This study was undertaken to explore the prevalence of perceived dysphoria in a poor, urban, predominantly Puerto Rican community. A cross-sectional sample of 704 adult respondents were asked a single validated question, with a five-item Likert scale response, about their level of perceived dysphoria. Chi-squared analysis and Spearman's correlation coefficients were used to assess the association of level of dysphoria with social characteristics, barriers to health care, health status, and substance use. During a 6-year period, 65 consecutive patients who had undergone anterior communicating artery aneurysmal surgery were reviewed at a follow-up examination from 6 to 78 months (mean 35 months) after operation. On admission 69% of cases had a good Hunt and Hess scale (grades I to II) and 31% a poor grade (grades III to V). The degree of subarachnoid hemorrhage (SAH) was determined by Fisher's grade. Two hundred and sixty eight microbial strains were tested for their antibiotic susceptibility. Azithromycin (sumamed) was shown to have an efficient inhibitory action on pneumococci, streptococci and hemophilic bacilli. Doxycycline and gentamicin were not sufficiently active against gram-positive cocci, the same as trimethoprime/sulfamethoxazole which was equally not sufficiently active against hemophilic bacilli. Adequate therapy of bronchopulmonary diseases requires the pathogen antibioticograms.

Rupture and secondary infection are common complications of hydatid cyst in the liver. Ultrasound and CT findings are reported in a case of hydatid cyst which has ruptured directly into the left colon. Rupture of hydatid cyst into a hollow viscus is extremely rare. CT demonstrated partial drainage of the cyst contents with the creation of an air-fluid level.

Oculocutaneous albinism (OCA) is a recessively inherited genetic condition prevalent throughout sub-Saharan Africa. We now describe a cluster of tyrosinase positive OCA (OCA2) cases belonging to the Tonga ethnic group living in the Zambezi valley of northern Zimbabwe. One-hundred and twenty-seven digits in Verdan zones I and II, and 24 thumbs in 106 patients were reviewed from 1979 to 1995. Patients were all followed for a minimum of 5 months to assess early results and complications, and to determine if a short course (minimum 3 days) of dextran 40 anticoagulation adversely affected digit survival. Carcinoma of the ampulla of Vater is an uncommon malignancy often treated at tertiary referral centers. Most published series are derived from these centers and show resectability rated of 80 to 90 percent, with overall 5 year survival rates of 25 to 60 per cent. This survey estimates the frequency of various adverse effects of the use of the drug cannabis. A sample of 1000 New Zealanders aged 18-35 years were asked to complete a self-administered questionnaire on cannabis use and associated problems. The questionnaire was derived from criteria for the identification of cannabis abuse which are analagous to criteria commonly used to diagnose alcoholism.
