular fibrocartilage complex (TFCC) tears, several repair techniques such as arthroscopic repair, open repair to the fracture site or reconstruction using a tendon graft have been introduced. This study reports the technique and outcomes of open foveal repair of the TFCC with excision of the ulnar styloid fragment in patients with symptomatic ulnar styloid non-union and distal radioulnar joint (DRUJ) instability. Methods: Consecutive patients with symptomatic ulnar styloi
