General surgery problems in patients with spinal cord injuries.

Twenty-four patients with spinal cord injuries were studied to correlate their responses to intra-abdominal disease with the level and completeness of the cord lesion. Patients with complete cervical lesions and lesions of the upper part of the thoracic region (C-4 to T-6) usually responded by early noniocalized abdominal pain associated with signs of autonomic dysreflexia. As the disease progressed to involve the parietal peritoneum, these patients were more capable of localizing pain to the corresponding dermatome, whereas patients with incomplete lesions were able to localize their pain ear

Motor Vehicle Injury and Kidney Damage: Trauma, Rhabdomyolysis, Acute Kidney Injury and Beyond.

Clinical Background: Automobile accidents constitute one of the main causes of morbidity and mortality worldwide. Kidney injury related to automobile accidents occurs through several mechanisms. Among the mechanisms are direct traumatic injury, rhabdomyolysis, acute renal failure, hypovolemic shock, and others. Kidney injuries after traffic accidents can have different forms of clinical presentation, which can include morphological and functional changes of the organ. Epidemiology: Annually, approximately 1.35 million individuals are fatal victims of motor vehicle accidents. Challenges: It is

Clinical and Radiological Comparison between Asian-Type and Conventional Reverse Shoulder Arthroplasty: A Multicenter Randomized Controlled Trial.

BACKGROUND: In patients with reverse shoulder arthroplasty (RSA), the relationship between the patient's body size and implant size is important for postoperative shoulder function. Asian patients, who have a short stature, could find the conventional RSA implant too big and tight. This study aimed to evaluate the clinical and radiological outcomes after RSA in Asian patients using a new implant designed to fit the body size of Asian patients. We also aimed to compare these outcomes with those who received a conventional implant. METHODS: This prospective study enrolled 120 patients who under

Type E2 glenoid bone loss orientation and management with augmented implants.

BACKGROUND: The purpose of this study was 2-fold: (1) to quantify type E2 bone loss orientation and its association with rotator cuff fatty infiltration and (2) to examine reverse baseplate designs used to manage type E2 glenoids. METHODS: Computed tomography scans of 40 patients with type E2 glenoids were examined for pathoanatomic features and erosion orientation. The rotator cuff fatty infiltration grade was compared with the erosion orientation angle. To compare reconstructive options in light of the pathoanatomic findings, virtual implantation of 4 glenoid baseplate designs (standard, ha

Outcome Reporting in Randomized Trials for Shoulder Disorders: Literature Review to Inform the Development of a Core Outcome Set.

OBJECTIVE: To explore the outcome domains and measurement instruments reported across randomized trials of any interventions for various shoulder disorders. METHODS: We searched for shoulder trials included in Cochrane reviews published up to Issue 10, 2015, or indexed in PubMed between 2006 and 2015. Trials were eligible for inclusion if they focused on any intervention for rotator cuff disease, adhesive capsulitis, shoulder instability, glenohumeral or acromioclavicular osteoarthritis, shoulder dislocation, proximal humeral or humeral head fractures, or unspecified shoulder pain. Determination and comparison of the smallest detectable change (SDC) and the minimal important change (MIC) of four-shoulder patient-reported outcome measures (PROMs).

BACKGROUND: There is a need for better interpretation of orthopedic treatment effects. Patient-reported outcome measures (PROMs) are already commonly used for patient evaluation. PROMs can be used to determine treatment effects in research as well as in clinical settings by calculating change scores, with pre- and post-treatment evaluation. The smallest detectable change (SDC) and minimal important change (MIC) are two important benchmarks for interpreting these change scores. The purpose was to determine the SDC and the MIC for four commonly used shoulder-related PROMs: Simple Shoulder Test (

A new technique for reconstruction of the proximal humerus after three- and four-part fractures.

The results of proximal humeral replacement following trauma are substantially worse than for osteoarthritis or rheumatoid arthritis. The stable reattachment of the lesser and greater tuberosity fragments to the rotator cuff and the restoration of shoulder biomechanics are difficult. In 1992 we developed a prosthesis designed to improve fixation of the tuberosity fragments in comminuted fractures of the proximal humerus. The implant enables fixation of the fragments to the shaft of the prosthesis and the diaphyseal fragment using screws, washers and a special toothed plate. Between 1992 and 20

[MR-based 3D-analysis of the pathomechanics of traumatic and atraumatic shoulder instability].

AIM: Until now it is unknown to what extent malpositioning of the scapula is a relevant factor in shoulder instability that should be considered in therapy. The objective was to analyse 3D-scapular kinematics and humeral head (de-)centering in patients with atraumatic and/or traumatic shoulder instability and to investigate the correlation between the two factors. METHOD: The shoulders of 28 healthy volunteers and of 14 patients each with atraumatic or traumatic instability were examined in various arm positions - with and without muscle activity - using open MR imaging. After 3D reconstructi

Return to Functional Activity After Pectoralis Major Surgical Treatment: A Comprehensive Review.

Pectoralis major is a strong, thick, and fan-shaped muscle that takes origination from the chest bone and medial part of the collarbone while passing to the arm; fibers converge in a U-shaped manner and insert onto the lateral lip of the bicipital groove of the humerus near the bicep tendon. The pectoralis major tendon is 3-5 cm thick and 40 mm wide. The pectoralis major tendon is powerful and useful for shoulder function; it plays a vital role in shoulder adduction and assists in internal rotation and humerus flexion. Injuries to the pectoralis major tendon are rare but are gaining popularity

Source and prevention of fetal deaths in motor vehicle crashes.

OBJECTIVE: Fetal deaths were analyzed for how they occurred in motor vehicle crashes investigated by NHTSA (new research) and published findings were evaluated from side impact safety research (review of pertinent literature). The aim was to improve protection of the fetus and pregnant occupant. METHODS: Crashes involving fetal deaths were analyzed from the 2000-2015 NASS-CDS (National Automotive Sampling System-Crashworthiness Data System) and 2017-2022 CISS (Crash Investigation Sampling System). Each case was downloaded and summarized for the vehicle, pre-crash maneuvers, crash type, matern

The neurovascular outcome of scapulothoracic dissociation.

PURPOSE: The purpose of this study was to define the role of acute revascularization in the management of scapulothoracic dissociation (SD), a rare injury of the upper extremity in which severe traction to the shoulder girdle results in combined arterial, brachial plexus, and musculoskeletal injury. METHODS: The results of a retrospective study of 11 patients with SD who were compared with 41 patients from the literature are presented. RESULTS: All 11 patients were involved in high-speed accidents. All patients had chest wall hematoma, absent radial pulse, and complete brachial plexus palsy.

The hip antero-superior labral tear with avulsion of rectus femoris (HALTAR) lesion: does the SLAP equivalent in the hip exist?

BACKGROUND: The purpose of this report is to describe a new lesion, the "Hip Antero-superior Labral Tear with Avulsion of Rectus femoris (HALTAR)". This injury may be seen in both adolescent and skeletally mature athletes and shares several characteristics with the "Superior Labral tear from Anterior to Posterior (SLAP)" lesion in the shoulder. METHODS: We present a case example, as well as a detailed anatomic description of the pathological condition. RESULTS: As the rectus femoris crosses two joints it is exposed to substantial forces during muscle contraction. Considering the peri-articul

Medical history and chronic pain.

OBJECTIVE: Can either a history of previous similar injury, including recurrence of injury, or an individual's symptoms, including time off work, predict chronic pain and/or chronic pain disability? METHODOLOGY: The literature search identified one systematic review and six observational studies (5 low back pain and 1 chronic shoulder pain) to provide evidence about these questions. RESULTS: Only one study evaluated subjects before a painful event. Other studies evaluated subjects over a range of time from onset of pain, including some selected for clinic treatment. This variability as well

Lateral Subcutaneous Locking Compression Plate and Small Incision Reduction for Distal-third Diaphyseal Humerus Fractures.

OBJECTIVE: Iatrogenic radial nerve injury is a great challenge for orthopaedic surgeons who deal with distal-third diaphyseal humerus fractures. Conventional open reduction and internal fixation (ORIF) remains the gold standard, but complications such as nonunion and iatrogenic radial nerve injury still occur. We fixed the fractures with a lateral locking compression plate (LCP) subcutaneously after small incision reduction to protect the radial nerve. This study reports the clinical and radiographic outcomes of our modified method. METHODS: Thirty-eight patients with distal-third diaphyseal

Shoulder dystocia: guidelines for clinical practice from the French College of Gynecologists and Obstetricians (CNGOF).

Shoulder dystocia (SD) is defined as a vaginal delivery in cephalic presentation that requires additional obstetric maneuvers to deliver the fetus after the head has delivered and gentle traction has failed. It complicates 0.5-1% of vaginal deliveries. Risks of brachial plexus birth injury (level of evidence [LE]3), clavicle and humeral fracture (LE3), perinatal asphyxia (LE2), hypoxic-ischemic encephalopathy (LE3) and perinatal mortality (LE2) increase with SD. Its main risk factors are previous SD and macrosomia, but both are poorly predictive; 50-70% of SD cases occur in their absence, and

[Does the disability of shoulder, arm and hand questionnaire (DASH) replace grip strength and range of motion in outcome-evaluation?].

Scoring systems currently used to evaluate functional disabilities in upper extremity conditions frequently combine so-called "objective" parameters such as grip strength and range of motion with "subjective" parameters such as pain. Since its introduction, the Disabilities of Arm, Shoulder and Hand Questionnaire has proven useful in the pre- and postoperative evaluation of functional limitations of the upper extremity. This study examines the relationship between DASH scores, the so-called "objective" parameters of grip strength and range of motion and those of "mixed scores" such as Krimmer

B-Mode Ultrasound Diagnostics of Orthopaedic Diseases in Clinical Avian Medicine: Comparative Study.

Orthopaedic disorders represent a significant clinical challenge in avian medicine, affecting both pet and wild birds. B-mode ultrasound (US) examination is well established in small-animal and equine medicine; it has seen limited application in avian medicine thus far. This study aimed to assess the diagnostic efficacy of B-mode US in comparison to radiography (RX) for orthopaedic avian disorders. A total of 55 birds from six orders were assessed clinically, radiologically, and sonographically. Statistical analysis was conducted for the overall cohort (n = 55) and for a fracture subgroup (n =
