During the last 20 years, physicians have generally regarded heart failure as a haemodynamic disorder in an attempt to explain patients' symptoms and disability. This model led to the widespread evaluation of peripheral vasodilators and the development of novel positive inotropic agents, but long-term use of these drugs failed to improve symptoms and was frequently accompanied by an increase in the risk of death. These clinical observations raised concerns about the validity of the haemodynamic hypothesis and led to the development of alternative models of heart failure--most importantly, the

Persistent pulmonary hypertension of the neonate (PPHN), described initially by Gersony in 1969 as persistent foetal circulation (PFC syndrome), results from a flawed transition from foetal to extrauterine pulmonary circulation. It is primarily characterised by persistence of, or return to, the suprasystemic pulmonary vascular resistance and pressure normally found in the foetus. The increased pulmonary pressure causes right to left shunting through the ductus arteriosus or the foramen ovale, or both. The resulting hypoxaemia and acidosis may produce further pulmonary vasoconstriction and lead

Chronic and acute hypertension have multiple untoward effects on the coronary circulation, several of which may either mimic or markedly worsen the clinical manifestations of coronary artery disease. Early after the onset of left ventricular hypertrophy secondary to hypertension, coronary vasodilator reserve is significantly impaired. During cardiac hypertrophy secondary to hypertension, the coronary arteries fail to enlarge in concert with ventricular enlargement. This failure results in a relative decrease by approximately 50% in the ratio of epicardial vessel diameter to the mass of myocard

We analyzed the tissue oxygen extraction in 25 patients with acute respiratory failure. Fourteen met the clinical criteria for the adult respiratory distress syndrome (ARDS). The 11 remaining patients had acute respiratory failure with causes different from ARDS. In all cases the changes in the oxygen extraction ratio (O2ER) and in the oxygen consumption (VO2) were evaluated after changing oxygen availability (O2A) with positive end-expiratory pressure (PEEP) and dobutamine infusion. The patients with ARDS showed a change in VO2 parallel to O2A changes, with a significant correlation (r = 0.85

An analysis of epidemiology of the post-traumatic infections of the locomotive system, prophylaxis and treatment in presented. Infections are the most severe complications of the open bone fractures and orthopedic surgery. An incident of inflammatory complications following the open bone fractures is very high (20% according to Burrie). An incidence of complications following orthopedic surgeries is somewhat lower--0.2-9.0%. Within 1976-1986, as much as 585 patients with chronic osteitis, including 222 patients with post-traumatic infections, were treated at the Department of Orthopedic Surger

Endothelium-derived nitrogen oxides, i.e., nitrogen oxide and/or nitrogen oxide-containing compounds, formed from L-arginine account, at least in part, for the biological activity of endothelium-derived relaxing factor(s). We have developed a rapid and sensitive assay to determine the basal and stimulated release of defined breakdown products of endothelium-derived nitrogen oxides, i.e., nitrite and nitrate. Formation and/or release of these nitrogen oxides was time- and concentration-dependently stimulated by various endothelium-dependent vasodilators and was decreased by NG-methyl-L-arginine

Intracavernous injection of vasoactive agents causing vasodilatation is widely recognized in the diagnosis and treatment of erectile dysfunction. However, papaverine, the drug most commonly used for this purpose, may produce priapism and fibrotic lesions, and alternatives without these disadvantages are desirable. In this study we investigated the effects of pinacidil, a vasodilator drug supposed to act through the opening of K+ channels, on isolated human corpus cavernosum penis. Besides abolishing spontaneous contractile activity, pinacidil effectively relaxed preparations precontracted by n

Papillary colorimetry is a qualitative and quantitative diagnostic method that defines the displacement of the temporal and nasal color ranges on the x;y plane of the German Industrial Standard (DIN) color chart. Starting with normal temporal and nasal papillary stain values, there are, for qualitative diagnosis, four different directions in which the color range can shift from normal values on the DIN color chart, i.e., in connection with simple optic atrophies, optic atrophies due to vascular conditions, hyperemic papillae, and deviations of the color range under vasodilative therapy. Adenosine is a renal vasoconstrictor that plays an important role in mediating renal adaptive responses to decreases in renal perfusion pressure. It is known that adenosine acts on the metabolism of arachidonic acid, but the direct repercussions of adenosine in the production of renal prostaglandins and leukotrienes have not been studied. This study was undertaken to evaluate the effect of the intrarenal infusion of adenosine upon the urinary elimination of arachidonic acid derivatives. Samples of urine were collected with lysine acetylsalicylate and determination of prostaglandins (PGs) and l

Forty patients who had undergone coronary artery graft surgery and who required vasodilator therapy for postoperative hypertension were given infusions of either alfentanil or morphine together with bolus doses of midazolam for sedation and analgesia while ventilation was controlled artificially. Sodium nitroprusside (SNP) was administered to both groups using a computer-controlled closed loop system which adjusted the infusion rate to maintain a preset target arterial pressure. Target pressure +/- 5, 10, 15 and 20 mm Hg was maintained longer in the group receiving alfentanil. This group also

Physiological overreactivity to mental stress has been correlated with the subsequent development of cardiovascular diseases. This coronary-prone behavior (referred to as "hot reacting") is distinct from the Type A behavior pattern and is seen in about 1 in 5 apparently healthy individuals. This paper describes a noninvasive system for measuring hemodynamics that can reveal the mechanisms raising the blood pressure of hot reactors or hypertensives. The subject is challenged with a variety of mental and physical stressors while the hemodynamic responses are monitored. Blood pressure elevations

Renal hypoperfusion such as occurs in shock creates an environment in which cellular injury and organ dysfunction can occur during the episode of shock as well as during reoxygenation and reperfusion. A severe decrement in oxygen delivery compromises energy (adenosine triphosphate) production, leading to various degrees of cell injury ranging from cell swelling to acute cortical necrosis. These different responses of the kidney to shock explain the multiple clinical presentations varying from an isolated loss of concentrating ability to prolonged anuria. Many cellular events contribute to rena

Two double-blind randomized crossover studies were carried out in healthy male subjects to determine the cardiovascular effects of fenoldopam and to assess whether they could be modified by metoclopramide. Blood pressure (BP) and heart rate (HR) were measured supine and upright, before and for up to 4 h after dosing. Single oral doses of fenoldopam 50, 100 or 150 mg or placebo were administered in one study. In the other study, subjects received 20 mg metoclopramide or placebo intravenously, 20 min before an oral dose of 100 mg fenoldopam or placebo. Fenoldopam produced decreases in diastolic

Beta blockade is the cornerstone of the therapy for ischemic heart disease and is increasingly used as first-line therapy for hypertension. Three chief properties that distinguish among different beta blockers are cardioselectivity, intrinsic sympathomimetic activity (ISA) and pharmacokinetic differences. Cardioselectivity appears to confer some advantages, although there is a potential and serious danger in giving any beta blocker to patients with asthma, heart failure or active peripheral disease. The degree of ISA in different beta blockers may vary. This property may diminish the degree of

3 patients with Bartter's syndrome were studied under metabolic ward conditions before and during administration of captopril 25 mg t.i.d. The drug induced an immediate and sustained reduction of blood pressure (by 14 and 18% after 1 h and 3 days, respectively) with no change in heart rate. During treatment renal plasma flow increased by 29%, but the glomerular filtration rate was unchanged. Renal vascular resistance decreased by 41%. There was an increase in urinary sodium excretion with a corresponding reduction of body weight. The potassium balance was not affected. The concentration of ang

From 1976 to 1981, 22 patients were treated for atherosclerotic renovascular disease and with widespread atherosclerosis. The average age was 56 years. Preoperatively, the average blood pressure (BP) was 211/123 mm Hg. The standard operation was aortorenal bypass, associated with resection of an abdominal aortic aneurysm in three cases. Four patients had nephrectomy. Most patients were referred after failure of a previous renal revascularization procedure. In three patients, most of the renal arterial system was thrombotic. All patients had preoperative Swan-Ganz catheter placement for monitor

Diazoxide was injected into the pulmonary artery in nine patients with primary pulmonary hypertension. There was no significant change in pulmonary artery pressure, which fell by more than 10 mmHg in only two patients. The pulmonary blood flow increased in all patients as a result of a fall in pulmonary vascular resistance (by 4 to 17 units). Systematic vasuclar resistance also fell as expected in all patients. Oral diazoxide was given to seven patients, two of whom showed sustained clinical improvement while remaining on treatment (400 to 600 mg daily). Five patients were unable to tolerate t

Circulatory disorders, which are well documented in most parts of the human body, are not well documented in the inner ear, although they are expected to occur. It has been previously shown that experimental occlusion of the labyrinthine artery in animals results in severe degenerative changes, fibrosis, and new bone formation in the cochlea. Accordingly, this paper presents presumptive evidence that depriving the human cochlea of its blood supply after surgical removal of an acoustic tumor results in severe degenerative changes that progress to total ossification of the cochlear spaces. This

The effect of the vasodilator prazosin vs placebo on exercise duration until marked dyspnea, and on left ventricular function measured by echocardiography, was evaluated in a double-blind, randomized study in 24 patients with chronic left ventricular failure despite digitalis and diuretic therapy. Compared with the double-blind placebo, prazosin reduced resting systolic and diastolic blood pressure and systolic blood pressure times heart rate, improved clinical symptoms, decreased cardiothoracic ratio measured by chest roentgenography, decreased left ventricular and left atrial dimensions, imp

Persistence of the fetal circulation is a recently recognized cause of severe cyanosis in newborn full term infants. Abnormally elevated pulmonary vascular resistance apparently stimulated by hypoxia, acidosis, and/or hyperviscosity results in cyanosis due to large right-to-left shunts through persistent fetal channels (patent foramen ovale and patent ductus arteriosus). Initial chest radiographs demonstrate clear lungs, decreased, normal, or mildly prominent pulmonary vascularity, and normal to moderately enlarged cardiac silhouettes. Angiocardiography, when required to rule out cyanotic cong
