Exploring nightly variability in sleep apnea and changes in severity classification with multi night sleep testing in a population based cohort of young children.

BACKGROUND: Obstructive sleep apnea (OSA) is typically evaluated with single-night sleep studies. In adults, night-to-night variability (NtNV) is well documented, but less so in children. The current study evaluated NtNV in apnea events for misclassification in disease severity in young children to investigate if multi-night sleep-recordings may be clinically relevant to improve diagnostic accuracy and affect treatment decisions. METHODS: Parents/guardians of healthy 4-9-year-old children participating in a study to evaluate prevalence of OSA were asked to record the child's sleep for up to 5…

Effects of a time-restricted eating intervention on sleep and body composition in adults with obstructive sleep apnea: A randomized cross-over clinical trial.

OBJECTIVES: This study aimed to evaluate the effects of an 8-h Time-Restricted Eating (TRE) intervention over 12 weeks on weight loss, body composition, and sleep in adults with Obstructive Sleep Apnea (OSA). METHODS: A randomized crossover clinical trial was conducted. Participants aged 30-65 years with a Body Mass Index (BMI) ≥30 kg/m2, diagnosed with OSA, of both sexes, with eating windows from 11 to 14 h were included. The participants were randomized into an 8-h TRE intervention (meals between 11 AM and 7 p.m.) or control group (eating window >10 h and nutritional guidance). Twelve parti…

REM sleep without atonia thresholds are associated with reduced cardiac MIBG uptake but not with DAT-SPECT binding in probable REM sleep behavior.

BACKGROUND: Quantitative thresholds for REM sleep without atonia (RWA) vary across populations, and their relationships with neuroimaging markers of Lewy body disease remain incompletely understood. OBJECTIVES: To examine whether previously proposed RWA thresholds for Japanese patients are associated with cardiac sympathetic and striatal dopaminergic imaging markers. METHODS: This multicenter study included 91 Japanese patients with probable RBD (isolated or PD-associated). Assessments included the RBD Questionnaire-Japanese version (RBDQ-JP), video-PSG, cardiac 123I-MIBG scintigraphy, and D…

Effects of resistance exercise combined with metformin on glycemic stability and sleep quality in elderly patients with type 2 diabetes mellitus with sleep disorders.

Sleep disorders in elderly type 2 diabetes mellitus (T2DM) patients worsen metabolic dysfunction. This study assessed resistance exercise plus metformin on glycemic stability and sleep quality in 180 elderly T2DM patients with sleep disturbances, divided into metformin alone (control) or metformin plus resistance exercise (intervention) for 12 weeks. Primary outcomes [fasting blood glucose (FBG), 2-hour postprandial glucose (2hPG), glycosylated hemoglobin (HbA1c), pittsburgh sleep quality index (PSQI) scores, electroencephalogram (EEG)-measured sleep architecture] and secondary outcomes [fasti…

The No-Apnea score for early obstructive sleep apnea detection in a sleep clinic: a study of diagnostic accuracy and comparative performance of three screening instruments.

This cross-sectional study investigated the No-Apnea tool's diagnostic ability to detect obstructive sleep apnea (OSA), across different severities (respiratory disturbance index [RDI] ≥ 5, ≥ 15, ≥ 30) compared to polysomnography (gold standard) in 2,087 consecutive patients attending a sleep clinic (age: 45.92 ± 13.46, male: 70.8%). We further compared the diagnostic performance of No-Apnea, NoSAS, and STOP-Bang tools. Across all RDI levels, No-Apnea showed superior sensitivity (96.9-99.0%, based on RDI thresholds), negative predictive value (NPV) (47.8-91.2%), negative likelihood ratio (0.10…

Evaluation of The Impact of Hypocretin Receptor 1 rs2271933 Polymorphism on Sleep Components in Chronic Migraine Patients with Poor Sleep Quality: A Subgroup Analysis.

INTRODUCTION: Long-reported dual comorbidity between migraine and sleep disorders suggests that some gene variations may play a role in this relationship. Our previous study found an association between poor sleep quality and the G allele of the hypocretin receptor 1 (HCRTR1) rs2271933 gene in patients with chronic migraine (CM). This study aimed to examine the relationship of this gene with some sleep parameters. METHODS: The present study was designed cross-sectional in the Mersin University Neurology Clinic between January 2000 and February 2018. Patients aged 18-75 years with CM according…

Randomised trial of home sleep apnoea testing compared to in-lab polysomnography for the evaluation of obstructive sleep apnoea in children: rationale and study protocol.

INTRODUCTION: Obstructive sleep apnoea (OSA) affects 1-5% of the paediatric population, including 55-90% of children with Down syndrome (DS), and has been associated with negative effects on neurocognitive development, cardiovascular health, immune development and quality of life. In-lab attended polysomnography (PSG) is currently the gold standard for the diagnosis of OSA in children, but it poses challenges due to the burden on families and limited testing facilities. Home sleep apnoea testing (HSAT), an unattended sleep test done at home, is an accepted alternative for adults but lacks suff…

Effects of Continuous Positive Airway Pressure on the Degree of Sleepiness, Functional Quality of Sleep, and Electrophysiological Hearing Responses in Individuals with Obstructive Sleep Apnea.

INTRODUCTION: Obstructive sleep apnea (OSA) affects nerve impulse transmission in the auditory pathway due to oxygen supply. Continuous positive airway pressure (CPAP) therapy improves oxygen levels, potentially alleviating sleepiness and enhancing central auditory pathway function. OBJECTIVE: To longitudinally evaluate the effect of CPAP on the degree of sleepiness, functional quality of sleep, and electrophysiological hearing responses of the central auditory pathways of patients with moderate to severe OSA. MATERIALS AND METHODS: There were 31 adults (21 men and 10 women), aged 20 to 70 y…

Poor sleep hygiene practices are associated with a higher increase in sleep problems during the COVID-19 pandemic: A latent change score model.

The Coronavirus 2019 (COVID-19) pandemic significantly influenced physical and mental health worldwide. The present study aimed to investigate changes in sleep problems across three waves of the COVID-19 pandemic, and to identify potential predictors of the inter-individual variability around these changes, with a particular focus on the role of detrimental sleep hygiene practices. A total of 352 participants completed an online survey of self-report questionnaires at three different waves of the COVID-19 pandemic: T1 (Spring 2020); T2 (Autumn-Winter 2020); and T3 (Spring 2021). The questionna…

Decrease in Sleep Duration and Poor Sleep Quality over Time Is Associated with an Increased Risk of Incident Non-Alcoholic Fatty Liver Disease.

The impact of changes in sleep duration and sleep quality over time on the risk of non-alcoholic fatty liver disease (NAFLD) is not known. We investigated whether changes in sleep duration and in sleep quality between baseline and follow-up are associated with the risk of developing incident NAFLD. The cohort study included 86,530 Korean adults without NAFLD and with a low fibrosis score at baseline. The median follow-up was 3.6 years. Sleep duration and quality were assessed using the Pittsburgh Sleep Quality Index. Hepatic steatosis (HS) and liver fibrosis were assessed using ultrasonography…

The hypoxic burden of sleep apnoea predicts cardiovascular disease-related mortality: the Osteoporotic Fractures in Men Study and the Sleep Heart Health Study.

AIMS: Apnoea-hypopnoea index (AHI), the universal clinical metric of sleep apnoea severity, poorly predicts the adverse outcomes of sleep apnoea, potentially because the AHI, a frequency measure, does not adequately capture disease burden. Therefore, we sought to evaluate whether quantifying the severity of sleep apnoea by the 'hypoxic burden' would predict mortality among adults aged 40 and older. METHODS AND RESULTS: The samples were derived from two cohort studies: The Outcomes of Sleep Disorders in Older Men (MrOS), which included 2743 men, age 76.3 ± 5.5 years; and the Sleep Heart Health…
