We introduce a novel approach for robust estimation of physiological parameters such as interbeat interval (IBI) and heart rate variability (HRV) from cardiac signals captured with wearable sensors in the presence of motion artifacts. Motion artifact due to physical exercise is known as a major source of noise that contributes to a significant decline in the performance of IBI and HRV estimation techniques for cardiac monitoring in free-living environments. Therefore, developing robust estimation algorithms is essential for utilization of wearable sensors in daily life situations. The proposed

Hereditary spastic paraplegias (HSP) represents a group of orphan neurodegenerative diseases with gait disturbance as the predominant clinical feature. Due to its rarity, research within this field is still limited. Aside from clinical analysis using established scales, gait analysis has been employed to enhance the understanding of the mechanisms behind the disease. However, state of the art gait analysis systems are often large, immobile and expensive. To overcome these limitations, this paper presents the first clinically relevant mobile gait analysis system for HSP patients. We propose an

BACKGROUND: With the character of high incidence, high prevalence and high mortality, stroke has brought a heavy burden to families and society in China. In 2009, the Ministry of Health of China launched the China national stroke screening and intervention program, which screens stroke and its risk factors and conducts high-risk population interventions for people aged above 40 years old all over China. In this program, stroke risk factors include hypertension, diabetes, dyslipidemia, smoking, lack of exercise, apparently overweight and family history of stroke. People with more than two risk

BACKGROUND: According to WHO stroke is a growing societal challenge and the third leading cause of global disease-burden estimated using disability-adjusted life years. Rehabilitation after stroke is an area of mutual interest for health care in many countries. Within the health care sector there is a growing emphasis on ICT services to provide clients with easier access to information, self-evaluation, and self-management. ICT-supported care programs possible to use in clients' home environments are also recommended when there are long distances to the health care specialists. The aim of this

BackgroundExtracting useful information from biomedical literature plays an important role in the development of modern medicine. In natural language processing, there have been rigorous attempts to find meaningful relationships between entities automatically by co-occurrence-based methods. It has been increasingly important to understand whether relationships exist, and if so how strong, between any two entities extracted from a large number of texts. One of the defining methods is to measure semantic similarity and relatedness between two entities.MethodsWe propose a hybrid ranking method th

BACKGROUND: Cloud based health platforms (CBHP) have tremendous capacity to meet patient's health needs. The benefits inherent in CBHP position it to be relevant for efficient healthcare delivery. Nonetheless, studies have shown that the adoption of new technologies is sometimes a challenge especially in developing nations. This study, therefore, aim to examine, identify and evaluate the factors affecting healthcare professionals' intention to accept the cloud-based health center (CBHC) in developing countries. The research study focuses on hospitals in North-central of Nigeria.

BACKGROUND: Reinforcement learning (RL) provides a promising technique to solve complex sequential decision making problems in health care domains. To ensure such applications, an explicit reward function encoding domain knowledge should be specified beforehand to indicate the goal of tasks. However, there is usually no explicit information regarding the reward function in medical records. It is then necessary to consider an approach whereby the reward function can be learned from a set of presumably optimal treatment trajectories using retrospective real medical data. This paper applies inver

BACKGROUND: Family history information (FHI) described in unstructured electronic health records (EHRs) is a valuable information source for patient care and scientific researches. Since FHI is usually described in the format of free text, the entire process of FHI extraction consists of various steps including section segmentation, family member and clinical observation extraction, and relation discovery between the extracted members and their observations. The extraction step involves the recognition of FHI concepts along with their properties such as the family side attribute of the family

BACKGROUND: Maintaining physical fitness is a crucial component of the therapeutic process for patients with cardiovascular disease (CVD). Despite the known importance of being physically active, patient adherence to exercise, both in daily life and during cardiac rehabilitation (CR), is low. Patient adherence is frequently composed of numerous determinants associated with different patient aspects (e.g., psychological, clinical, etc.). Understanding the influence of such determinants is a central component of developing personalized interventions to improve or maintain patient adherence. Medi

BACKGROUND: The incidence of cardiac implantable electronic device infection (CIEDI) is low and usually belongs to the typical imbalanced dataset. We sought to describe our experience on the management of the imbalanced CIEDI dataset.
METHODS: Database from two centers of patients undergoing device implantation from 2001 to 2016 were reviewed retrospectively. Re-sampling technique was used to improve the classifier accuracy.
RESULTS: CIEDI was identified in 28 out of 4959 procedures (0.56%); a high imbalance existed in the sizes of the patient profiles. In univariate analyses, replacement pr

This paper presents a novel real-time smart system performing fall detection, location, and notification based on floor vibration data produced by fall downs. Only using floor vibration as the recognition source, the system incorporates a person identification through vibration produced by footsteps to inform who is the fallen person. Our approach operates in a real-time style, which means the system recognizes a fall immediately and can identify a person with only one or two footsteps. A collaborative in-network location method is used in which sensors collaborate with each other to recognize

BACKGROUND: There is growing interest in sensor-based assessment of upper limb tremor in multiple sclerosis and other movement disorders. However, previously such assessments have not been found to offer any improvement over conventional clinical observation in identifying clinically relevant changes in an individual's tremor symptoms, due to poor test-retest repeatability.
METHOD: We hypothesised that this barrier could be overcome by constructing a tremor change metric that is customised to each individual's tremor characteristics, such that random variability can be distinguished from clin

OBJECTIVE: Cardiovascular Implantable Electronic Devices (CIEDs) are used extensively for treating life-threatening conditions such as bradycardia, atrioventricular block and heart failure. The complicated heterogeneous physical dynamics of patients provide distinct challenges to device development and validation. We address this problem by proposing a device testing framework within the in-silico closed-loop context of patient physiology.
METHODS: We develop an automated framework to validate CIEDs in closed-loop with a high-level physiologically based computational heart model. The framewor

BACKGROUND: Time of death estimation in humans for the benefit of forensic medicine has been successfully approached by Henssge, who modelled body cooling based on measurements of Marshall and Hoare. Thereby, body and ambient temperatures are measured at the death scene to estimate a time of death based on a number of assumptions, such as initial body temperature and stable ambient temperature. While so far, practical use of the method resorted to paper print outs or copies of a nomogram using a ruler, increasingly, users are interested in computer or mobile device applications. We developed a

BACKGROUND: In low-income settings, community health workers (CHWs) are frequently the first point of contact for newborns. Mobile technology may aid health workers in classifying illness and providing referral and management guidance for newborn care. This study evaluates the potential for mobile health technology to improve diagnosis and case management of newborns in Bangladesh.
METHODS: A mobile application based on Bangladesh's Comprehensive Newborn Care Package national guidelines (mCNCP) was developed to aid CHWs in identifying and managing small and sick infants. After a 2-day trainin

The current measuring metric and reporting methods for assessing maternal mortality are seriously flawed. Evidence-based prevention strategies require consistently reported surveillance data and validated measurement metrics. Main Body: The denominator of live births used in the maternal mortality ratio reinforces the mistaken notion that all maternal deaths are consequent to a live birth and, at the same time, inappropriately inflates the value of the ratio for subpopulations of women with the highest percentage of pregnancies ending in outcomes other than a live birth. Inadequate methods for

OBJECTIVE: Birth asphyxia is a major newborn mortality problem in low-resource countries. International guideline provides treatment recommendations; however, the importance and effect of the different treatments are not fully explored. The available data are collected in Tanzania, during newborn resuscitation, for analysis of the resuscitation activities and the response of the newborn. An important step in the analysis is to create activity timelines of the episodes, where activities include ventilation, suction, stimulation, etc. Methods: The available recordings are noisy real-world videos

BACKGROUND: Fetal heart rate (FHR) monitoring is a screening tool used by obstetricians to evaluate the fetal state. Because of the complexity and non-linearity, a visual interpretation of FHR signals using common guidelines usually results in significant subjective inter-observer and intra-observer variability.
OBJECTIVE: Therefore, computer aided diagnosis (CAD) systems based on advanced artificial intelligence (AI) technology have recently been developed to assist obstetricians in making objective medical decisions.
METHODS: In this work, we present an 8-layer deep convolutional neural ne

OBJECTIVE: This paper proposes a novel framework for the segmentation of phonocardiogram (PCG) signals into heart states, exploiting the temporal evolution of the PCG as well as considering the salient information that it provides for the detection of the heart state.
METHODS: We propose the use of recurrent neural networks and exploit recent advancements in attention based learning to segment the PCG signal. This allows the network to identify the most salient aspects of the signal and disregard uninformative information.
RESULTS: The proposed method attains state-of-the-art performance on
