Economic evaluation of digital health interventions to improve quality-adjusted life years in adolescents living with HIV in Ethiopia.

INTRODUCTION: The rapid expansion of mobile phone access has enabled low-cost SMS interventions to support adherence to antiretroviral therapy (ART). Although text messaging improves adherence and retention in HIV care, its cost-effectiveness for enhancing quality of life remains uncertain, particularly among adolescents in resource-limited settings. This study evaluated whether SMS reminders improve retention in care and quality-adjusted life years (QALYs) among adolescents living with HIV in Ethiopia. We hypothesized that SMS reminders would increase adherence and retention, leading to measu

Changes in hepatitis C virus infections after implementation of an expanded treatment program in New Mexico state prisons.

BACKGROUND: Numerous leading health organizations have developed plans to eliminate hepatitis C virus (HCV), but the success of these efforts can only be assessed through systematic monitoring of infections. People in prison are at very high risk for HCV infection and New Mexico (NM) prisons have one of the highest rates of HCV in the US For nearly two decades NM Corrections Department (NMCD) and Project ECHO® have been partnering to treat HCV in NM state prisons, including a significant expansion in treatment in 2020. Since the start of the treatment scale-up in 2020, 2981 people have been tr

Rowing Against Climate Adversity and Lack of Family Support: The Everyday Lives of Migrants' Wives in Rural Mozambique.

There are numerous studies on the participation in migratory work by men from the Gaza Province, whether to the mines in South Africa or Mozambican cities, in particular the city of Maputo. However, studies that analyze the psychosocial conditions of migrants' wives about their relationships with their family, friends, neighbors, and their teenage children are still incipient. For this reason, we assert that this study is typical of sub-Saharan Africa and reveals the vulnerability of middle-age and aging women. In all these studies, there are common aspects that characterize the vulnerability

Older Adult Sepsis Survivors Discharged to Skilled Nursing Facilities: Age-Adjusted Charlson Comorbidity Index as a Predictor of 6-Month Mortality.

BACKGROUND: Sepsis is a critical global health issue, particularly affecting older adults. Despite advances in acute sepsis management, the long-term outcomes for survivors, particularly those transitioning to skilled nursing facilities (SNFs), remain poorly characterized. AIM: To evaluate the prognostic value of the age-adjusted Charlson Comorbidity Index (ACCI) in predicting 6-month mortality among older adult sepsis survivors discharged to SNFs. STUDY DESIGN: An observational cohort study of older adult sepsis patients in the intensive care unit (ICU) of a tertiary academic medical centre

Canakinumab for the treatment of postprandial hypoglycaemia: study protocol for a randomised, placebo-controlled, parallel-group, double-blind, multicentric, superiority trial-the CanpHy study.

INTRODUCTION: Postbariatric hypoglycaemia (PBH) is a complex medical condition with a significant impact on patients' quality of life. The underlying mechanisms remain to be elucidated. We have shown that food ingestion increases IL-1β and subsequently stimulates insulin secretion. We therefore hypothesised that overactivation of the IL-1β pathway could lead to PBH by promoting excessive insulin secretion after a meal. In a proof-of-concept study, we have shown that acute treatment with the IL-1 receptor antagonist anakinra can attenuate PBH after a single liquid mixed meal. This study aims to

Barriers to therapy adherence in narcolepsy.

BACKGROUND: Treatment adherence (TA) in narcolepsy is a complex phenomenon influenced by various factors beyond patient-related aspects. The management of narcolepsy involves non-pharmacological and symptomatic pharmacological treatment. Factors such as chronic daytime sleepiness, cognitive deficits, psychiatric comorbidities and adverse effects of pharmacological treatment are aspects of narcolepsy that could undermine TA, impacting patients' ability or willingness to consistently follow treatment plans. The aim of this study was to identify the factors influencing TA in narcolepsy and to det

Monthly engagement with EIP keyworkers was associated with a five-fold increase in the odds of engagement with psychosocial interventions.

BACKGROUND: Early intervention in psychosis (EIP) supports people who are experiencing their first episode of psychosis (FEP). A new Model of Care (MoC) for EIP services was launched in Ireland in 2019. Three EIP demonstration sites were chosen to test this MoC through a 'hub and spoke' approach. These services were a new way of organising care for people experiencing FEP, based upon a recovery model of care, and which sought to standardise care, improve access by clinically led multidisciplinary teams. This included newly created EIP keyworker roles whereby keyworkers assumed responsibilities

Fertility trends by HIV status in a health and demographic surveillance study in Magu District, Tanzania, 1994-2018.

BACKGROUND: Sub-Saharan Africa (SSA) has the highest fertility rates and highest HIV disease burden globally. However, it is not clear how the rapid expansion of anti-retroviral therapy (ART) for HIV has impacted the fertility gap between HIV-infected and uninfected women. We used data from a Health and Demographic Surveillance System (HDSS) in north-western Tanzania to explore trends in fertility rates and the relationship between HIV and fertility over the 25-year period. METHODS: From 1994 to 2018, births and population denominators from the HDSS population were used to obtain age-specific

Planning for Mpox on a College Campus: A Model-Based Decision-Support Tool.

BACKGROUND: In spring and summer 2022, an outbreak of mpox occurred worldwide, largely confined to men who have sex with men (MSM). There was concern that mpox could break swiftly into congregate settings and populations with high levels of regular frequent physical contact, like university campus communities. OBJECTIVE: To estimate the likelihood of an mpox outbreak and the potential effect of mitigation measures in a residential college setting. DESIGN: A stochastic dynamic SEIR (susceptible, exposed but not infectious, infectious, or recovered) model of mpox transmission in a study popula

Parental leave in medical school: supporting students as parents.

CONTEXT: The overlap between medical school, residency, and childbearing potential increases the likelihood a woman will pursue parenthood within her, or her partner's, medical training. Parental leave benefits mothers, fathers, and infants. Adequate parental leave promotes physical recovery, mental health, infant bonding, improved breastfeeding, appropriate childhood immunization, and familial engagement. Despite the risks and benefits, the United States does not have national paid maternity, paternity, or parental leave requirements. Complicating matters for medical trainees, parental leave

Systematic identification of plausible pathways to potential harm via problem formulation for investigational releases of a population suppression gene drive to control the human malaria vector Anopheles gambiae in West Africa.

BACKGROUND: Population suppression gene drive has been proposed as a strategy for malaria vector control. A CRISPR-Cas9-based transgene homing at the doublesex locus (dsxFCRISPRh) has recently been shown to increase rapidly in frequency in, and suppress, caged laboratory populations of the malaria mosquito vector Anopheles gambiae. Here, problem formulation, an initial step in environmental risk assessment (ERA), was performed for simulated field releases of the dsxFCRISPRh transgene in West Africa. METHODS: Building on consultative workshops in Africa that previously identified relevant envi

Motherhood and decision-making among women living with HIV in developed countries: a systematic review with qualitative research synthesis.

BACKGROUND: Women living with HIV (WLH) lack evidence-based information about reproductive options while managing pressures from family, clinicians, and communities to give up the idea of having children. As the reproduction intentions of WLH are not well understood, stigmatizing behaviors force them to hide their disease to avoid rejection by their family, partner, and social networks. Compliance with social norms, fear of stigma, and discrimination influence their experience. Current research is individual qualitative studies lacking the synthesis perspective necessary to guide intervention
