Concentration-time relationships for the effects of inhaled trichloroethylene on signal detection behavior in rats.

The risk from inhaled volatile organic compounds (VOCs) is presently assessed on the basis of lifetime exposure to average concentrations of the vapor. This strategy yields rational predictions of risk if the product of concentration (C) and the duration of exposure (t) yields constant effects on health (Haber's Rule). The validity of this assumption was evaluated by assessing the acute behavioral effects of inhaled trichloroethylene (TCE) vapor at various values of C and t. Adult male Long-Evans rats (n = 11) were trained to perform a signal detection task in which a press on one lever produc

[The choice of the intervention in the surgical treatment of nontoxic diffuse multinodular goiter].

The choice between sub-total (STT) and total (TT) thyroidectomy in surgical management of multinodular goitre should be based on the disease pathophysiology and the critical review of short and long-term results of these treatments. In order to make a comparative evaluation the authors carried out a retrospective analysis on a series of patients operated from 1970 to 1993 and on the results of a 16.3 years mean follow-up. Of 551 patients operated on the thyroid gland, 389 (70.6%) affected by multinodular goitre were considered. 340 were female and 49 male (39 years medium age). 341 (87.6%) und

A look at recent hyperventilation studies: outcomes and recommendations for early use in the head-injured patient.

During the past decade, mounting controversy surrounding the use of hyperventilation for the treatment of head injury has raised concerns about its safety and therapeutic benefits. A recent investigation of the medical literature was conducted to determine if outcomes for the head-injured patient continued to support the use of indiscriminate, and often unmonitored, hyperventilation in the pre-hospital and early hospital phases of care, and to determine if current investigators are recommending the use of hyperventilation for the initial treatment of all unconscious head-injured patients. Asystoles during infancy recorded by home memory monitors. Benign events?

OBJECTIVES: To assess the frequency and clinical significance of asystole (sinus arrest > or = 2.0 seconds) and the incidence of bradycardia in infants prescribed home cardiorespiratory monitors and to test the hypothesis that asystoles are more likely to occur in preterm infants. DESIGN: Prospective, consecutive sample of monitor printouts. METHODS: All 291 printouts from the memory monitors of 161 patients received during a 2-month period were analyzed. SETTING: University hospital providing primary and referral care. MAIN OUTCOME MEASURES: Asystoles and bradycardias; clinical course of

[Management of craniocerebral trauma in a neurosurgery center].

Aggressive treatment of patients with severe head injury increases the chance for survival and good functional outcome in most cases. To prevent irreversible cerebral lesions, the key point of treatment is the management of intracranial hypertension caused by intracranial hematomas, brain edema and impaired circulation of cerebrospinal fluid (CSF). Therapeutic standards are surgery of traumatic hematoma, osmotherapy and mild hyperventilation for brain edema, and CSF drainage. In highly elevated intracranial pressure (ICP) administration of barbiturates and forced hyperventilation can be consid

Ventriculostomy infections: the effect of monitoring duration and catheter exchange in 584 patients.

The investigators undertook a retrospective analysis of ventriculostomy infections to evaluate their relationship to monitoring duration and prophylactic catheter exchange. In 1984, the results of an epidemiological study of ventriculostomy-related infection were published. One of the conclusions of the paper was that the incidence of ventriculostomy-related infections rose after 5 days of monitoring. This led to the recommendation that catheters be prophylactically changed at 5-day intervals if prolonged monitoring was required. A recent randomized prospective study on central venous catheter

Position of jugular oxygen saturation catheter in patients with head trauma: assessment by use of plain films.

OBJECTIVE: The purpose of this study was to establish a plain radiographic technique for the assessment of the position of a jugular oxygen saturation catheter in patients with head trauma. MATERIALS AND METHODS: In the experimental study, jugular oxygen saturation catheters were introduced into the internal jugular veins of four cadavers. Correct positioning of the catheter tips was monitored by CT. Concurrent anteroposterior radiographs of the skulls were obtained with the tubes angled in a transverse plane and in a sagittal plane at intervals of 5 degrees and within a total range of 70 deg

Automated EEG monitoring in defining a chronic epilepsy model.

There has been a recent surge of interest in chronic animal models of epilepsy. Proper assessment of these models requires documentation of spontaneous seizures by EEG, observation, or both in each individual animal to confirm the presumed epileptic condition. We used the same automatic seizure detection system as that currently used for patients in our institution and many others. Electrodes were implanted in 43 rats before intraamygdalar administration of kainic acid (KA). Animals were monitored intermittently for 3 months. Nine of the rats were protected by anticonvulsants [pentobarbital (P

Somatosensory evoked potential monitoring following severe closed head injury.

This paper describes the results of somatosensory evoked potential (SSEP) monitoring in 65 patients with severe head injury. Intracranial pressure (ICP) monitoring data were available for 63 patients, and arterial-jugular oxygen content (AVDO2) data for 52 patients. Eighty-nine percent of patients with no SSEP activity beyond 50 msec post-stimulus in either hemisphere died or were vegetative survivors (3 month Glasgow Outcome Score). All 17 patients with a good or moderate outcome had long latency cortical activity (i.e. > or = 70 msec post-stimulus) in both hemispheres. Among patients with ab

Magnetic resonance imaging and P-31 magnetic resonance spectroscopy provide unique quantitative data useful in the longitudinal management of patients with dermatomyositis.

OBJECTIVE: To evaluate the utility of magnetic resonance imaging (MRI) and P-31 magnetic resonance spectroscopy (MRS) in the longitudinal management of patients with dermatomyositis (DM). METHODS: The study group consisted of 11 patients, including 3 children, all of whom had a clinical diagnosis of DM. A control group of 8 subjects was studied simultaneously. MRI included images as well as calculations of T1 and T2 relaxation times. The P-31 MRS protocol evaluated metabolic status (i.e., inorganic phosphate/phosphocreatine ratios and phosphocreatine and ATP levels) during rest, exercise, and

The Automatic Patient Symptom Monitor (APSM): a voice mail system for clinical research.

Double-blind clinical trials become very tedious when symptoms are measured rather than objective laboratory and physical parameters. The standard "diary card" method is labor intensive for patients and impractical to use for more than a few weeks. In chronic relapsing disorders it would be far better for patients to record symptoms one or more times per day, at defined times, for weeks, months or even years. The Automatic Patient Symptom Monitor (APSM) is a voice processing system designed to achieve this goal. APSM calls patients at home every night, addresses each patient by name and then a

Lack of yohimbine effect on ambulatory blood pressure recording: a double-blind cross-over trial in parkinsonians with orthostatic hypotension.

Yohimbine is an alpha 2-adrenergic antagonist which has been proposed for the treatment of orthostatic hypotension (OH). We investigated in a double-blind placebo-controlled crossover trial the effects of yohimbine (2 mg tid) on blood pressure (BP) and heart rate (HR) using ambulatory monitoring of blood pressure (AMBP) in 17 patients (69 +/- 5 years) with Parkinson's disease (PD) suffering from non drug-induced OH. At the end of each sequence of 4 weeks, mean systolic (SBP) and diastolic (DBP) BP and HR (+/- SD), BP variability (coefficient of variation of the mean) and nychtemeral rhythm wer

Use of the esophageal stethoscope in pediatric transports.

INTRODUCTION: Monitoring cardiac and respiratory function during helicopter transports can be difficult, so the authors devised a study of the esophageal stethoscope for pediatric patients. SETTING: The study was conducted at Pediflite in Tennessee, which transports between 250 to 300 pediatric patients every year. MATERIALS AND METHODS: A disposable esophageal stethoscope was used as an adjunct to other monitoring systems in intubated patients. Prior to transport, the stethoscope was placed orally by the respiratory therapist and advanced to the point where heart sounds are heard best, at w

On-line monitoring of global cerebral hypoxia in acute brain injury. Relationship to intracranial hypertension.

Global cerebral oxygenation, perfusion pressure, and expired CO2 tension were continuously monitored in 69 adults with acute severe closed brain trauma. Cerebral oxygenation was assessed by continuous fiberoptic monitoring of jugular bulb oxyhemoglobin saturation, in conjunction with continuous monitoring of arterial oxyhemoglobin saturation. Jugular desaturation associated with arterial desaturation (hypoxemic cerebral hypoxia) was evaluated and managed. A total of 121 episodes of combined arterial and jugular desaturation were documented, 76 in the presence of gross abnormalities identified

[Care for brothers and sisters of crib death children. Evaluation of a support project as an alternative to home monitoring].

A pilot-study was carried out on support measures chosen by 40 families having a subsequent child after their experience with a cot-death infant. Cardio-respiratory monitors were used in one group and weighing scales in the other. General support measures given to both groups included the keeping of symptom charts and weekly visits by health-nurses. Parents who used a home-monitor relied upon the medical-technical approach of the problem, while parents using scales were mainly given confidence by the personal attention of the health-nurse. Monitor-parents more often called the paediatrician fo
