Twenty-six patients who presented to our shoulder service with a symptomatic meso-os acromiale were reviewed. All had been initially treated for impingement symptoms. Nonoperative treatment had failed in all patients. One patient had also undergone an arthroscopic acromioplasty without benefit. The diagnosis of symptomatic os acromiale was made on the basis of radiographs and point tenderness over the acromion coupled with signs of rotator cuff pathology. We assessed these patients after treatment by internal fixation and bone grafting. Fixation was achieved with either K-wires or screws and t

AIM: The purpose of this study was to identify typical radiological patterns of osseointegration following implantation of the Zweymüller-Alloclassic total hip system.
METHOD: The follow-up included the clinical and radiological evaluation of 115 consecutive hips. For each case the screw direction of the cup was calculated individually to fully observe the bony surroundings by properly tilted X-ray beam.
RESULTS: After 8.1 years, the Harris hip score was 88.5 points on average. Trabecular condensations leading toward the threads of the cup were observed in 100 cases, underlining the importan

BACKGROUND: The Synthes cervical spine locking plate (CSLP) was originally designed by Mörscher and colleagues, who refined it to prevent the migration and loosening of a locking screw by using a cross-split screw head that could be locked into the plate. This eliminated the requirement of the posterior cortex purchase and thereby decreased the risk of spinal cord injury. The purpose of this report is to review our experience using the cervical spine locking plate system and to evaluate its ability to stabilize the cervical spine.
METHODS: Twenty patients with cervical trauma, cervical spondy

We have reviewed retrospectively 80 patients who were treated for traumatic fractures of the femur with a Grosse-Kempf nail to assess the incidence and causes of persisting pain in the proximal thigh. At a mean of 21 months after operation 33 patients had residual pain severe enough to interfere with their lifestyle or mobility. This was in the region of the scar on the greater trochanter in three-quarters of the patients. Only four showed no radiological abnormality. There was nonunion of the fracture in two, Paget's disease in one, breakage of the nail in two and prominence of the proximal l

The variable screw placement (VSP) system uses transpedicular screws and slotted plates to achieve constrained segmental fixation of the spine. A prospective multicenter study of 250 patients was performed under a Food and Drug Administration approved protocol to test the safety and effectiveness of this system. Complete 2-year follow-up data are reported for 169 of 223 (73%) patients who had fixation of three spinal levels or fewer. "Clinical success" was defined as an excellent or good result or improvement of two grades in a 10-point combined pain and function scale. "Fusion success" was de

This study investigated the concept of using plates to attach endoprostheses to bone after segmental resection for bone tumours in an animal model. Titanium alloy plates integrated with the prosthesis and coated with hydroxyapatite were attached to bone by screws. This type of uncemented fixation relied on the induction of periosteal bone formation into and around the plates to secure the implant to bone. Two, three, and six-slotted plate designs were investigated. On retrieval, each plate was securely fixed by new bone. Bone apposition on the external surface of the plates occurred through a

BACKGROUND: Surgery for adult spinal deformity requires optimal patient-specific spino-pelvic-lower extremity alignment. Distal fixation in thoracolumbar spinal deformity surgery is crucial when arthrodesis to the sacrum is indicated. Although we had performed sacro-pelvic fixation with bilateral S1 and bilateral single iliac screws previously, iliac screw loosening and/or S1 screw loosening occurred frequently. So, the authors attempted to fuse spino-pelvic lesions with the dual iliac screws and S1 pedicle screws.
METHODS: Twenty-seven consecutive adult spinal deformity patients underwent th

CASE: A 17-year-old boy presented after a gunshot wound; computed tomography showed a comminuted posterior-wall acetabular fracture and a bullet 90% embedded. Through a Ganz surgical dislocation, a standard 2.5 mm drill bit was advanced 8 to 10 mm into the projectile, its flutes functioning as a self-tapping screw to rock and extract the bullet en bloc, followed by trochanteric osteotomy fixation. He regained painless full activity by 2 months; 33-month imaging confirmed durable union with no osteolysis.
CONCLUSION: The "Motown Corkscrew" technique provides a cost-neutral, controlled method t

Posterior cruciate ligament (PCL) avulsion fractures at the tibial attachment site are managed using various techniques. Some surgeries involve internal fixation with an adjustable double-loop plate, anterior-to-posterior suture suspension fixation, hollow lag screw fixation, and steel wire fixation. In this case, an X-shaped knot and double posteromedial portals are used to retain the posterior septum for fixation. In this technique, we describe double posteromedial portals are used in this method. The internal joint is fixed with an X-shaped knot, and the external joint is fixed with SwiveLo

A clinical case series of three patients is presented using a novel implant design to not only address primary stability but also to prevent damage to the labial bone plate and improve the interdental space for papillae preservation with immediate tooth replacement therapy. This unique implant design features an apicocoronal inverted body-shift in diameter (wide to narrow), shape (tapered to cylindrical), thread depth (deep to shallow), and thread pattern (V-shaped to square) to achieve uncompromised primary stability and esthetics, particularly in extraction sockets, in a singular body form.

Because of the high failure rates, large alveolar cleft defects cannot be successfully closed with bone grafting. Vega introduced the use of the hyrax screw for the closure of such defects by distraction osteogenesis. However, his technique was more invasive with a difficult adaptation of the appliance at the surgical table. To overcome these limitations, a completely tooth-borne trifocal distraction appliance was devised and placed after osteotomy in a 22-year-old repaired group 3 cleft patient who had a 15-mm alveolar defect on the right side. The tooth-borne trifocal distraction appliance w

Mueller-Weiss disease is an alteration of the tarsal navicular that is primarily due to anomalous ossification of the bone and lateral deviation of the talar head associated with screw-like movement through the axis of the subtalar joint. This syndrome tends to be associated with various degrees of flatfoot and hindfoot valgus combined with subtalar joint varus. Ancient cases of Mueller-Weiss disease have not been described in specialized literature. We present the case of an adult male from the Hellenistic period (Ptolemaic dynasty; fourth to first century BC), the skeleton for which was foun

The purpose of this prospective study was to correlate preoperative gadolinium-enhanced MRI scans with intraoperative bleeding of the proximal fragment and postoperative union in a series of consecutive patients with established scaphoid nonunions. In 60 patients (6 females, 54 males) with a mean age of 29 years, scaphoid perfusion was judged preoperatively as normal, impaired or absent using a gadolinium-enhanced MRI scan. Scaphoid reconstruction was performed using a nonvascularized bone graft and screw fixation. Perfusion of the proximal fragment was assessed intraoperatively in 49 of 60 pa

Femoral neck fracture as a complication of slipped capital femoral epiphysis (SCFE) is rare. Even rarer is a femoral neck nonunion as an additional complication. This is the first case reported in the literature of a failed valgus osteotomy for a femoral neck nonunion. A salvage operation involving a step-cut valgus/flexion/internal rotation osteotomy, open reduction and internal fixation, with a blade plate and cannulated screw, placement of an allograft femoral strut, and allograft bone grafting was successfully performed. Femoral neck fractures following SCFE fixation are more difficult to

Among the patients with a cervical injury admitted to our clinic between 1980 and 1988, 18 cases underwent a ventral fusion operation (Cloward) and 8 of them needed additional vertebrectomy. The injury in all of these cases was due to falls and presented with major compression fractures. In each case the vertebral body was resected and replaced by a "T" shaped iliac bone graft. The graft was fixated by a screw or wire when necessary, than the patients were immobilized by a bed rest for 2 weeks duration. During the follow-up period, cervical alignment was found to be stable in all patients. The

BACKGROUND: The temporary use of external fixators is an established method for stabilising fractures, particularly for injuries that do not initially allow definitive treatment. Despite its frequent use, it remains unclear whether the interval between fixator application and subsequent definitive fracture treatment affects microbial colonisation of the fixation screws prior to internal fixation.
METHODS: This prospective, single-centre study included patients aged 18 years or older between April 2023 and April 2024 who received a temporary fixator during initial fracture management. During d

BACKGROUND: Factors associated with early re-collapse after posterior fixation of thoracolumbar burst fractures have been sparsely examined in previous studies. Early re-collapse is challenging to predict due to multifactorial risk. Therefore, we aimed to investigate factors associated with vertebral re-collapse within the first 3 months following posterior fixation.
METHODS: This retrospective study used data from the Swedish Fracture Register (2015-2019) and included patients aged 18 to 66 years with a single-level thoracolumbar burst fracture between T11 and L3 treated with posterior fixat

OBJECTIVE: Considering the high incidence and complexity of unstable posterior pelvic ring fractures, and the need for less invasive and more effective treatment options, this study aims to introduce a novel minimally invasive, safe, and simple internal fixation method for the treatment of unstable posterior pelvic ring fractures using the sacroiliac joint locking plate (SJP) system, and to provide biomechanical validation and clinical evaluation of this method.
METHODS: Biomechanical research was conducted using standard pelvic bone models from Synbone, Switzerland, to create Denis II zone u
