Therapeutic outcome and patient adherence to repeated onabotulinumtoxinA detrusor injections in chronic spinal cord-injured patients and neurogenic detrusor overactivity.

BACKGROUND/PURPOSE: To investigate the continuous therapeutic effects and urinary incontinence severity after repeated detrusor injections of 200-U of onabotulinumtoxinA (BoNT-A) in chronic spinal cord-injured (SCI) patients. METHODS: Between 2006 and 2010, patients with chronic SCI and refractory neurogenic detrusor overactivity (DO) were treated with repeated sets of 200-U BoNT-A injected into 20 sites every 6 months. All patients underwent urological examinations and video-urodynamic studies at baseline and after each BoNT-A treatment. The outcomes were measured using Urogenital Distress I…

Utilization of Real-World Evidence in the Evaluation of Post-Surgical Incontinence in Men Undergoing Surgical Treatment for Benign Prostatic Hyperplasia.

INTRODUCTION AND OBJECTIVES: Post-surgical urinary incontinence (PSI) can occur after benign prostatic hyperplasia (BPH) surgery. Estimating incontinence risks across modalities largely relies upon reports from single surgeons or expert groups. Real-world data (RWD) provides a broader view of practice settings and surgeon experience. We used RWD to assess the incidence, duration, and risk factors of PSI. METHODS: We conducted a retrospective cohort study using U.S. healthcare system electronic health records data compiled by Truveta of 97,970 men who underwent transurethral resection of the p…

Tanagho bladder neck reconstruction in the treatment of adult incontinence.

We studied the effectiveness of tubularized bladder neck reconstruction in the treatment of 8 patients with complex incontinence using urodynamic and clinical methods. The patients had undergone Tanagho bladder neck reconstruction within the last 10 years. Three of the 8 patients were judged unsuitable for artificial sphincter implantation because of severe scarring, and loss of urethral and vaginal tissue. There were 7 women with epispadias or severe urethral damage as a consequence of obstetrical or gynecological procedures. Five patients underwent 7 concurrent procedures at the time of blad

Urethral fistula repair, including urethral elongation and an anti-incontinence procedure - a case report.

Vesicovaginal fistula remains a significant cause of female urinary incontinence and impaired quality of life worldwide. Urethral-vaginal fistulas are less common, and difficult to manage, posing difficulties in terms of diagnostic evaluation, the surgical approach to closure, and how the associated complication of stress urinary incontinence is addressed. In this case report the management of a 34-year-old woman who presented with a urethral fistula following initial surgery for a urethral abscess is presented, including fistula closure; lengthening of the urethra; and addressing the loss of …

Urethral Coitus in a Case of Vaginal Agenesis - Is Only Vaginoplasty Enough to Treat the Urinary Problems?

Urethral coitus is an extremely rare condition. Megalourethra and urinary incontinence due to urethral coitus in vaginal agenesis are unusual manifestations because these patients usually present with primary amenorrhea before becoming sexually active and receive treatment. A 24-year-old woman came to our clinic because of primary amenorrhea, sexual dysfunction, dyspareunia, megalourethra and urinary incontinence five months after her marriage due to urethral coitus. Based on these clinical and radiological findings a diagnosis of Mayer-Rokitansky-Kuster-Hauser Syndrome was made and patient un…
