[Role of an anesthesiologist-resuscitation specialist in organ donation for transplantation].

There is an annual reduction in the number of donors worldwide. An anesthesiologist-resuscitation specialist is a key figure in the whole sy … tween the treatment of a patient with multiple organ dysfunction and the management of a donor with brain death.

Placental transfusion insult in the predisposition for SIDS: a mathematical study.

A difference has been observed between the newborn hearing screening tests of thirty-one SIDS cases versus control infants that survived the first year of life [Rubens DD, Vohr BV, Tucker R, O'Neil CA, Chung W. Newborn oto-acoustic emission hearing screening tests. Preliminary evidence for a marker of susceptibility to SIDS. Early Hum Dev 2008;84(4);225-9]. This study is motivated by the hypothesis that the predisposition for SIDS may be caused by inner ear and brainstem damage from a high venous pressure insult at birth that disrupts an infant's ability to detect rising CO(2) levels following

Vaginal delivery using cranioclasia following prenatal diagnosis of a giant fetal intracranial teratoma.

Intracranial teratoma, although a rare disease, is nonetheless the most common of the congenital tumors of the central nervous system in the neonatal period. It can be diagnosed antenatally by ultrasound. The prognosis for fetal forms of teratoma is very grim. We report here the case of an immature teratoma identified at 26 weeks' gestation and discuss its obstetric consequences, in particular, the technical procedures for a medically-indicated termination of pregnancy in view of the massive increase in head circumference associated with this tumor.

Islet isolation and transplantation from an annular pancreas: a case report.

CONTEXT: Annular pancreas is an uncommon congenital anomaly formed by a thin band of normal pancreatic tissue encircling the duodenum. CASE REPORT: We report the first case of an islet isolation and transplantation from an annular pancreas. The pancreas together with duodenum was procured from a 32-year-old previously healthy man after diagnosis of brain death. The pancreas including the annular portion was distended well after intraductal collagenase perfusion. A total of 276,064 islet equivalent was recovered and transplanted into a type 1 diabetic patient. CONCLUSIONS: Bearing in mind the…

Differences in survival, brain damage, and cerebrospinal fluid cytokine kinetics due to meningitis caused by 3 different Streptococcus pneumoniae serotypes: evaluation in humans and in 2 experimental models.

BACKGROUND: Experimental meningitis with Streptococcus pneumoniae serotypes 1, 3, and 9 has resulted in pronounced differences in disease severity, but clinical meningitis studies addressing serotype-related differences in case-fatality rates are lacking. METHODS: Study subjects were Danish patients with pneumococcal meningitis due to serotype 1 (n=38), 3 (n=69), or 9V (n=59) during 1990-2002 for whom clinical information was available. The 3 serotypes were tested for brain damage and cerebrospinal fluid (CSF) inflammatory kinetics in 2 experimental models of meningitis. RESULTS: Patients wi

Repeated cerebral ischemia induced hippocampal cell death and impairments of spatial cognition in the rat.

We developed a method of causing strong ischemic insult only in vulnerable nerve cells, such as hippocampal cells, without causing hemiplegia or difficulty in moving, by repeating cerebral ischemia for a brief time with a short interval periods. The rats subjected to 10 min of cerebral ischemia exhibited no impairment of spatial cognition at the test trial 7 days after final reperfusion. However, when the 10 min ischemia was repeated twice with a 1 hr interval, the rats exhibited a significant decrease in number of correct choices and increase in number of errors. Three times of repeated cereb

[Involvement of excitatory amino acids in the brain damage of neonatal asphyxia].

Besides their role as neurotransmitters, excitatory amino acids (EAAs) in the developing brain are crucially involved in plasticity and excitotoxicity which are modified by their distinct ontogeny. Postsynaptic EAA system activities, particularly of the N-methyl-D-aspartate and quisqualate receptors, are transiently enhanced early in life. This transient enhancement is presumably beneficial to the immature brain because physiologic activation of the EAA system plays a critical role in plasticity of early learning and morphogenesis. At the same time, this transient hypersensitivity renders the

Wernicke's encephalopathy and central pontine myelinolysis induced by hyperemesis gravidarum.

A 21-year-old woman developed hyperemesis gravidarum, complicated by Wernicke's encephalopathy and central pontine myelinolysis. For the latter, time course of the lesions on magnetic resonance imaging suggests a pathogenetic role for hypophosphatemia rather than for hyponatremia.

[Birth injuries in calves and perinatal calf losses in a large dairy herd].

Post-mortem examinations were carried out on 65 calves which were stillborn or died within 24 hours of birth. All calves came from a large dairy unit with about one thousand cattle of the Schwarzbuntes Milchrind breed. To evaluate the perinatal losses, the farm records of parturitions from 1985-1990 were used. The calves had gross damage of the CNS, the musculature, subcutaneous tissue and internal organs such as lung and intestines. It may be assumed that a lesser degree of similar damage would also be present in the surviving calves. The level of perinatal losses is determined particularly b

Correlates of survival and the Daumas-Duport grading system for astrocytomas.

In order to examine the correlation between prognosis and the histological features of nuclear atypia, mitosis, endothelial proliferation, and necrosis in supratentorial adult astrocytomas, the authors reviewed 251 such cases treated at the Massachusetts General Hospital between 1972 and 1980. One point was given for the presence of each feature. The total number of features was translated into a grade as follows: none of the four features = Grade 1 (one patient), one feature = Grade 2 (36 patients), two features = Grade 3 (33 patients), and three or four features = Grade 4 (181 patients). The

[Mitral valve prolapse: a severe abnormality?].

The reputation of mitral valve prolapse being a benign condition is based to a great extent on the fact that complications are rare in minor forms, but also because a number of studies of the condition included normal subjects, especially of the female sex. The prevalence of mitral valve prolapse in the general population is 4 to 5%. Approximately 20% of these patients have marked redundancy of valve tissue and are particularly exposed to complications. The incidence of infective endocarditis in cases with an audible murmur is 0.05% per year. The incidence of mitral regurgitation increases wit

Nutrition during ten-week life support with successful fetal outcome in a case with fatal maternal brain damage.

A 31-year-old woman had fatal intracerebral bleeding at the beginning of the 21st week of gestation. After several days, there was evidence that the brain was dead. Combined enteral and parenteral nutrition was continued until the 32nd week of gestation when cesarean section was performed because of drug-resistant hypotension. A full-term normal 1600-g male was delivered and the later development of the child was normal. This case report demonstrates the possibility of providing nutritional requirements to the fetus even if the mother has had a fatal injury.

Sudden infant death syndrome: 1987 perspective.

The pathophysiology of SIDS remains unknown. Although a multifactorial cause appears plausible on the basis of available data, new data are needed to determine which components of this multifactorial hypothesis are most important and whether other factors need to be added. We need to better understand control of breathing in the newborn infant and the manner in which maturation of cardiorespiratory control progresses during infancy. The unique period of vulnerability for SIDS, in which risk is less in the neonate than at 2 to 6 months of age, remains unexplained. Is there a worsening in some a

Cot deaths in Australia, 1985.

A survey has been carried out of the pathology of cot deaths in Australia. Infection seems to play a bigger role in deaths in Melbourne than in some other areas such as Western Australia, but a smaller role than in the United Kingdom. The number of subclinical lesions that are found in infants who have suffered a cot death seems to be similar to that in the United Kingdom. The high cot death rate in Tasmania appears to be a truly elevated rate and not due to diagnostic variables. The "possibly preventable" postperinatal death rate in several areas is high. Many individuals are carrying out exc
