De-escalation of inhaled corticosteroids and other maintenance asthma therapies in dupilumab-treated patients with severe asthma: a real-world study.

BACKGROUND: Biologic therapies for severe T2-high asthma demonstrated oral corticosteroid-sparing effects, but evidence on the harmlessness of de-escalating inhaled corticosteroids and other in patients controlled with dupilumab remains limited. OBJECTIVE: To evaluate, in a real-world setting, the clinical impact of de-escalating high-dose ICS, LAMA and LTRA in patients with severe asthma controlled with dupilumab. METHODS: We conducted a retrospective bicentric observational study including 54 patients with severe asthma who initiated dupilumab between January 2022 and December 2023. Change

Efficient pulmonary-targeted therapy and mechanistic insights of a novel glutathione dry powder inhaler in a model of radiation-induced lung injury.

Radiation-induced lung injury (RILI) remains a serious complication of thoracic radiotherapy, with limited treatment options. While reduced glutathione (GSH) has therapeutic potential, its efficacy is hindered by poor pulmonary bioavailability via systemic administration. This study developed a novel glutathione dry powder inhaler (GSH-DPI) through rational formulation optimization. The optimized GSH-DPI exhibited excellent aerosol performance, with a fine particle fraction of 84.48% and sustained release in simulated lung fluid. In a murine RILI model, GSH-DPI (50 mg/kg, intrapulmonary admini

Inhalational Agents and Novel Ultra-short-acting Sedatives in Modern Dental Anesthesia: From Pharmacologic Mechanisms to Advanced Hybrid Protocols.

Modern dental sedation is transitioning from traditional long-acting agents toward fast-acting pharmacology characterized by organ-independent metabolism and rapid offset. In this article, we review the pharmacodynamics of inhalational agents (nitrous oxide, sevoflurane) and comprehensively evaluate novel intravenous agents: remimazolam, remifentanil, and fospropofol. Many practitioners are looking beyond traditional propofol, prompted by its narrow safety margins and the intensive airway training required to use it responsibly in a dental setting. We analyze mechanism-specific advantages, dos…

Size-Dependent Pulmonary Toxicity and Whole-Body Distribution of Inhaled Micro/Nanoplastic Particles in Male Mice from Chronic Exposure.

The ubiquitous presence of micro/nanoplastics (MP/NP) in the atmosphere has raised significant concerns about their potential health risks through inhalation, yet the effects of natural respiratory exposure remain underexplored. This study addresses this critical knowledge void by utilizing a whole-body inhalation exposure system to investigate the distribution, accumulation, and pulmonary toxicity of polystyrene MP/NP (1.5 × 105 particles/m3) in male ICR mice (n = 16/group). Fluorescently labeled MP/NP revealed the highest particle accumulation in the lungs, followed by the bloodstream and sp…

Reduced treatment response to inhaled corticosteroids in current smokers with COPD, regardless of blood eosinophil count: insights from the FLAME trial.

Inhaled corticosteroids (ICSs) benefit patients with chronic obstructive pulmonary disease at high risk of exacerbations with raised blood eosinophil count (BEC). Emerging evidence suggests current smokers show a reduced response to ICS. This post-hoc analysis of the FLAME trial explored the impact of smoking status on the efficacy of long-acting beta-2 agonist (LABA)+ICS versus LABA+long-acting muscarinic antagonist (LAMA) for preventing exacerbations. Our findings indicate that LABA+LAMA is superior to LABA+ICS in preventing moderate to severe exacerbations in current smokers and inferior in

The Failure of an Auto-Substitution Protocol of Short-Acting Nebulizers for Long-Acting Inhalers to Reduce Cost of Care in a Quaternary Teaching Hospital.

Background: COPD is a common diagnosis driving in-patient admissions. A major component of in-patient COPD management involves nebulized or inhaled bronchodilators. In many hospitals, the in-patient pharmacy limits medication availability and may auto-substitute short-acting nebulized bronchodilators (SANBs) for long-acting inhalers (LAIs) to cut costs; the effect of such policy change on patient care is unknown. Methods: We performed a retrospective, observational study at a quaternary teaching hospital to analyze respiratory therapy utilization in subjects with COPD on home LAI, who were adm

Machine learning-based prediction of aerodynamic performance in arformoterol-lactose dry powder inhaler formulations using surface roughness features.

Dry powder inhalers (DPIs) are widely used for pulmonary drug delivery, and their aerodynamic performance is highly dependent on particle surface morphology. This paper presents a machine learning-based framework to quantitatively predict DPI aerodynamic behavior using surface roughness parameters from scanning electron microscopy (SEM) images. Ten DPI formulations were prepared with various storage conditions and compositions. Thirteen surface roughness features were extracted using ImageJ and SurfCharJ and used as inputs for predictive models, including multilayer perceptrons (MLPs), support

The effect of different inhaled corticosteroid and long-acting bronchodilator combinations on the airway microbiome in patients with severe COPD: a randomised trial (MUSIC).

BACKGROUND: The microbiome is associated with exacerbation risk, quality of life and mortality in COPD. Inhaled corticosteroid (ICS) treatment has been reported to alter the microbiome through modulating host defence. How ICS alters the microbiome and whether effects are equal between different ICS preparations is debated. The aim of the MUSIC trial was to investigate whether commonly used ICS therapies have different effects on the airway microbiome in COPD. METHODS: This was a multicentre randomised controlled trial. After a 4-week washout period during which they withdrew from ICS, patient

Budesonide/glycopyrronium/formoterol fumarate co-suspension metered dose inhaler relieves cough after lobectomy: a randomized controlled study.

BACKGROUND: Cough after pulmonary resection (CAP) has emerged as a prevalent complication. However, existing treatment protocols for CAP lack standardization. The components of the budesonide/glycopyrronium/formoterol fumarate co-suspension metered dose inhaler (BGF MDI) have been consistently documented for their effectiveness in cough management. Consequently, we initiated this clinical trial to evaluate the efficacy and safety of BGF MDI in mitigating CAP. METHODS: Enrolled participants exhibited no pre-existing cough before undergoing lobectomy. The patients were randomly assigned in a 1:
