PURPOSE: Optimal treatment of chronic unstable acromioclavicular (AC) joint dislocations (stage 3-5 according the Rockwood classification) is still debated. Anatomic coracoclavicular (CC) reconstruction is a reliable option in terms of two-dimensional radiographic reduction, clinical outcomes, and return to sports, but there remain concerns regarding anterior-posterior stability of the AC joint with CC ligament reconstruction alone. The aim of the present study was to describe the mid-term results of a new hybrid technique with CC and AC ligament reconstruction for chronic AC joint dislocation

BACKGROUND: Intensive robot-assisted arm training in the chronic phase of stroke recovery can lead to clinical improvement. Combinatorial therapeutic approaches are sought to further optimize stroke recovery. Transcranial direct current stimulation (tDCS) is one candidate to combine with robotic training, as transient increases in excitability and improvements in motor behavior have separately been reported.
OBJECTIVE: To determine whether tDCS, delivered prior to robotic training, could augment clinical improvement.
METHODS: We conducted a dual-site, randomized controlled trial in 82 chroni

BACKGROUND: Throwing athletes with ulnar collateral ligament (UCL) injury and symptomatic valgus instability can expect to return to the same or higher level of play. Reconstruction with tendon graft is the dominant method of surgical treatment. Recent evidence suggests that spanning the joint with a suture anchored on both sides is biomechanically equivalent to reconstruction, with faster time to return to play. The authors developed a hybrid UCL reconstruction technique augmented with a suture brace to improve joint stability.
PURPOSE/HYPOTHESIS: The purpose of this study was to biomechanic

INTRODUCTION: Traumas of large vessels of mediastinal and shoulder girdle cause significant life risk and constitute serious diagnostic and therapeutic problems because of lack of time for accurate treatment planning. Particularly difficult to treat are blunt trauma of large arteries in patients under the influence of drugs or with a progressive hypovolemic shock.

UNLABELLED: The aim of our study is to analyse the results of our surgical technique for the treatment of proximal humeral fractures and fracture dislocations using locking plates in conjunction with calcium sulphate bone-substitute augmentation and tuberosity repair using high-strength sutures. We used the extended deltoid-splitting approach for fracture patterns involving displacement of both lesser and greater tuberosities and for fracture-dislocations. Optimal surgical management of proximal humeral fractures remains controversial. Locking plates have become a popular method of fixation. H

We retrospectively reviewed supracondylar distal humeral fractures that had been treated with fracture-specific plating and definitive fixation through a lateral approach with a medial triceps mobilization technique. We determined the incidences of preoperative and postoperative radial nerve palsies (RNP) to evaluate the impact of the plating and fixation technique on RNP. Seventy-three patients treated at our institution from 2006 through 2009 were included in the study. The patients were assigned to 2 groups: 57 to the blunt injury group (motor vehicle collisions, falls, struck pedestrians,

Secondary surgery following obstetric brachial plexus palsy is usually performed between two and six years of age, but also later when the patient presents later. Surgery consists of contracture releases and transpositions of muscles and tendons. Indication for surgery must be assessed and discussed individually. Only a real functional improvement in ADL is a success. We describe usual techniques according to topography and present an overview of our results. This knowledge should influence all decisions about reconstructive surgery in these children.

Streptococcal-toxic-shock syndrome is caused by virulent strains of exotoxin-producing streptococcus, almost always group-A organisms such as Streptococcus pyogenes. It has often been described in the setting of surgical wounds, burns, childbirth, diabetics, elderly, neonates, and immunocompromised hosts, where the portal of entry is the skin. Our patient was on steroids and nonsteroidal anti-inflammatory drugs for chronic rheumatoid arthritis and developed this deadly infection after a fall.

BACKGROUND: Achieving stable fixation in highly unstable proximal humeral fractures (PHFs) remains a significant challenge in orthopedic surgery. Research increasingly highlights the importance of medial calcar support in these complex injuries. This study aimed to compare and analyze the clinical outcomes of two surgical approaches for treating Neer three- and four-part PHF in elderly patients: fixation with the proximal humeral internal locking system (PHILOS) alone, and fixation with PHILOS augmented by a bionic nanohydroxyapatite/polyamide 66 (n-HA/PA66) composite material.

Tendon injuries and other tendon disorders are a source of major concern in competitive and recreational athletes and in many working conditions requiring repetitive movements. The exact etiology, pathophysiology, and healing mechanisms of the various tendon complaints are, however, only partly known and even origin of pain in the chronic tendon disorders is unknown. Thus, the treatment strategies recommended for tendon complaints vary considerably and the given treatment is frequently based on empirical evidence only. Corticosteroid injections are one of the most commonly used treatments for

BACKGROUND: Osteoarthritis of the sternoclavicular (SC) joint is a rare condition that leads to decreased function and persistent pain, ultimately altering the function of the shoulder and keeping individuals from their desired activities. SC resection in the setting of primary and posttraumatic osteoarthritis is the most common surgical treatment for these patients, but midterm results are lacking.
PURPOSE/HYPOTHESIS: The purpose was to assess the clinical outcomes, pain levels, return to sports rate, and survivorship after open SC joint resection in the setting of painful primary SC joint o

BACKGROUND: Proximal humeral fractures are traditionally treated with open reduction and internal fixation (ORIF), but reverse total shoulder arthroplasty (RTSA) has emerged as an increasingly popular treatment option. Although ORIF with angular locking plates is a common treatment for proximal humerus fractures, prior reports suggest high failure and complication rates. Although RTSA has become an increasingly popular option for complex proximal humeral head fractures given its low complication rates, there are concerns it may lead to limited postoperative ROM. Thus, the optimal treatment for
