Cortical Button Fixation: A Better Patellar Tendon Repair?

BACKGROUND: Patellar tendon ruptures require surgical repair to optimize outcomes, but no consensus exists regarding the ideal repair technique. Cortical button fixation is a secure method for tendon repair that has not been studied in patellar tendons. HYPOTHESIS: Cortical button repair is biomechanically superior to the standard transpatellar repair and biomechanically equivalent to suture anchor repair. STUDY DESIGN: Controlled laboratory study. METHODS: Twenty-three fresh-frozen cadaveric knees were used to compare 3 techniques of patellar tendon repair after a simulated rupture at the

Ulnar subluxation of a ruptured EPL tendon at the metacarpophalangeal joint: case report.

We report a case of ulnar subluxation of a ruptured extensor pollicis longus tendon at the metacarpophalangeal joint due to tethering of the distal tendon and an abnormal vector of pull. In this case, thumb flexion with the tethered EPL tendon resulted in attenuation of the radial sagittal band of the extensor hood. After plication of the radial sagittal band and relocation of the EPL tendon, adequate thumb extension was restored.

Immediate controlled active motion following zone 4-7 extensor tendon repair.

This article describes a splint management program for zone 4-7 extensor tendon repairs that allows for immediate controlled active motion (ICAM) of the repair and greater arcs of motion for adjacent digits. The splint is designed to relieve tension on the tenorrhaphy by positioning the involved digit in slight metacarpophalangeal joint hyperextension relative to the uninvolved digits with a simple yoke splint designed to control the metacarpophalangeal joints and a second splint to control wrist position. Cadaver and intraoperative trials support this technique, and 140 patient cases managed

[Open versus arthroscopic shoulder stabilization. An analysis of the treatment costs].

In this study the total costs of clinical open and arthroscopic anterior shoulder stabilization were evaluated, analyzed and compared. From 1988 to 1998 147 patients underwent open (Bankart) or arthroscopic (ASK) anterior shoulder stabilization. We randomized two groups of 30 patients for each method (Bankart: 25 male, 5 female, 29 years of age; ASK: 25 male, 5 female, 26 years of age) and evaluated the costs of their clinical treatment. The total cost was significantly ( p<0.05, Mann-Whitney U-Test) higher for the open (5639 euro) than for the arthroscopic (4601 euro) therapy. There was a sig

Early motion of the ankle after operative treatment of a rupture of the Achilles tendon. A prospective, randomized clinical and radiographic study.

BACKGROUND: Different regimens of early motion of the ankle after operative treatment of a ruptured Achilles tendon have been suggested since the late 1980s. However, as far as we know, no controlled studies comparing these regimens with conventional immobilization in a cast have been reported. METHODS: In a prospective study, seventy-one patients who had an acute rupture of the Achilles tendon were randomized to either conventional postoperative management with a cast for eight weeks or early restricted motion of the ankle in a below-the-knee brace for six weeks. The brace was modified with

A comparison of post-operative mobilization of flexor tendon repairs with "passive flexion-active extension" and "controlled active motion" techniques.

We report a comparative study of the outcome of flexor tendon repairs mobilized by either a "passive flexion-active extension" or a "controlled active motion" regimen. We show that the controlled active motion regimen conferred significant benefits on the final range of motion and extensor lag. The rupture rate was raised with "controlled active motion" but this was not greater than levels reported by other authors using "passive flexion-active extension" regimens.

External fixation use in the management of massive upper extremity trauma.

Immobilization of fractures associated with instability and comminution by an external skeletal device facilitates wound care, skin coverage, and ultimate reconstructive surgery. External skeletal fixation was applied in 20 patients who sustained complex open injuries of the upper extremity. All injuries were associated with extensive soft tissue and skin defects. Associated neurovascular injuries included 14 nerve injuries and five major vascular injuries. The Hoffman fixation device was used in the majority of cases. Skin coverage was obtained in all patients by an appropriate method of skin

Comparison of Clinical Outcomes, Revision Rates, and Sports Participation Between Hamstring and Quadriceps Tendon Autografts for Anterior Cruciate Ligament Reconstruction in Pediatric and Adolescent Patients: A Retrospective Cohort Study.

BACKGROUND: In recent years, anterior cruciate ligament (ACL) injuries in children and adolescents (age ≤18 years) have been increasing, and the quadriceps tendon (QT) autograft has been gaining popularity for ACL reconstruction. However, there is no consensus regarding the graft choice for ACL reconstruction in these young patients. There has been a paucity of literature comparing the functional outcomes of hamstring tendon (HT) and QT autografts for ACL reconstruction in patients aged ≤18 years. PURPOSE/HYPOTHESIS: The purpose of this study was to compare the outcomes of HT and QT autograft

Brachial Plexus Birth Injury due to Proximal Humeral Physeal Separation: A Case Report.

CASE: A 9-day-old male neonate presented with swelling over his left shoulder and reduced movements of the left upper extremity since birth. Radiographs raised a suspicion of a shoulder dislocation. Ultrasonography confirmed the diagnosis of proximal humeral physeal separation. Magnetic resonance imaging (MRI) revealed edema of the infraclavicular part of the brachial plexus. Conservative management was followed, and the injury remodeled completely at 2 years. CONCLUSION: Neonatal proximal humeral physeal separation poses a diagnostic challenge. Ultrasonography is helpful for diagnosis. MRI i

Bipolar Dislocation of the Proximal Phalanx of Toe: A Rare Injury.

We report a very rare case of bipolar fracture with dislocation of the proximal phalanx or floating proximal phalanx of the toe. The literature has shown that closed reduction gives a lesser chance of success considering the inherent anatomy of the foot. We report a case of a 40-year-old male with an open injury to his right foot involving bipolar dislocation of the proximal phalanx of the third toe with extensor digitorum longus tendon injury and fracture of the neck of the fourth proximal phalanx of the right foot. With the realization that open reduction and Kirschner wire fixation are good

Treatment of soft tissue injuries to the dorsum of the metacarpophalangeal joint (Boxer's knuckle).

We retrospectively reviewed the surgical treatment for 16 cases of traumatic soft tissue injury to the metacarpophalangeal joint (Boxer's knuckle). A history of trauma was present in all cases and there was an associated extensor tendon dislocation in seven cases. Eight cases were initially treated conservatively, but their symptoms persisted. Intraoperative findings included rupture of the extensor hood or joint capsule in all cases. Surgical closure of the rupture of the joint capsule resulted in a successful outcome in all cases. We consider that conservative treatment of this injury may no

Acute effects of the FIFA 11+ warm up protocol on distal mechanical and physiological tissue properties and performance in male and female soccer players.

OBJECTIVE: Soccer is one of the sports with the highest number of injuries in both sexes with a prevalence of 13% around the ankle-foot area. Various strategies have been proposed to prevent injuries like FIFA 11+ warm-up. Its reliability has been questioned due to the diversity of results obtained depending on the movement pattern studied and the body region analyzed. Few studies have analyzed its acute effects on distal structures, such as plantar soft tissue properties, local perfusion or temperature. The objective of this study was to determine the acute effects of performing the FIFA 11+

Tibial Tuberosity Avulsion in an Adult Female With Patellar Tendon and Retinacular Repair Enhanced by Platelet-Rich Plasma (PRP) Augmentation: A Case Report.

Tibial tuberosity avulsion fractures are rare injuries, most commonly seen in adolescent males during high-energy activities. These fractures are uncommon, specifically in adults, due to the closure of the physis. In particular, skeletally mature female avulsions are rarely reported in the literature. Significant displacement of the tibial tubercle often necessitates surgical management to restore the extensor mechanism and maintain knee stability. In this case report, we present a 21-year-old female who sustained a tibial tuberosity avulsion fracture with an associated patellar tendon rupture

Analysis of Quadriceps Fatigue Effects on Lower Extremity Injury Risks During Landing Phases in Badminton Scissor Jump.

The scissor jump (SKJ) is vital in badminton, particularly for backcourt shots, but fatigue increases lower limb load and injury risk. This study investigates how quadriceps fatigue affects biomechanical characteristics and load during SKJ landing, aiming to understand its impact on injury risk. This study involved 27 amateur male badminton players from Ningbo University. Quadriceps fatigue was induced via knee exercises and footwork drills. Biomechanical data before (prior fatigue-PRF) and after fatigue (post fatigue-POF) were recorded using a force platform and motion capture system. Muscle

Is fascia lata a viable graft for MPFL reconstruction? An overview of surgical technique and scoping literature review.

Lateral patellar dislocation is irrefutably one of the most common knee injuries, while subsequent medial patellofemoral ligament (MPFL) damage requires proper orthopedic care. Treatment of these injuries is regularly associated with the need for MPFL reconstruction surgery. This operation, often in combination with other procedures, can result very fruitful in restoring knee kinematics. There is a considerable variety of options for the orthopedic surgeon to choose from regarding graft selection when performing MPFL reconstruction surgery, including the semitendinosus tendon, the gracilis, an

Exploring the relationship between trunk muscles and lower limb injuries in Australian badminton players.

Due to its dynamic nature, lower limb injuries are common in badminton. Overuse injuries of the knee, including tendon related conditions, are the most common. During jumping and landing, force transference and dissipation through the trunk is required, with the trunk muscles playing a vital role. However, the relationship between knee pain and the ability to voluntarily contract the trunk muscles has not yet been explored in badminton players. A cross-sectional study of Australian badminton players was therefore conducted. Players performed a single leg decline squat to identify those with kn

Reconstruction of a C7-T1 brachial plexus lower root injury by transferring multiple nerves and a free gracilis muscle: Case report.

Lower-type brachial plexus injuries (BPI) are uncommon, and traditional reconstruction with tendon transfer procedures generally produce mediocre results. However, the advent of nerve transfers has rejuvenated the reconstructive options for peripheral nerve and spinal cord injuries. In this paper, we report the case 32 year-old patient with a C7-T1 avulsion BPI in whom multiple motor and sensory nerve transfers, combined with a free gracilis muscle flap transfer, were used to restore upper-limb functional defects. Five months after injury, several nerves were transferred (posterior division of
