Nosocomial infections in a pediatric intensive care unit.

In a prospective 30-month study of nosocomial infections in a pediatric ICU (PICU), the incidence, sites, and causes of infection were determined. Factors associated with increased risk of infection were investigated. In 1,388 patients who remained in the PICU for a minimum of 72 h, 116 infections occurred (6.1 infections/100 admissions). Primary bacteremias comprised 38% of PICU infections and lower respiratory infections comprised 15%. The remaining infections were divided equally among GI, skin, eye, upper respiratory, postoperative wounds, and other sites. Coagulase-negative staphylococci,…

The tender tooth. Dentoalveolar, pericoronal, and periodontal infections.

Dental caries and periodontal disease are the most common afflictions of the tooth. These infectious diseases cause considerable pain and discomfort and ultimately loss of the tooth. Apart from local effects, these infections may extend beyond natural barriers and result in complications that can vary in severity from the excruciating pain of acute pulpitis to life-threatening infections of the deep fascial spaces of the head and neck. In this article, the clinical and pathologic features of dental caries, pulpitis, periapical abscess, pericoronitis, and periodontal infections are discussed as…

How do African game animals control trypanosome infections?

African trypanosomiasis is endemic over much of sub-saharan Africa. But whereas domestic animals - especially cattle - often succumb to the infection, wild mammals generally show a high degree o f resistance. Many species o f wildlife living in tsetse-infested areas carry trypanosome infections, and so act as important reservoir hosts, but generally show no obvious ill-effects. How they survive the infection is an important question in understanding mechanisms o f trypanotolerance, yet relevant data are sparse and scattered. Here, Ayub Mulla and Roy Rickman review some of the previous studies …

Immunopathophysiology of Babesia bovis and Plasmodium falciparum infections.

Babesia bovis and Plasmodium falciparum are both vector-borne parasites primarily infecting the erythrocytes of their respective hosts. They have obvious differences, yet the diseases caused by these parasites share many common features. Both have generated a considerable body of research but, perhaps because of the classical distinction between veterinary and medical parasitology, many of the similarities between the two have been neglected. As this review shows however, many of the pathophysiological changes in B. bovis infections are poorly described for P. falciparum - and vice versa. Exam…

Sero-diagnosis of Fasciola hepatica infections in cattle.

An antigen was prepared from metabolic products which were produced by maintaining Fasciola hepatica adults in RPMI tissue culture medium for 24 h. The antigen compared favourably with a soluble extract of adult flukes when used in an enzyme-linked immunosorbent assay (ELISA). Experimentally infected calves were used to evaluate the sensitivity of the test and a survey from areas of both traditionally high and low incidence of the disease was carried out. Evidence is presented which suggests that this metabolic antigen could be used for the sero diagnosis of naturally occurring infections. In …

Immunopathogenic mechanisms in human immunodeficiency virus (HIV) infections.

Infection with HIV can result in a complex array of immunopathogenic effects. HIV infection involves both a direct quantitative depletion of T4 lymphocytes as well as an indirect qualitative effect on the function of several types of immune effector cells. The combination of T4-cell destruction and functional abnormalities contributes to the broad scope of immunologic aberrations and opportunistic diseases seen in HIV-infected individuals. In addition, HIV infection of monocyte/macrophages may play an important role as a reservoir or sanctuary of infection in the host and contribute to the cha

Systemic fungal infections: diagnosis and treatment. I. Sporotrichosis.

Sporotrichosis is a fungal infection caused by the dimorphic fungus Sporothrix schenckii. Although this fungal microorganism is labeled a true pathogen, its virulence is less than that of other fungal pathogens, such as Coccidioides immitis, Histoplasma capsulatum, and Blastomyces dermatitidis. This article discusses its clinical manifestations, experimental models, diagnosis, and therapy.

Systemic fungal infections: diagnosis and treatment. I. Paracoccidioidomycosis.

Paracoccidioidomycosis is the designation for the disease caused by the dimorphic fungus Paracoccidioides brasiliensis. It is a disease of great importance in Latin America and constitutes an important diagnosis for consideration in patients from endemic areas who have pulmonary or systemic disease compatible with fungal infection. In the laboratory, basic work has uncovered important interactions between the fungus and the human sex steroid system. These studies may have important implications in our approach to this and other infectious diseases.

Systemic fungal infections: diagnosis and treatment. I. Coccidioidomycosis.

Coccidioidomycosis is a highly variable disease. Initial respiratory tract infection can lead to self-limited pneumonia, pulmonary complications, and extrapulmonary disease. The early infection requires no therapy, except in immunosuppressed patients and other selected patients. Treatment for pulmonary complications may include surgery for cavities or pyopneumothorax (resulting from rupture of a cavity) and antifungal therapy for chronic pneumonia. The majority of extrapulmonary disease occurs in the skin, bones and joints, or meninges and is an indication for treatment with antifungal agents

Anaerobic infections in the head and neck region.

Anaerobic bacteria form the predominant flora of the oral cavity, outnumbering facultative organisms by 10-1,000: 1. The type of anaerobic bacteria and their concentration depend on the anatomical site and the degree of anaerobiosis in the different sites in the mouth. Three groups of anaerobic bacteria inhabit the oral cavity; the strict anaerobes, the moderate anaerobes, and the microaerophilic group of organisms. The majority of anaerobic bacterial infections occurring in the region of the mouth, head and neck are caused by the commensal flora. These infections include dental and periodonta…

Viral infections in pediatric bone marrow transplant patients.

The records of 96 pediatric patients with aplastic anemia or a malignancy who underwent bone marrow transplantation between 1979 and 1986 at The Children's Hospital of Philadelphia were reviewed for laboratory evidence of viral infections. The most common viral diseases identified were herpes simplex virus (HSV), cytomegalovirus and adenoviruses, which were found in 19 (20%), 17 (18) and 17 (18) patients, respectively. HSV was more common in patients with than without graft vs. host disease (GVHD) (9 of 30; 30% vs. 10 of 66; 15%), but the difference did not reach statistical significance. Late…

Treatment of nontuberculous mycobacterial infections in pediatric patients.

Two cases of Mycobacterium avium-intracellulare complex (MAC) infections are described, and the diagnosis, clinical features, and management of MAC infections are reviewed. In case 1, a four-year-old boy was diagnosed as having both acquired immunodeficiency syndrome (AIDS) and disseminated MAC infection. He was treated with a combination of isoniazid, ethambutol hydrochloride, rifabutin, and clofazimine. Results of susceptibility testing showed that the MAC was susceptible to rifabutin and ethambutol with intermediate susceptibility to isoniazid. The child developed severe adverse effects tha…

The neuromuscular manifestations of human immunodeficiency virus infections.

We studied 14 patients with neuromuscular disorders and concomitant infection with human immunodeficiency virus to define clinical syndromes and prognosis. Eight patients had painful sensorimotor peripheral neuropathy; two, chronic inflammatory demyelinating polyneuropathy; two, mononeuropathy or mononeuropathy multiplex; one, recurrent myoglobinuria; and one, chronic proximal weakness and elevated creatine kinase levels. All eight patients with painful neuropathy had overt symptoms of acquired immunodeficiency syndrome. Chronic inflammatory demyelinating polyneuropathy was the first manifesta

Treatment of penetrating abdominal trauma and gynecologic infections.

Both penetrating abdominal trauma and gynecologic infections are polymicrobial and require antibiotics with broad-spectrum activity against both aerobic and anaerobic pathogens. In an open study, ticarcillin disodium/clavulanate potassium was used to treat patients with penetrating abdominal trauma, intraabdominal infection or gynecologic infection. Ticarcillin disodium/clavulanate potassium was administered at a dose of 3.1 g every four or every six hours, and therapy lasted for a minimum of three days. Among 20 patients with intraabdominal infections that involved abdominal organ perforation…

Infections caused by dematiaceous fungi: chromoblastomycosis and phaeohyphomycosis.

Because of the increased incidence of infections caused by dematiaceous fungi, it is important to clearly define the conceptual basis for the clinical nomenclature for infections caused by these fungi. When considering dematiaceous fungal infections, there is a continuum of diseases that can be separated into the three categories of chromoblastomycosis, phaeohyphomycosis, and mycetoma. The diseases chromoblastomycosis and phaeohyphomycosis are compared and contrasted in this chapter.

Vaccines for prevention of head and neck infections.

Many current vaccines are directed against pathogens that infect the upper airway on their way to causing greater damage elsewhere in the body. This article focuses on vaccines against pathogens that contribute importantly to infections of the head and neck per se. The discussion includes vaccines for diphteria, mumps, measles, HIB infections, and infections caused by Streptococcus pneumoniae.

Experimental Cryptosporidium parvum infections in opossums (Didelphis virginiana).

Five nursing opossums (Didelphis virginiana) were each inoculated with 5 x 10(6) Cryptosporidium parvum oocysts of calf origin. Following inoculation, endogenous stages of C. parvum were observed in the ileum, cecum, and colon of these opossums. Two of three noninoculated pouch mates acquired infections during the study based on examinations of feces and tissue sections of all eight opossums. Mild diarrhea was observed in four of seven opossums harboring C. parvum, although none died as a result of the infection. Under the conditions of this study, C. parvum appeared to be only mildly pathogen…
