Oculomotor nerve and muscle abnormalities in congenital fibrosis of the extraocular muscles.

Congenital fibrosis of the extraocular muscles is an autosomal dominant congenital disorder characterized by bilateral ptosis, restrictive external ophthalmoplegia with the eyes partially or completely fixed in an infraducted (downward) and strabismic position, and markedly limited and aberrant residual eye movements. It has been generally thought that these clinical abnormalities result from myopathic fibrosis of the extraocular muscles. We describe the intracranial and orbital pathology of 1 and the muscle pathology of 2 other affected members of a family with chromosome 12-linked congenital

Effect of inferior oblique anterior transposition on vertical deviation.

PURPOSE: To evaluate the efficacy and complications of simplified graded inferior oblique anterior transposition (IOAT) in treating at least 10 PD vertical deviation in the primary position and inferior oblique muscle overaction (IOOA). METHODS: This retrospective study reviewed the medical records of 65 patients treated with simplified graded IOAT procedures for both vertical deviation and IOOA. Patients were grouped according to vertical deviation in the primary position. Group 1: vertical deviation of not > 20 PD; the inferior oblique (IO) muscle was fixed 2 mm posterior and 2 mm temporal

Associations between Attention-Deficit/Hyperactivity Disorder and Ocular Abnormalities in Children: A Population-based Study.

Purpose: Attention-deficit/hyperactivity disorder (ADHD) was reported to be associated with disturbances in the prefrontal circuitry and seems to be associated with dysfunctions of eye motility. This study aimed to explore associations between ADHD and ocular abnormalities, including amblyopia, hypermetropia, astigmatism, and heterotropia, using a large, nationwide population-based dataset in Taiwan. Methods: We retrieved our sample for this cross-sectional study from the Taiwan National Health Insurance Research Database. In total, 116,308 children with ADHD were selected as the study group an

Inferior oblique muscle anteriorization in congenital superior oblique palsy.

BACKGROUND: Inferior oblique muscle overaction of variable amounts is usually present with congenital superior oblique palsy. Inferior oblique muscle anteriorization has been described as a suitable surgical procedure in this entity. The aim of this study was to investigate the effect of inferior oblique muscle anteriorization in patients with congenital superior oblique palsy on vertical, torsional and horizontal alignment. PATIENTS AND METHODS: The study was designed as an institutional retrospective cohort study. 45 patients with congenital superior oblique palsy (15 female, 30 male; mean

Visual results after primary intraocular lens implantation or contact lens correction for aphakia in the first year of age.

PURPOSE: The optimal role of intraocular lenses (IOLs) in infants remains a controversial topic. Some ophthalmologists advocate correction w … ntation results in improved visual acuity, improved binocular vision outcome and less occurrence of strabismus, but a higher rate of complications requiring reoperation. Further studies with a larger pediatric patient group are necessary to confirm the optimal treatment of aphakia after unilateral congenital cataract extraction.

Ocular tilt reaction due to a mesencephalic lesion in juvenile polyarteritis nodosa.

PURPOSE: To describe a case of ocular tilt reaction caused by vasculitic lesions in the midbrain in a child with polyarteritis nodosa. DESIGN: Observational case report. METHODS: A 5-year-old girl with a chronic illness developed diplopia associated with a left head tilt, right hypertropia, torsional nystagmus, slowed vertical saccades and poor convergence. Fundoscopic examination demonstrated conjugate leftward torsion of the eyes consistent with a sustained ocular tilt reaction.Renal angiography confirmed polyarteritis nodosa and cerebral magnetic resonance imaging demonstrated mesencephal

Motility and binocularity outcomes in vitrectomy versus scleral buckling in retinal detachment surgery.

BACKGROUND: Ocular motility defects and loss of binocularity are well-recognised problems following retinal detachment surgery. It is presumed that scleral buckling is primarily responsible for these effects. The increasing use of vitrectomy in the management of retinal detachment might be expected to reduce the incidence of these defects. METHOD: Two groups of patients presenting with primary uncomplicated rhegmatogenous retinal detachments were examined following a single surgical repair. The first group underwent vitrectomy (n = 17), the second group, scleral buckling/external surgical tec

Surgical outcomes of four inferior oblique weakening techniques: a graded, severity-based approach.

BACKGROUND: Inferior oblique muscle overaction (IOOA) is a common cause of vertical strabismus. Most published comparisons cover two or three techniques. We evaluated 1-year outcomes of a severity-guided algorithm comparing four weakening techniques for IOOA: disinsertion, myectomy, anterior transposition (AT), and nasal transposition (NT). METHODS: In this retrospective comparative cohort study, 286 eyes of 200 patients with IOOA (grade + 1 to + 4) were treated by disinsertion (n = 141), myectomy (n = 29), AT (n = 94), or NT (n = 22), selected by the operating surgeon according to preoperati…

Autophagy and Mitochondrial Dysfunction in Tenon Fibroblasts from Exfoliation Glaucoma Patients.

PURPOSE: To test the hypothesis that autophagy dysfunction is involved in exfoliation syndrome (XFS), a systemic disorder of extracellular elastic matrices that causes a distinct form of human glaucoma. METHODS: Fibroblasts derived from tenon tissue discards (TFs) from filtration surgery to relieve intraocular pressure in XFS patients were compared against age-matched TFs derived from surgery in primary open-angle glaucoma (POAG) patients or from strabismus surgery. Differential interference contrast light, and electron microscopy were used to examine structural cell features. Immunocytochemi…

The effect of developmentally-at-risk status on the reliability of the iScreen® photorefractive device in young children.

BACKGROUND: Hereditary spastic paraplegia (HSP) with thin corpus callosum (CC) is a rare neurodegenerative disorder classified as a complicated form of spastic paraplegia. Some patients with HSP with thin CC have previously been described in Japanese families, and the genetic locus was linked to chromosome 15q13-15. OBJECTIVE: Our objective was to further clinically and genetically characterize HSP with thin CC. PATIENTS: We describe the clinical, structural, and functional follow-up and the genetic characterization of 2 sisters aged 26 and 31 years who had severe spastic paraplegia and cogn

Comparison of orbital fracture repair performed within 14 days versus 15 to 29 days after trauma.

PURPOSE: To examine whether orbital floor and/or medial wall fracture repair delayed for 15 to 29 days is as effective as early surgery. METHODS: A retrospective review is reported comparing outcomes of early fracture repairs (performed 1-14 days after trauma) to delayed fracture repairs (performed 15-29 days after trauma). Ocular motility, diplopia, and time to resolution of diplopia postoperatively are the main endpoints. RESULTS: Fifty-eight patients were included in the study: 36 underwent early fracture repair (average 9 days after trauma) and 22 underwent delayed fracture repair (avera
