Substituted judgment in health care decision-making: An evolutionary concept analysis.

BackgroundSubstituted judgment guides health care decisions for patients who lack decision-making capacity, yet its meaning remains unsettled across legal, clinical, philosophical, and technological literatures. In practice, it is often conflated with best interests, family preference, and clinician judgment, while artificial intelligence preference predictors have renewed debate about what the standard is meant to preserve.AimTo clarify the concept of substituted judgment in health care decision-making using Rodgers' evolutionary method of concept analysis.Research designAn evolutionary conce

Personalizing BPH Management: Bladder Stone Removal Alone vs. With Concomitant Laser Enucleation-A Multicenter Perspective With Patient Reported Outcomes and Decisional Regret Analysis.

INTRODUCTION: The necessity of routinely combining bladder stone removal with surgical treatment for BPH remains debated. We compared the efficacy and safety of bladder stone removal with and without concomitant outlet surgery in patients with BPH. METHODS: We conducted a multicenter ambispective cohort study comparing bladder stone removal alone vs. removal with concomitant holmium laser enucleation of the prostate (HoLEP). BPH patients with bladder stones > 1 cm were included. Exclusion criteria ruled out alternative causes of bladder stones. The "stone removal-only" group (n = 63) underwen

Factors associated with knowledge, confidence, self-efficacy, and satisfaction in African American men's decisions about prostate cancer screening.

BACKGROUND: African American men (AAM) have persistently had the highest incidence and mortality rates for prostate cancer (PrCa) in the United States. Considering that current guidelines recommend the practice of shared decision-making to determine whether or not to undergo a Prostate-Specific Antigen (PSA) test for the early detection of PrCa, this study focuses on the identification of key factors influencing AAM decisions regarding having or not having PSA screenings. METHODS: Cross-sectional study of 152 AAM who completed study surveys. Statistical analysis included frequencies, means, a

Veterinarians' brief guide to confidence intervals, standardized effect size, and number needed to treat: understanding the impact of treatments and disease.

Veterinarians in all settings strive to practice evidence-based medicine, but current methods of reporting scientific study results create barriers to clinical translation of findings. The p-value is the most commonly used measure of statistical significance, but fails to convey information about clinical relevance of findings. In this article, the limitations of the p-value as a standalone measure and the importance of reporting additional metrics such as confidence intervals (CI), standardized effect size (ES), and number needed to treat (NNT) in veterinary research are discussed. Confidence

Perioperative management of antihypertensive agents.

A large proportion, possibly over half, of patients presenting for preoperative evaluation will be taking antihypertensive agents. The multiple classes of agents and their use in different combinations can make management decisions challenging. Poor blood pressure control and lack of evidence or conflicting evidence for certain agents can further complicate management. Appropriate antihypertensive management is important because it can have an effect on meaningful perioperative outcomes, including mortality. In this review, we discuss the factors that should be considered when making preoperat

Challenges in the provision of ICU services to HIV infected children in resource poor settings: a South African case study.

The HIV/AIDS epidemic has placed increasing demands on limited paediatric intensive care services in developing countries. The decision to admit HIV infected children with Pneumocystis carinii pneumonia (PCP) into the paediatric intensive care unit (PICU) has to be made on the best available evidence of outcome and the ethical principles guiding appropriate use of scarce resources. The difficulty in confirming the diagnosis of HIV infection and PCP in infancy, issues around HIV counselling, and the variance in the outcome of HIV infected children with PCP admitted to the PICU in African studie

Feasibility Drives Adoption of Volumetric Stone Measurement: Results from an International Survey.

INTRODUCTION: Accurate assessment of stone burden is essential for clinical decision-making in urolithiasis. Although stone volume is considered a more accurate representation of stone burden than maximal diameter, most clinical practice continues to rely on linear measurements. The extent to which volumetric assessment is used in daily practice remains unclear. Therefore, this study aimed to evaluate current global practices in stone burden assessment and identify factors associated with the adoption of volumetric measurement. MATERIALS AND METHODS: An international cross-sectional survey wa…

Adhesive small bowel obstruction in the virgin abdomen: comparable phenotype, divergent management.

BACKGROUND: In small bowel obstruction (SBO), the absence of prior abdominal surgery is commonly interpreted as a marker of higher-risk pathology and is often associated with a lower threshold for early operative intervention. Whether this assumption reflects differences in disease phenotype or decision-making remains uncertain. METHODS: This post hoc analysis of a prospective, multicenter cohort study included consecutive adults with CT-confirmed mechanical SBO. Analyses were restricted to patients with adhesion-related SBO to enable comparison between virgin abdomen adhesive SBO (vaASBO) an

Feasibility of mK-line and mSC-line in segments choice for laminoplasty of multilevel cervical spondylotic myelopathy.

OBJECTIVES: Laminoplasty (LAMP) is a common procedure for multilevel cervical spondylotic myelopathy (MCSM). The traditional K-line is a guide for LAMP candidate selection but is inferior to the modified K-line (mK-line) in predicting clinical outcomes. The spinal cord line (SC-line) is another indicator that considers anterior compression but is not typically used for selecting surgical segments. This study intended to propose and validate the combined application of modified spinal cord line (mSC-line) with mK-line for surgical decision-making in MCSM patients. METHODS: This study included
