Assessment of anthropometric, systemic, and lifestyle factors influencing bone status in the legs of spinal cord injured individuals.

The aim of the present study was to assess the influence of muscle spasms, systemic or lifestyle factors on bone mass and geometry of the femur and the tibia in people with long-standing spinal cord injury (SCI). Fifty-four motor complete SCI people with paralysis duration of between 5 and 50 years were included in the study. Spasticity was measured by means of the Ashworth scale. Distal epiphyses and mid shafts of the femur, tibia, and radius were measured by peripheral quantitative computed tomography. From the epiphyseal scans, trabecular and total bone mineral density (BMDtrab and BMDtot)

Estimated moments at L5/S1 level and muscular activation of back extensors for six prone back extension exercises in healthy individuals.

The purpose of this study was to identify the prone back extension (PBE) exercises that offer the most resistance to the back extensor muscles. Twenty males with no previous history of low back injury performed two repetitions of eight exercises. These consisted of two maximal isometric voluntary activations (MVA) and six different PBE exercises. The participants, while lying prone on a bench, were asked to raise either their trunk or legs to the horizontal position and hold for one second (static phase) before returning to the original position. The exercises differed according to the particu

[Further experiences with transplantation of the head of the fibula].

We are presenting our experience with vascularized epiphyseal transplantation of the upper end of the fibula, based distally on the anterior tibial artery in two patients, one suffering from septic epiphysitis of the hip with complete loss of the head and neck of the femur, the other suffering from radial club hand following septic loss of the radius, excluding its proximal and distal articular segments. In the first patient, the fibular epiphysis bridged the bone defect and provided growth at the neck of the femur. This restored hip stability, reduced limb-length discrepancy, initiated some d

Pathology of acute and chronic ischaemic neuropathy in atherosclerotic peripheral vascular disease.

The peripheral nerve pathology in ischaemic limbs with atherosclerotic peripheral vascular diseases (PVD) is difficult to ascertain because of the limited number of reports. In addition, it has been debated whether chronic ischaemia per se could cause morphological abnormalities in peripheral nerves. In this prospective study, we examined pathological findings in the sural, saphenous, deep peroneal, superficial peroneal and tibial nerves, taken from seven acutely and nine chronically ischaemic amputated legs in which ischaemia was due to non-diabetic severe PVD. For morphological comparison, n

Thermal effects on skeletal muscle tensile behavior.

The effect of temperature on the mechanical failure properties of rabbit skeletal muscle (tibialis anterior and extensor digitorum longus) was examined. For all tests, one leg was maintained at 25 degrees C and the contralateral leg at 40 degrees C. Muscles were pulled to failure according to assignment into one of three groups: 1) passive failure at 10 cm/sec, 2) passive failure at 1 cm/sec, or 3) active (muscle is stimulated to contract as it is pulled) failure at 10 cm/sec. Load to failure was higher in the cold muscle for all groups tested. Total deformation was the same except in Group 1,

Biomechanics of Rugby Tackle Techniques in Youth and Adult Players: A Systematic Review of Age-Specific Injury Risk and Performance Implications.

BACKGROUND: Tackling is a vital skill in rugby that greatly influences both performance outcomes, such as carry distance and possession gain, and injury risk, such as the frequency of concussions and cervical injuries. Approximately 72% of head impacts in rugby occur during tackles, leading to concussions and cervical trauma. Technical skill in tackle mechanics is therefore central to player safety. OBJECTIVES: This systematic review evaluates tackle biomechanics across age groups to identify differences in technique and injury patterns and to inform age-specific coaching strategies. METHODS

Battlefield exoskeleton usability for lower-limb trauma in prolonged field care: A mixed-methods approach.

Prolonged field care is increasingly important in military operations, where austere environments and delayed medical evacuations necessitate extended injury management. Tibia and fibula fractures are common, survivable battlefield injuries that demand such care. Current treatment options involve simple splints, which require the injured Service member to be carried on a litter, and exoskeletons, which are not suited for use in combat environments. The Intrepid Battlefield Exoskeleton (IBEX) was developed to address this critical gap in battlefield trauma care by stabilizing lower leg fracture

Efficacy of Z-shaped supine position in robot-assisted radical prostatectomy: study protocol for a randomized controlled trial.

BACKGROUND: Prostate cancer is the second most common cancer among men worldwide and is frequently managed with robot-assisted radical prostatectomy (RARP). Standard patient positioning during RARP, specifically the lithotomy and steep Trendelenburg positions with a head-down angle of 25°-45°, can lead to complications such as peripheral nerve injury, elevated intraocular pressure (IOP), dizziness, nausea, and vomiting. This study introduces an alternative "Z-shaped supine position," aimed at reducing postoperative position-related complications and improving patient comfort. METHODS: This si

An update on biofilms in acute and chronic wounds: Incidence, clinical evidence, diagnosis, prevention, and treatment.

Pathogenic biofilms are a critical barrier to wound healing, driving infection persistence, delayed tissue repair, and antimicrobial resistance (AMR) and tolerance. This recognition has driven a paradigm shift from a traditional planktonic model of infection to a biofilm-mediated framework, where structured microbial communities are encased within a protective extracellular polymeric substance (EPS) matrix. These communities are highly prevalent in chronic wounds, with reported detection rates of 60%-100% across diabetic foot ulcers, venous leg ulcers, and pressure injuries. The establishment

Foot Flexibility and Alignment Are Weakly Correlated with Multisegment Foot Kinematics During the Landing Task.

BACKGROUND: Flat feet show increased foot motion during dynamic tasks. Although the increased motion is one of the risk factors for sports-related injuries, few studies have investigated how foot flexibility affects foot kinematics during landing. This study aimed to investigate the relationship between foot alignment, foot flexibility, and multisegment foot kinematics during a double-leg landing task in healthy participants. METHODS: Thirty-eight healthy volunteers (50% female) participated in this study. Their mean ± SD age was 22.0 ± 1.3 years; height, 166.1 ± 8.9 cm; and mass, 57.5 ± 8.8

A comparison of stiffness of six knee braces with application for posterolateral corner reconstructions.

Posterolateral corner knee injuries are clinically significant, and often require surgical reconstruction. The optimal knee brace following posterolateral corner reconstructions has not yet been determined via clinical nor biomechanical study. We sought to evaluate the stiffness of six types of knee braces to determine the ideal brace type for reducing varus forces, which may have clinical utility for posterolateral corner knee reconstruction rehabilitation. Six different groups of knee braces underwent mechanical testing: cruciate braces, cruciate braces with a valgus bend, medial unloaders,

Eccentric exercise 24 h prior to hypobaric decompression increases decompression strain.

PURPOSE: Animal studies have shown that recent musculoskeletal injuries increase the risk of decompression sickness (DCS). However, to date no similar experimental study has been performed in humans. The aim was to investigate if exercise-induced muscle damage (EIMD)-as provoked by eccentric work and characterized by reduced strength and delayed-onset muscle soreness (DOMS)-leads to increased formation of venous gas emboli (VGE) during subsequent hypobaric exposure. METHODS: Each subject (n = 13) was on two occasions exposed to a simulated altitude of 24,000 ft for 90 min, whilst breathing ox

Restoration of hamstring function following sciatic nerve resection at the greater sciatic foramen with reconstruction involving acellular nerve allograft and vascularized sural nerve autograft: A case report.

Segmental loss of the sciatic nerve secondary to oncologic resection or trauma is detrimental to hamstring and leg function. The diameter of this nerve and the length of its axons spanning the lower extremity create significant challenges in reconstruction and optimizing return of sensory or motor function. The purpose of this report is to describe outcomes of a free vascularized sural nerve graft to preserve hamstring function in a large proximal sciatic nerve defect beginning at the greater sciatic foramen. A 44-year-old female underwent neoadjuvant chemotherapy and radiation for treatment o

Evaluation of pediatric distal femoral physeal fractures and the factors impacting poor outcome requiring further corrective surgery.

Fractures of the distal femur carry a significant risk of physeal arrest and resulting growth complications which often require additional surgeries to correct the deformity. This study examines the risk of needing corrective procedures as a child approaches skeletal maturity. A retrospective analysis of patients treated at a single institution for distal femoral physeal fractures from 2000 to 2015 was performed. Association between sex, age, Salter-Harris (SH) class, and fracture displacement with the risk of physeal arrest were examined. Association between years of growth remaining to skele

Tendon lengthening after achilles tendon rupture-passive effects on the ankle joint in a cadaveric pilot study simulating weight bearing.

BACKGROUND: In recent years, primary Achilles tendon ruptures have increased due to the aging population's participation in physically demanding activities. These injuries commonly occur during recreational sports and frequently lead to a long-term reduction in activity despite treatment. Non-operative methods of treatment for Achilles tendon ruptures may result in the Achilles healing in a lengthened position compared to the pre-injury state. This study uses a cadaveric model that simulates static weight bearing to explore the effect of a lengthened Achilles tendon on ankle joint load distrib
