Spreading brain lesions in a familial Creutzfeldt-Jakob disease with V180I mutation over 4 years.

BACKGROUND: We report a female patient with familial Creutzfeldt-Jakob disease with V180I mutation (fCJD with V180I), who was serially followed up with magnetic resonance imaging (MRI) and electroencephalogram (EEG) for up to four years. CASE PRESENTATION: At 6 months after the onset, diffusion-weighted images (DWI) and fluid-attenuated inversion recovery (FLAIR) of brain MRI revealed an increased signal intensity in the bilateral frontal, temporal, and parietal cerebral cortex with left dominancy except for the occipital lobe. However, her follow-up MRI at four years showed the high-signal r

Abnormal tau deposition in neurons, but not in glial cells in the cerebral tissue surrounding arteriovenous malformation.

We report an autopsy case of arteriovenous malformation (AVM) of the right frontal lobe in a 50-year-old man, in whom post mortem examination revealed massive tau deposition in the affected cerebral cortex. The patient was diagnosed as having AVM at the age of 21 years, and died of unknown cause at the age of 50 years. Immunostaining with anti-phosphorylated tau antibody (AT8) revealed many NFTs and neuropil threads, but not glial tau accumulation, in the right frontal cortex surrounding the AVM. The NFTs and neuropil threads contained both 3-repeat and 4-repeat tau. Ultrastructurally, the NFT…

An atypical case of neuro-Whipple: Clinical presentation, magnetic resonance spectroscopy and follow-up.

We report a case of a 53-year-old man with a 2-year history of progressive gait and balance disturbance, supranuclear ophthalmoparesis, mild dysarthria and dysmetria. EMG revealed a lower limb axonal sensory-motor neuropathy, while MR imaging demonstrated a small focal lesion in the right frontal lobe, mild diffuse hyperintensity of the periventricular white matter and diffuse brain atrophy. Magnetic resonance spectroscopy revealed a mild decrease in N-acetyl-aspartate peak and an increase in the choline peak in the small right frontal lesion and within 6 voxels of interest in normal appearing…

Somatosensory evoked potentials in progressive supranuclear palsy.

We report median and digital nerve somatosensory evoked potentials (SEPs) in 14 patients with probable progressive supranuclear palsy (PSP) … ion such as ideomotor apraxia and cortical sensory loss are uncommon in classical PSP. Furthermore, frontal lobe dementia which is frequently present in PSP patients, is thought to be of subcortical origin resulting from striato-frontal deafferentation. We propose that cortical neurofibrillary pathology may lead to subclinical intracortical disinhibition accou …

Direct and indirect revascularization for moyamoya disease surgical techniques and peri-operative complications.

We have performed surgical treatment for Moyamoya disease using the superficial temporal artery to middle cerebral artery (STA-MCA) anastomo … tal branch of the middle cerebral artery is indispensable for improving cerebral circulation of the frontal lobe. A small arachnoid membrane opening and water-tight closure are also important to avoid post-operative subdural and subcutaneous fluid collection. Intracranial abscesses secondary to nasal, sinus, and orbital infections in adults and children.

Nineteen cases of intracranial abscesses secondary to infection of the midface are reported. The most common underlying cause was bacterial sinusitis. Other etiologic factors included mucormycosis following steroid therapy, Wegener's granulomatosis, nasal dermoid cyst and sinus tract, tooth abscesses, aspergillosis following chemotherapy for leukemia, squamous cell carcinoma of the frontal sinus, infected methylmethacrylate plate for a prior skull fracture, and a case of gauze packing left in the sinus following surgery. Anaerobic organisms were the predominant cause of the abscesses. The most

Brain abscesses in Northern Ireland: a 30 year community review.

One hundred and seventy-two cases of intracranial abscesses, obtained from Neurosurgical and Centralised Autopsy Records for Northern Ireland for the 30 years, 1947--1976, have been reviewed. The incidence of the disease has fallen from five to three per million of population per year over the past three decades. Intracranial abscesses were three times as common in males as in females. Twenty-nine per cent of the abscesses were in the temporal lobe, 25% frontal, 10% parietal, 6% cerebellar, 3% occipital, and 7% were either subdural or in deep sites such as the thalamus; the remainder (20%) wer

Severe Pediatric Open Skull Fracture With Exposed Brain Matter: A Case Report.

Head trauma in the pediatric population carries a high rate of morbidity and mortality. The major causes of head trauma are related to falls, recreational activities, motor vehicle accidents, and gunshot wounds. Traumatic brain injury (TBI) can occur after severe head trauma and is defined as an alteration in brain function, or other evidence of brain pathology, caused by an external force. Intracranial edema and herniation are common consequences of a TBI in pediatric patients and are commonly relieved via decompressive craniectomy.  This case study describes a 13-year-old male presenting to

Why can spontaneous intracranial hypotension cause behavioral changes? A case report and multimodality neuroimaging comparison with frontotemporal dementia.

Frontotemporal Brain Sagging Syndrome (FBSS) is a rare condition characterized by the presence of spontaneous intracranial hypotension associated with behavioural disturbances mimicking the behavioural variant of Frontotemporal dementia (bvFTD). It has been suggested that behavioural symptoms are caused by damage to the connectivity of the frontal lobes due to the brain sagging. However, no studies have directly explored brain connectivity in patients with FBSS. Here, we report a new case of FBSS with persistent behavioural disturbances, whom we compared to 20 patients with bvFTD and to 13 cog…

EEG Spectral Coherence Analysis in Nocturnal Epilepsy.

OBJECTIVE: Electroencephalography (EEG) is widely employed in the study of sleep disorders. This paper exploits the identification of cyclic alternating patterns (CAPs), a periodic ubiquitous phenomenon nested in the sleep stages, to analyze the EEG spectral coherence in subjects affected by nocturnal frontal lobe epilepsy (NFLE) and healthy controls. METHODS: For each EEG recording, we extracted several CAP A1 subtype 4 s time series. We analyze the coherence between each pair of electrodes for each individual to obtain its distribution for each frequency range of interest to investigate dif…

Brain Perfusion in Corticobasal Syndrome with Progressive Aphasia.

BACKGROUND: Brain perfusion may differ between patients with corticobasal syndrome (CBS) with and without aphasia. METHODS: Twenty-six (9 males and 17 females; mean age 76 ± 5.3 years) patients with CBS were enrolled in the study. Brain MRI and single-photon emission computed tomography were performed in all subjects. Language was evaluated using the Standard Language Test of Aphasia. The patients were divided into two subgroups according to the presence or absence of progressive aphasia. Differences in the regional cerebral blood flow (rCBF) between the two groups were detected based on voxe

Executive dysfunction in adults with moyamoya disease is associated with increased diffusion in frontal white matter.

BACKGROUND AND PURPOSE: Alteration of the cerebrovascular reserve (CVR) in the frontal lobes has been associated with cognitive dysfunction in adults with moyamoya disease (MMD). Elevation of the apparent diffusion coefficient (ADC) in normal-appearing white matter on conventional MRI may occur as a consequence of chronic haemodynamic failure. In the present study, the authors examined the relation of ADC with CVR and cognitive dysfunction in adults with MMD. METHODS: The authors measured ADC and CVR in the normal-appearing frontal white matter. CVR was calculated using dynamic susceptibility…

The main defective manifestations of extra-personal unilateral spatial neglect concern the side of space contralateral to the hemispheric le … erative responses were found to be much more frequent in patients with spatial neglect and when the frontal lobe or subcortical structures were damaged. The results are interpreted in the context of a multi-componential view of the neglect syndrome, with reference to a distinction originally put forward by Derek Denny-Brown (1958) between 'frontal or magnetic' …

Synergistic effects of cerebrovascular reactivity and gray matter volume on executive function in aging.

BackgroundAge-related impairments in cerebrovascular reactivity (CVR) and gray matter volume (GMV) both contribute to cognitive decline, yet their regional interplay and joint impact on cognition remain unclear.ObjectiveTo investigate how CVR and GMV are coupled and interact regionally to influence cognition in normal aging.MethodsWe examined 181 healthy younger adults (18-34 years) and 120 older adults (60-89 years). CVR and GMV (% intracranial volume) in major brain lobes were quantified using resting-state multi-echo functional MRI and structural MRI, respectively. Age-related alterations w

Fronto-parietal cortex activation during walking in patients with Parkinson's disease adopting different postural strategies.

BACKGROUND: Cortical activation patterns in patients with Parkinson's disease (PD) may be influenced by postural strategies, but the underlying neural mechanisms remain unclear. Our aim is to examine the role of the fronto-parietal lobes in patients with PD adopting different postural strategies and the effect of dual task (DT) on fronto-parietal activation. METHODS: Two groups of patients with PD adopting either the posture first strategy (PD-PF) or the posture second strategy (PD-PS) were examined respectively when in the "OFF" state while single-walking task (SW) and DT.
