Cerebral Infarction During Laparoscopic Ovarian Cystectomy in a Neonate: A Case Report.

INTRODUCTION: Cerebral infarction following abdominal surgical procedures is a rare but serious complication in children. We report a rare case of a full-term neonate who developed a cerebral infarction after laparoscopic ovarian cystectomy. CASE: A neonate weighing 2350 g was prenatally diagnosed with a 30 mm complex ovarian cyst that was suspected to be torsional. At 3 days of age, she underwent laparoscopic ovarian cystectomy under general anesthesia. During the procedure, the twisted ovarian cyst, located in the upper abdomen, was excised after fluid drainage. The surgery lasted 64 min, w

Impact of Penicillin Allergy on Antibiotic Prophylaxis Selection and Surgical Site Infection Rates in Plastic Surgery: A Systematic Review.

Background: Patients undergoing plastic and reconstructive procedures with penicillin allergies (PAs) often receive second-line antibiotic agents. Evidence from other surgical specialties suggests this can lead to increased complications and greater morbidity. However, limited research exists in plastic surgery regarding this issue. This systematic review consolidates existing literature on the impact of PAs and alternative antibiotic agent use in plastic surgery, highlighting findings and future research directions. Methods: A systematic search of SCOPUS, PubMed (including MEDLINE), EMBASE, a

[The use of botulinum toxin in hernia surgery: results of a survey in certified hernia centers].

BACKGROUND: Botulinum toxin application into the abdominal wall prior to major hernia repair can reduce the complexity of surgery and has been increasingly used in recent years, even if it is an off-label use. OBJECTIVE: To what extent is botulinum toxin used in hernia surgery in German-speaking countries and what is the current evidence in the literature? MATERIAL AND METHODS: In a voluntary online survey of German Society for General and Visceral Surgery (DGAV)-certified competence centers and reference centers for hernia surgery, aspects of botulinum toxin application were surveyed and th

Intraoperative local instillation anesthesia using injection technique from J-VACTM drain for postoperative pain relief in male-type chest wall contouring surgery.

For postoperative acute pain during mastectomy, a few studies have reported the usefulness of an intraoperative local anesthesia instillation technique in which analgesics are injected through a drain placed under the skin, intraoperatively. This study presented a novel administration method, and the efficacy of local instillation anesthesia in male-type chest wall contouring surgery was assessed. Fifty-four patients underwent chest wall contouring surgery under general anesthesia. The 27 patients in each of the study and control groups, with our instillation technique and without the techniqu

Gene-Specific DNA Methylation Linked to Postoperative Cognitive Dysfunction in Apolipoprotein E3 and E4 Mice.

BACKGROUND: Geriatric surgical patients are at higher risk of developing postoperative neurocognitive disorders (NCD) than younger patients. The specific mechanisms underlying postoperative NCD remain unknown, but they have been linked to genetic risk factors, such as the presence of APOE4, compared to APOE3, and epigenetic modifications caused by exposure to anesthesia and surgery. OBJECTIVE: To test the hypothesis that compared to E3 mice, E4 mice exhibit a more pronounced postoperative cognitive impairment associated with differential DNA methylation in brain regions linked to learning and

A comparison of the ultrasound-guided modified-thoracolumbar interfascial plane block and wound infiltration for postoperative pain management in lumbar spinal surgery patients.

OBJECTIVES: Pain management is an important issue following lumbar spinal surgery. Wound infiltration is a technique that a local anesthetic solution is infiltrated into the tissues around the surgical area. Previous studies reported that US-guided modified thoracolumbar interfacial plane (mTLIP) block after lumbar spinal surgery provided effective analgesia. In this study, we aimed to compare the analgesic efficacy of the US-guided mTLIP block and wound infiltration following lumbar disc surgery. METHODS: 60 patients aged 18-65 years, ASA classification I-II, and scheduled for lumbar disc su

Internal Maxillary Artery Preoperative Embolization Using n-Butyl Cyanoacrylate and Pushable Coils for Temporomandibular Joint Ankylosis Surgery.

BACKGROUND: Temporomandibular joint (TMJ) ankylosis causes disability through impaired digestion, mastication, speech, and appearance. Surgical treatment increases range of motion with resultant functional improvement. However, substantial perioperative blood loss can occur (up to 3 L) if the internal maxillary artery (IMAX) is injured as it traverses the ankylotic mass. Achieving hemostasis is difficult because of limited proximal IMAX access and poor visualization. Our aim is to investigate the technical feasibility and preliminary safety of preoperative IMAX embolization in patients undergo

Rationale and design of the PeriOperative ISchemic Evaluation-2 (POISE-2) trial: an international 2 × 2 factorial randomized controlled trial of acetyl-salicylic acid vs. placebo and clonidine vs. placebo in patients undergoing noncardiac surgery.

BACKGROUND: Worldwide, 200 million adults undergo major noncardiac surgery annually, and 10 million of these patients will have a major vascular complication. Low-dose clonidine and low-dose acetyl-salicylic acid (ASA) may prevent major perioperative vascular complications. We therefore initiated the POISE-2 trial to establish the perioperative effects of these 2 interventions. METHODS: The POISE-2 trial is a 2 × 2 factorial randomized controlled trial of low-dose ASA vs. placebo and low-dose clonidine vs. placebo in 10,000 patients at risk for a perioperative cardiovascular event who are und
