Medical therapy for vestibular schwannoma.

Vestibular schwannomas (VS) symptomatically affect between 1 in 500 and 1 in 1000 people in their lifetime. Most VS cases are sporadic, with no associated tumors or family history, but ∼5% occur in the context of NF2-related schwannomatosis (formerly neurofibromatosis 2). The mainstays of the current management strategy are observation, surgery, and radiosurgery/radiotherapy. However, in the last 12 years, bevacizumab, an antivascular endothelial growth factor (VEGF) antibody, has been utilized for patients with NF2 and rapidly growing tumors or deteriorating hearing. Other drugs have been stu

Vestibular schwannomas: Key research questions and priorities.

Vestibular schwannomas (VS) are the most common tumors in the cerebellopontine angle and represent one of the most challenging skull-base tumors to treat. Over the past decade, studies have significantly improved our understanding of the natural history of these tumors and the genetic alterations that lead to their development. Nevertheless, a number of key questions remain unanswered in VS, including the development of prognostic molecular biomarkers and biomarkers associated with hearing loss, the role of microsurgical resection vs SRS in tumors that are eligible for both modalities, the rol

Spatiotemporal Deconvolution of Hydrophone Response for Linear and Nonlinear Beams-Part I: Theory, Spatial-Averaging Correction Formulas, and Criteria for Sensitive Element Size.

This article reports spatiotemporal deconvolution methods and simple empirical formulas to correct pressure and beamwidth measurements for spatial averaging across a hydrophone sensitive element. Readers who are uninterested in hydrophone theory may proceed directly to Appendix A for an easy method to estimate spatial-averaging correction factors. Hydrophones were modeled as angular spectrum filters. Simulations modeled nine circular transducers (1-10 MHz; F/1.4-F/3.2) driven at six power levels and measured with eight hydrophones (432 beam/hydrophone combinations). For example, the model pred

CrossMoDA 2021 challenge: Benchmark of cross-modality domain adaptation techniques for vestibular schwannoma and cochlea segmentation.

Domain Adaptation (DA) has recently been of strong interest in the medical imaging community. While a large variety of DA techniques have been proposed for image segmentation, most of these techniques have been validated either on private datasets or on small publicly available datasets. Moreover, these datasets mostly addressed single-class problems. To tackle these limitations, the Cross-Modality Domain Adaptation (crossMoDA) challenge was organised in conjunction with the 24th International Conference on Medical Image Computing and Computer Assisted Intervention (MICCAI 2021). CrossMoDA is

EANO guideline on the diagnosis and treatment of vestibular schwannoma.

The level of evidence to provide treatment recommendations for vestibular schwannoma is low compared with other intracranial neoplasms. Therefore, the vestibular schwannoma task force of the European Association of Neuro-Oncology assessed the data available in the literature and composed a set of recommendations for health care professionals. The radiological diagnosis of vestibular schwannoma is made by magnetic resonance imaging. Histological verification of the diagnosis is not always required. Current treatment options include observation, surgical resection, fractionated radiotherapy, and

Tumor Biology of Vestibular Schwannoma: A Review of Experimental Data on the Determinants of Tumor Genesis and Growth Characteristics.

OBJECTIVE: Provide an overview of the literature on vestibular schwannoma biology with special attention to tumor behavior and targeted therapy. BACKGROUND: Vestibular schwannomas are benign tumors originating from the eighth cranial nerve and arise due to inactivation of the NF2 gene and its product merlin. Unraveling the biology of these tumors helps to clarify their growth pattern and is essential in identifying therapeutic targets. METHODS: PubMed search for English-language articles on vestibular schwannoma biology from 1994 to 2014. RESULTS: Activation of merlin and its role in cell s

Effect of Probe-Tone Frequency on Ipsilateral and Contralateral Electrical Stapedius Reflex Measurement in Children With Cochlear Implants.

OBJECTIVES: The upper loudness limit of electrical stimulation in cochlear implant patients is sometimes set using electrically elicited sta … ct stapedius reflexes and assign eSRT levels. In the integrated middle ear analyzer system, an Interacoustics Titan middle ear analyzer was used to perform middle ear measurements in synchrony with research software running on an AB Neptune speech processor, which controlled the delivery of electrical pulse trains at varying levels to the test subject. Immitance audiometry.

Immitance audiometry is a safe, simple, reliable, and relatively objective method of determining middle-ear function that provides advantages for examining the difficult patient because minimal cooperation is needed. Acoustic-immitance measurements obtain sophisticated data that give us valuable information about the middle ear mechanism as a whole. It can be used in a quiet office environment. Immitance audiometry can confirm a doubtful otoscopic diagnosis and screen for ear disease. There are limitations to observer reliability with otoscopy, which has good sensitivity but poor specificity,

Assessment of tympanometry abnormalities of the tympanic membrane.

Isolated abnormalities of the tympanic membrane have been postulated to affect tympanometry. The authors tested 33 abnormal tympanic membranes in 19 patients with tympanosclerosis and/or thin membranes (having no middle or fibrous layer) against a control group of 32 normal ears in 16 age- and sex-matched patients. All selected patients were free of middle ear disease on the basis of normal hearing, normal pneumatic otoscopic findings, and the presence of crossed acoustic reflexes or normal findings at paracentesis. All patients in the control group had normal type A tympanograms, while 20 (61

Transmandibular-cervical approach for treatment of a CSF fistula through the Eustachian tube.

The aim of this article is to explain a surgical approach to solve a complication on a patient treated at our hospital. It is a transmandibular-cervical approach to eliminate a cerebrospinal fluid fistula in the Eustachian tube produced after the removal of an acoustic neurinoma in a patient operated multiple times without success.

Speech: behavior of middle ear muscle during stuttering.

Behavior of the middle ear muscle during speaking was observed in five stutterers by means of the Zwislocki acoustic impedance bridge. Change in impedance did not always parallel precisely the changes in speecli sound level. Impedance changed during the initiation and dluring the course of the stuttering block.
