OBJECTIVE: We report the rates of optimal abdominopelvic cytoreduction and the sites of recurrence in stage IV ovarian cancer patients, with particular attention to the potential impact of thoracic cytoreduction on treatment results in patients with intra-thoracic spread.
METHODS: A historic cohort study of all stage IV ovarian cancer patients diagnosed between 1994 and 2010 and underwent abdominopelvic cytoreductive surgery. Controls were stage IIIc patients. Statistical analyses included χ(2) test, Cox proportional hazards regression models and Kaplan-Meier curves with log-rank tests.

BACKGROUND: Single-incision laparoscopic splenectomy has been reported and was proved to be feasible and safe. Yet reports on single-incision laparoscopic surgery (SILS) for massive splenomegaly is not seen, neither is SILS for gastroesophageal devascularization; another correlated procedure for patients complicated with bleeding esophageal varices. From February 2011 to March 2012, 6 male patients with a median age of 45.1 years (range, 25 to 65 y) who were diagnosed of liver cirrhosis, portal hypertension, splenomegaly, and hypersplenism underwent SILS by the same surgical team in our hospit

BACKGROUND: Since Theodor Kocher reduced the mortality rate of thyroidectomy from the 40% reported by Billroth to 0.2% in 1895, a collar incision with open removal of the thyroid gland is the standard procedure. In the past decade, efforts were made to reduce incision size and surgical access trauma by the use of endoscopic techniques. A first attempt was replacement of the central "Kocher incision" with lateral neck incisions and endoscopic removal of a thyroid lobe by Hüscher on 8 July 1996. This lateral access was limited to removing only one lobe of the gland. The most common technique to

BACKGROUND: Dissection around the superior mesenteric artery (SMA) is a crucial and one of the most challenging processes in laparoscopic pancreaticoduodenectomy (LPD) owing to the deep anatomic location and the indispensable role of SMA. Multiple approaches to SMA during LPD have been proposed, but most of them require the transection of the jejunum or the neck of pancreas in advance and may cause significant tension on mesenteric vessels that may lead to accidental bleeding.1-3 Here, we describe an in situ anterior approach to the SMA that aids in the early assessment of tumor resectability

Background In recent times, the supraorbital approach via eyebrow incision has gained tremendous popularity in targeting the anterior skull base and few middle cranial fossa lesions, over the more traditional pterional and frontotemporal approaches. However, the extremely narrow viewing angle through this approach requires frequent adjustments of the operating table and microscope for optimal visualization. Illumination via such a small opening in such deep-seated location was another limiting factor. Keeping these problems and cumbersomeness of microscope in mind, experienced surgeons gradua

BACKGROUND: We sought to determine whether or not there are differences in disease progression after radical or nonradical (debulking) surgical procedures for malignant pleural mesothelioma.
METHODS: Over a 49-month period, 132 patients with malignant pleural mesothelioma underwent surgery. Fifty-three underwent extrapleural pneumonectomy and 79 underwent nonradical procedures. Time to evidence of clinical disease progression was recorded, as was the site(s) of that disease.
RESULTS: One-hundred nineteen patients were evaluable, of which 59% (22 radical; 48 nonradical) had disease progressio

Malignant pleural mesothelioma (MPM) increases the risk of venous thromboembolic (VTE) events. This risk is higher following extrapleural pneumonectomy (EPP) as part of trimodality therapy, where VTE can be catastrophic. In our series, the impact of warfarin in preventing a pulmonary embolus (PE) after neoadjuvant chemotherapy and EPP for MPM was analysed. A retrospective analysis of 21 consecutive patients undergoing EPP for MPM was conducted. The first 10 patients (Group A) had VTE prophylaxis by subcutaneous enoxaparin and compression stockings commenced a day prior to surgery, intraoperati

RATIONALE: Severe asthma represents 5-10% of all asthma, yet remains problematic and poorly understood. Although it is increasingly recognized as consisting of numerous heterogenous phenotypes, their immunopathology, particularly in the distal airways and interstitium, remains poorly described.
OBJECTIVES: To identify the pathobiology of atypical difficult asthma.
METHODS: We report 10 from a total of 19 patients (17 women and 2 men) meeting asthma and severe asthma definitions, requiring daily systemic corticosteroid (CS) use, with inconsistent abnormalities on chest computed tomography sca

INTRODUCTION: A chylothorax can occur following any intrathoracic procedure. It is generally straightforward to make the diagnosis but optimal management can be problematic.
METHODS: Between 1995 and 2002, three women and one man aged from 13 to 58 years were treated for chylothorax after thoracic surgery. Their initial illnesses were a right pulmonary hydatid cyst associated with hepatic disease, a tumour of the posterior mediastinum, an oesophageal carcinoma and metastases in the left lung.
RESULTS: These patients had: a pulmonary and hepatic cystectomies, a resection of the mediastinal tu

BACKGROUND: The posterolateral corner (PLC) of the knee comprises important static and dynamic stabilizers, including the lateral collateral ligament, popliteus tendon, biceps femoris tendon, the popliteofibular ligament, and the posterolateral capsule. These structures are disrupted in many knee injuries, yet may be overlooked due to the complex anatomy, subjective clinical diagnostic exams, and poorly defined imaging techniques. Complete PLC lesions rarely heal with non-operative treatment, and can lead to significant instability and poor outcomes, and are therefore most successfully treated

BACKGROUND: The flexor pronator slide is an effective treatment option for ischemic contracture and contracture related to spastic cerebral palsy, but little is known about the use of the flexor pronator slide in other non-ischemic contractures. I propose a flexor pronator slide to simultaneously correct wrist and finger flexor contractures and preserve the muscle resting length. To avoid overcorrection of the deformity, I propose the use of a wide-awake local anesthesia with no tourniquet (WALANT) procedure, in which the patient is able to continually assist the surgeon in assessing the contr

BACKGROUND: Quadriceps autograft, though well established for anterior cruciate ligament reconstruction, is underutilized in posterior cruciate ligament (PCL) reconstruction largely due to slow adoption. All-inside meniscal ramp repair and quadriceps tendon autograft PCL reconstruction have been described in isolation, but not concomitantly in a video journal.
INDICATIONS: PCL reconstruction is indicated in grade 3 isolated tears with instability that have not improved with nonoperative management and in instances with associated injuries such as meniscal ramp tears, as observed in this 18-ye

INTRODUCTION: Pleural metastases are common in thymic epithelial tumors (TET), which include pleural recurrence and stage IVa at initial diagnosis. However, the specific risk factors predicting its recurrence (i.e., pleural recurrence group) and prognostic factors for pleural metastasis (i.e., stage IVa) remain unclear. This review aims to identify and discuss the predictors associated with pleural metastases in patients with TET.
METHODS: A systematic literature search was conducted on PubMed, MEDLINE, Embase, and Cochrane for articles published between 1/1/1990 and 3/11/2023. The selection

Introduction. Thymoma is the most common tumour of the anterior mediastinum. Video-Assisted Thoracic Surgery technique of thymoma resection is spreading world-wide, but the thoracoscopic method is still contentious in many ways. Authors evaluate the early and mid-term results of a 17 years period of VATS unilateral approach at 2 Hungarian thoracic surgical centers. Method. Depending on the anatomical situation of the thymoma, we performed thymectomy, or partial thymectomy (thymomectomy) for the Masaoka–Koga I–II–III stage thymoma from the right or left side through 2 or 3 intercostal ports. CASE: An eighty year old African-American female was evaluated for cough, chest pain, asymptomatic anemia and 21 pound weight loss over a six month period. Computerized tomography (CT) revealed a spiculated 2.8 cm right upper lobe lung nodule, other smaller nodules and lymphadenopathy. Gallium scan revealed abnormal uptake of radiotracer in lacrimal, hilar and mediastinal glands. Broncho-alveolar lavage showed CD4/CD8 ratio of 2:1 with 15% lymphocytes. Biopsy of right upper lobe lesion and mediastinoscopic lymph node biopsy showed numerous matured uniform non-caseating granulomatous inflammati

OBJECTIVE: Urinary tract endometriosis involves the bladder and/or the ureters and is present in approximately 1% of women with endometriosis [1]. Bladder endometriosis is the most frequent type of urinary tract endometriosis, occurring in about 70% to 85% of cases [2,3]. Bladder endometriosis is defined as the presence of endometrial glands and stroma in the detrusor muscle. Surgically, there are 2 ways of excising this disease. The first is by transurethral bladder resection of the tumor, and the second is laparoscopic/robotic/open partial cystectomy of the bladder endometriosis. Because the

BACKGROUND: Surgery for colon cancer is time sensitive, with surgical outcomes influencing long-term oncologic success. A textbook outcome is a measure of an ideal recovery from surgery, free of complications. Obesity increases the technical demands of surgery and places patients at higher risk for complications. Understanding how an operative platform influences the ability to achieve a textbook outcome in patients with obesity is important, as national trends suggest an increasing prevalence.
PURPOSE: To evaluate the association of the operative platform on the rate of textbook outcome in p
