Normalisation of intact parathyroid hormone serum level confirms sufficient resection of parathyroid tissue after total parathyroidectomy in patients with secondary hyperparathyroidism. We describe a 13 year-old Ethiopian girl with vitamin D deficiency rickets. Hypercalcemia, increased serum alkaline phosphatase and PTH levels, together with low serum levels of 25-hydroxyvitamin D and 24,25-dihydroxyvitamin D suggested the co-existence of primary hyperparathyroidism. The surgical removal of a parathyroid adenoma led to bone healing and normalization of blood chemistry. We conclude that vitamin D deficiency masked the hyperparathyroidism and hypercalcemia, while excess PTH secretion delayed the cure of rickets until successful parathyroidectomy had been carried out.

The consequences of bone disease in primary hyperparathyroidism (PHPT) are still a matter of discussion. We studied transiliac bone biopsies of 24 patients (9 men and 15 women) with mild to moderate PHPT, proven by surgery. Histomorphometric data were compared with control values obtained from autopsy bone samples. We report bone biopsy findings in two hemodialysis patients with a history of secondary hyperparathyroidism treated by parathyroidectomy. Discrete, punched-out lesions in trabecular bone, which show little evidence of resorption or osteoblast activity, probably correspond to previous zones of osteoclastic tunneling resorption. Because we have seen these lesions in no other clinical setting, we suggest that they are residua of healed secondary hyperparathyroidism in dialysis patients. Their persistence may be due in part to aluminum intoxication.

PURPOSE: Risk of vertebral fractures is increased in primary hyperparathyroidism (PHPT). Bone qualities affected by PHPT are not necessarily reflected in bone mineral density (BMD), requiring imaging modalities with capability to capture bone microstructure and geometry.
