Derivation and validation of a Short Form of the Mini-Mental State Examination for the screening of dementia in older adults with a memory complaint.

BACKGROUND AND PURPOSE: To validate a Short Form of the Mini-Mental State Examination (SMMSE) as a screening test for dementia in older ambulatory individuals followed in a memory clinic for a memory complaint. METHODS: A total of 202 cognitively healthy individuals, 100 individuals with a mild cognitive impairment and 304 demented individuals sent for a memory complaint by their primary care physician to a memory clinic were prospectively included in this cross-sectional study. They were randomized into derivation (n = 303) and validation (n = 303) groups. The SMMSE score was built from six

Latent tuberculosis infection screening in Minnesota's critical access hospitals.

The prevalence of tuberculosis (TB) declined worldwide during most of the 20th century. However, between 1980 and 1990, the incidence rate rose. Subsequently, the United States, through the Centers for Disease Control and Prevention (CDC), and the United Nations increased efforts to control TB and reduce its burden on population health. One of those efforts has been offering guidance about screening health care workers. This article reports on the findings of an investigation into whether Critical Access Hospitals in Minnesota are following the CDC guidelines for screening and treatment for la

Targeted surveillance for postnatal hearing loss: a program evaluation.

OBJECTIVE: The importance of monitoring hearing throughout early childhood cannot be understated. However, there is a lack of evidence available regarding the most effective method of monitoring hearing following the newborn screen. The goal of this study was to describe a targeted surveillance program using a risk factor registry to identify children with a postnatal hearing loss. METHODS: All children who were born in Queensland, Australia between September 2004 and December 2009, received a bilateral 'pass' on newborn hearing screening, and had at least one risk factor, were referred for t

Pooled-testing procedures for screening high volume clinical specimens in heterogeneous populations.

Pooled testing is a procedure commonly used to reduce the cost of screening a large number of individuals for infectious diseases. In its simplest form, pooled testing works by compositing a set of individual specimens (e.g., blood or urine) into a common pool. If the pool tests negative, all individuals within it are diagnosed as negative. If the pool tests positive, retesting is needed to decode the positive individuals from the negative individuals. Traditionally, pooled testing has assumed that each individual has the same probability of being positive. However, this assumption is often un

Tropical bacterial gastrointestinal infections.

The bacterial gastrointestinal infections cholera, salmonellosis, shigellosis, campylobacteriosis, and diarrheagenic Escherichia coli are prevalent in tropical regions. These diseases impose an immense cost and contribute significantly to childhood morbidity and mortality. Management is hampered by limited access to diagnostic facilities and by antimicrobial drug resistance. Rapid point-of-care assays aim to reduce treatment delay and encourage rational use of antimicrobial agents. Control through safe drinking water, good sanitation, and vaccination against typhoid and cholera in high-risk po

Two-site automated chemiluminescent assay for measurement of immunoreactive renin.

Measurement of renin is important for the clinical assessment of hypertensive patients and for the screening for primary aldosteronism. The aim of this study was to evaluate the performances of an automated immunoassay for measurement of immunoreactive renin. Functional sensitivity, in vitro stability, and reference values were determined. Method comparison with the plasma renin activity assay was also performed. Our results demonstrate that the Liaison(®) direct renin assay may assist the clinician in the assessment of hypertensive patients and in the screening for primary aldosteronism.

Expression of ESBL-like activity in infrequently encountered members of the family Enterobacteriaceae.

A collection of 94 unusual members of the Enterobacteriaceae were screened for the presence of extended spectrum β-lactamases (ESBLs) using the MicroScan ESβL plus dried confirmation panel. Presumptively positive strains were then confirmed for the presence of an ESBL by double disk diffusion, E-test strips (AB Biodisk, Solna, Sweden) and PCR for SHV, TEM, and CTX-M2 genes. Of the 18 strains initially positive on the ESβL panel only three strains (Leminorella grimontii, Klebsiella ozaenae, and Kluyvera ascorbata) were positive by confirmation methods. These results suggest laboratories should

Public health implications of sickle cell trait: a report of the CDC meeting.

Although the issue of whether sickle cell trait (SCT) is clinically benign or a significant health concern has not yet been resolved, the potential health risk to affected individuals is of vital importance and represents a tremendous challenge in protecting, promoting, and improving the health of the approximately 300 million people worldwide and 3 million people in the U.S. who possess the trait. In response to a request by the Sickle Cell Disease Association of America, in December 2009, the CDC convened a meeting of partners, stakeholders, and experts to identify the gaps in public health,

Psychosocial factors influencing breast cancer risk appraisal among older women.

Although the incidence of breast cancer increases with age, many older women are uninformed about the increased risk and have lower mammography screening rates than younger women. Understanding older women's perceptions of risk might assist health care providers in offering appropriate resources that result in screening. In this study, we explored psychosocial components influencing older women's breast cancer risk appraisal. To identify key psychosocial components of breast cancer risk appraisal, we conducted focus group interviews. Data saturation occurred with four groups (N = 36) of older

Prevalence of immunoglobulin A deficiency in Chinese blood donors and evaluation of anaphylactic transfusion reaction risk.

OBJECTIVES/AIMS: We investigated the incidence of immunoglobulin A (IgA) deficiency in Chinese population. BACKGROUND: The frequency of IgA deficiency, defined as a serum IgA level of <0.05 mg dL(-1) , has been broadly studied in different ethnic groups. Individuals with IgA deficiency may form specific antibodies against IgA, which can cause an anaphylactic response when the patient receives an IgA-containing blood transfusion. METHODS: A sandwich enzyme-linked immunosorbent assay was performed to screen for IgA deficiency and particle gel immunoassay used for confirmation. IgA antibodies w

Alpha-1-antitrypsin deficiency in early childhood.

Alpha-1-antitrypsin deficiency (AATD), which predisposes liver disease in children, is often undiagnosed. Isoelectric focusing in 161 infants with liver dysfunction revealed 14.7% severe and 12.2% moderate AATD. Positive PAS-D and immunohistochemical staining was found in 60% of severe AATD, but in moderate AATD, only immunohistochemistry was positive in 100%. Bilirubinostasis, hepatomegaly, splenomegaly, cholestasis, hepatomegaly associated with cholestasis, acholia, high transaminases, and low birthweight were significantly more frequent in severe than in moderate AATD. Both AATDs showed sig

Maternal serologic screening to prevent congenital toxoplasmosis: a decision-analytic economic model.

OBJECTIVE: To determine a cost-minimizing option for congenital toxoplasmosis in the United States. METHODOLOGY/PRINCIPAL FINDINGS: A decision-analytic and cost-minimization model was constructed to compare monthly maternal serological screening, prenatal treatment, and post-natal follow-up and treatment according to the current French (Paris) protocol, versus no systematic screening or perinatal treatment. Costs are based on published estimates of lifetime societal costs of developmental disabilities and current diagnostic and treatment costs. Probabilities are based on published results and

Enhanced tuberculosis identification through 1-month follow-up of smear-negative tuberculosis suspects.

SETTING: Bandim Health Project, Bissau, Guinea-Bissau. OBJECTIVE: To conduct tuberculosis (TB) screening among former TB suspects in whom TB had been ruled out on initial consultation and therefore assumed to be TB-negative (aTBneg). DESIGN: In a cohort follow-up study, 'aTBneg suspects' were screened for symptoms from 1 month after the initial negative sputum smear examination. Symptomatic individuals were referred for clinical re-examination and human immunodeficiency virus (HIV) testing. RESULTS: Among 428 TB suspects presenting over a 10-month period in 2007, 80% (343) were smear-negati
