Patulous internal auditory canal.

OBJECTIVE: To investigate a rare anomaly of the internal auditory canal known as a patulous canal and its relationship to hearing impairment. METHODS: High-resolution computed tomographic scans of the temporal bones of patients who presented between August 2001 and August 2002 were reviewed. The patients' medical charts were evaluated for age, sex, and hearing impairment, and the computed tomographic scans were examined for the presence of a patulous canal. RESULTS: The study group included 645 patients who underwent high-resolution computed tomography of the temporal bones for various reaso

Thymus, kidney and craniofacial abnormalities in Six 1 deficient mice.

Six genes are widely expressed during vertebrate embryogenesis, suggesting that they are implicated in diverse differentiation processes. To determine the functions of the Six1 gene, we constructed Six1-deficient mice by replacing its first exon by the beta-galactosidase gene. We have previously shown that mice lacking Six1 die at birth due to thoracic skeletal defects and severe muscle hypoplasia affecting most of the body muscles. Here, we report that Six1(-/-) neonates also lack a kidney and thymus, as well as displaying a strong disorganisation of craniofacial structures, namely the inner

Imaging findings of cochlear nerve deficiency.

BACKGROUND AND PURPOSE: High-resolution T2-weighted fast spin-echo MR imaging provides excellent depiction of the cisternal and intracanalicular segments of the vestibulocochlear and facial nerves. Absence or reduction in caliber of the cochlear nerve (deficiency) has been described in association with congenital sensorineural hearing loss (SNHL). Depiction of cochlear nerve integrity may be important for diagnosis and management of SNHL. METHODS: We retrospectively reviewed high-resolution T2-weighted fast spin-echo MR images of 22 patients examined for SNHL who had deficiency of the cochlea

Comparison of measures of frequency resolution and recruitment in patients undergoing neuro-otological investigation.

Loudness recruitment and reduced frequency resolution both occur in cochlear types of hearing loss. One theory of loudness recruitment suggests that, as intensity is coded partly by spread of excitation across the nerve fibre array, recruitment is a direct consequence of the broad spread of excitation associated with poor frequency resolution. The present study investigated the relationship between these two quantities. The study involved a simple measure of frequency resolution (three-point psychoacoustical tuning curve, PTC) and conventional measures of recruitment obtained from patients und

ABR amplitude and dispersion variables. Relation to audiogram shape and click polarity.

ABR amplitude behaviour to condensation (C) and rarefaction (R) clicks was investigated in normal ears, ears affected by Ménière's disease and ears with high-frequency hearing loss (HF ears). The wave IV-V amplitude-intensity function was steeper in ABRs evoked by R than by C clicks. This may suggest that two different cochlear generator components, one intensity-dependent as well as one polarity-dependent, contribute to click-evoked ABRs. Wave IV-V amplitude was significantly higher in Ménière ears compared to HF ears in spite of comparable 2-4 kHz hearing loss in the two groups. Hence, audio

Endolymphatic sac ballooning surgery for Menière's disease.

A newly developed technique (endolymphatic sac ballooning surgery [ESBS]), based on revisional sac surgery observations, was conducted on 135 patients with intractable Meniere's disease. The surgery involves the insertion and fan-folding of a thin Silastic sheet in the endolymphatic sac (ES) lumen to increase its volume. The results of this sac ballooning surgery proved more effective for treatment of incapacitating Meniere's disease symptom complex than did primary ES surgery (ESS) with either the nonballooning sac techniques we employed previously or ESBS revision operations in which primary

Hearing preservation following a transtemporal resection of an acoustic schwannoma: a case report.

The transtemporal or translabyrinthine approach is generally regarded as the most reliable method of ensuring total tumor removal and preservation of facial nerve function in the resection of a cerebellopontine angle tumor. This is particularly true in removing a tumor that has a significant extension into the patient's internal auditory canal. An anticipated deficit associated with the approach is a total hearing loss in the patient's hearing on the operated side. We have encountered a patient, however, who was serially documented as having serviceable hearing after a transtemporal removal of

Screening and assessment of childhood deafness: experience from a centralized multi-disciplinary service.

A paediatric hearing assessment unit was set up in Lothian in 1978 to provide a clear referral pathway for children with suspected sensorineural hearing impairment and particularly for those children detected by the health visitor screening test. Over a 6-year-period the incidence of bilateral sensorineural deafness was 1.3/1000 suggesting that all children in the region were being seen. The mean age of diagnosis fell significantly over this period. Children in high risk groups, through adverse perinatal events or a positive family history, were not detected any earlier than other children. Th

Clinical verification of a unilateral otolith test.

In a previous study we reported promising results for a new test to differentiate in vivo unilateral otolith functions. That study pointed to a need for further validation on known pathological cases. In this presentation we will detail the results gathered on a group of clinically verified vestibular defectives (verum) and a normal (control) group. The subjects in the verum group were former patients of the ENT clinic of the university hospital. These subjects had usually suffered from neurinoma of the VIIth cranial nerve or inner ear infections. All had reguired surgical intervention includi

[Vestibular compensation. Review of the literature and clinical applications].

Vestibular compensation is an excellent model for the study of plasticity of the adult central nervous system. Therefore it has been the subject of several studies in humans and animals, which will be briefly summed up by the authors. Lesions of the labyrinth or vestibular neurectomy are immediately followed of postural and oculomotor disorders, as well as by dynamic deficits of the various vestibular reflexes (vestibulo-ocular and vestibulonucal reflexes). While the former problems always recede in all species, the restoration of the dynamic properties of vestibular reflexes largely depends u

Vestibular nerve section and saccus decompression: an evaluation of long-term results.

The 1972 AAOO committee (Alford, 1972) guidelines brought some uniformity into the evaluation of therapy for Menière's Disease. We have adhered to its recommendations in this long-term follow-up report of 21 saccus decompressions and 29 vestibular nerve sections performed on 46 patients between 1968 and 1977. Comparisons between these and other groups have been possible with regard to: control of vertigo; hearing; tinnitus; and development of hydrops in the contralateral ear. All the vestibular nerve section group have enjoyed sustained relief from vertigo. Class D results (recurrent vertigo)

Internal auditory canal vascular loops: audiometric and vestibular system findings.

Prominent loops of the anterior inferior cerebellar artery in the cerebellopontine angle are found frequently during anatomic studies of this region. These vascular loops are suspected of causing hearing loss, tinnitus, and vertigo, and surgery has been advocated to separate the vascular loop from the eighth cranial nerve. Previous reports have described pathologic anatomy, surgical approaches, and results of treatment. In this study, we report the results of a uniform battery of audiometric and vestibular system test results administered to fifteen patients with prominent vascular loops in th

Endolymphatic mastoid shunt for treatment of Meniere's disease: a five year study.

The endolymphatic mastoid shunt seems to be an effective treatment for those patients whose Meniere's disease is refractory to medical and/or supportive treatment. This series is composed of 48 patients who were followed for up to 5 years postoperatively, with 1 year being the minimum follow-up period; 81% of these patients obtained a satisfactory relief of their vertigo. The morbidity of the procedure is low with no severe or total hearing loss reported in this series. Surgery should not be recommended until the symptoms have caused severe incapacitation; however, this delay must be tempered

Acoustic neuroma surgery. Translabyrinthine-transtentorial approach via the middle cranial fossa.

In our approach for acoustic tumors, the method of Morrison and King and that of Bochenek and Kukwa have been modified into one method. This modified method is basically a neuro-otological-neurosurgical team approach, extending the operative field by drilling the temporal bone and cutting the superior petrosal sinus, tentorium, and posterior fossa dura according to the size of the tumor. Therefore, for tumors slightly protruding into the posterior fossa from the prous of the internal auditory canal, only the bone adjacent to it is removed (Bochenek et al's method). For larger tumors, labyrinth
