No evidence of association between CAG expansions and essential tremor in a large cohort of Italian patients.

Essential tremor (ET) is one of the most common movement disorders. However the pathogenesis is as yet unknown, although a genetic cause has long been recognised. Clinical and molecular evidences suggested that the ET gene might contain a CAG expanded region. In a cohort of Italian ET patients Repeat Expansion Detection (RED) approach did not demonstrate large CAG expansions. We extended the study towards specific targets: the channel proteins hSKCa3 and CACNL1A4. Direct assessment of CAG stretches within these two genes did not demonstrate any CAG expansion in affected subjects. Also a case-c…

Idiopathic torsion dystonia: assignment of a gene to chromosome 18p in a German family with adult onset, autosomal dominant inheritance and purely focal distribution.

Idiopathic torsion dystonia (ITD) is a group of movement disorders which is usually inherited in an autosomal dominant manner with reduced penetrance. Most patients with ITD present with focal dystonia at adult age. However, thus far, this common subform remained unmapped chromosomally. In contrast, a rare early onset, more generalized form of ITD has been mapped to chromosome 9q34. Our linkage study in a large pedigree with seven definitely affected, six possibly affected and 16 phenotypically unaffected family members assigns an ITD gene for the common focal form with a maximal lod score of …

[Drug-induced extrapyramidal disorders].

We analyze 402 drug-adverse events consisting of movement disorders or aggravation of parkinsonisms, submitted to Sistema Español de Farmacovigilancia until 1994. Our aim is to know patient characteristics and the drugs related with these submissions. Most of them (64) belong to calcium-entry blocker group (31%) and benzamides (27%). Case age intervals more frequent were 11-30 and 60-80 years-old and the events affect predominantly females. The percentage of serious adverse events were near 80%. We think that drug-related parkinsonisms have high prevalence rate and that the role of calcium-ent…

Practice parameters for the use of stimulants in the treatment of narcolepsy. Standards of Practice Committee of the American Sleep Disorders Association.

Narcolepsy is a disorder of the central nervous system characterized by excessive sleepiness, cataplexy, and other rapid-eye-movement (REM)-sleep phenomena such as sleep paralysis and hypnagogic hallucinations. Although stimulants are the only effective treatment for the sleepiness of narcolepsy, no clinical guidelines on the use of stimulants in the treatment of narcolepsy have been published that address the following factors: appropriate doses; development of tolerance; potential for side effects, adverse reactions and abuse; and use in children and pregnant or breast-feeding women. These p

Essential tremor and buccolinguofacial dyskinesias.

Essential tremor (ET) has been described in association with major dyskinesias, such as dystonia musculorum deformans, torticollis, and dystonic writer's cramp. In a series of 104 patients affected by ET, the incidence of buccolinguofacial dyskinesias (BLFD) has been investigated, including a comparison with another dyskinesia involving the facial muscles--mimic muscle tremor. The association of ET and BLFD may be casual; however, on the other hand, it might suggest the existence of a subgroup of patients suffering from a non-specific sign of motor control impairment, such as ET, in which othe

Experimental dystonia induced by quaternary-chlorpromazine.

When quaternary-chlorpromazine (Q-CPZ) was administered intraventricularly (ICV) to rats, it induced a lateralized dystonic reaction, which progressed to head-to-tail barrel rolling. The syndrome persisted for approximately 10 minutes, was not antagonized by pretreatment with drugs used to treat extrapyramidal movement disorders, and could not be mimicked by ICV administration of dopamine antagonists. Unlike known dopamine antagonists, Q-CPZ does not alter dopamine turnover, cause prolactin release in vivo, or bind to dopamine/neuroleptic receptors in vitro. These data suggest that Q-CPZ diffe…

Pediatric Interventions in a Sanfilippo Syndrome Patient Under General Anesthesia: A Case Report.

Background: Mucopolysaccharidosis (MPS) Type III (MPS III) or Sanfilippo syndrome is a rare autosomal recessive inherited metabolic disorder. This disorder is responsible for lysosomal storage disorder at the cellular aspect. Due to lysosomal enzyme perturbance leading to the alteration of macromolecule metabolisms, this cellular perturbance causes multiple severe systemic and mental outcomes. Sanfilippo syndrome is the most frequent lysosomal disorder among the different types of MPS. Case Presentation: A 9-year-9-month-old female was presented at our private clinic accompanied with her paren

[Sleep changes with aging].

Many factors contribute to the alteration of sleep in older adults. Most of their complaints can be explained by the modifications of the sleep organisation observed in this population. Sleep architecture is altered with aging. Insomnia and excessive daytime sleepiness can reflect an alteration of the circadian rhythm. This population is also frequently exposed to specific sleep disorders such as the restless leg syndrome, periodic limb movements or obstructive sleep apnea syndrome. Sleep disorders in patients with Alzheimer's disease is a daily preoccupation at home or in institution. Circadi

Eye signs and tic disorders: Gilles de la Tourette's syndrome.

BACKGROUND: Eye tics are among the most common manifested symptoms of tic disorders, including Gilles de la Tourette's syndrome (GTS). METHODS: A review of medical literature or ocular signs and symptoms of tic disorders was conducted. RESULTS: Tic disorders are characterized by repetitive and often disabling movements that primarily involve the eyes, face, neck, and voice apparatus. In addition, patients with tics may have eye problems related to medications used to control tics. Some eye signs are atypical in tic disorders and their presence may suggest alternate diagnoses. CONCLUSION: Ey

De novo DHDDS variants cause a neurodevelopmental and neurodegenerative disorder with myoclonus.

Subcellular membrane systems are highly enriched in dolichol, whose role in organelle homeostasis and endosomal-lysosomal pathway remains largely unclear besides being involved in protein glycosylation. DHDDS encodes for the catalytic subunit (DHDDS) of the enzyme cis-prenyltransferase (cis-PTase), involved in dolichol biosynthesis and dolichol-dependent protein glycosylation in the endoplasmic reticulum. An autosomal recessive form of retinitis pigmentosa (retinitis pigmentosa 59) has been associated with a recurrent DHDDS variant. Moreover, two recurring de novo substitutions were detected i

CBT and positive psychology interventions for clinical depression promote healthy attentional biases: An eye-tracking study.

BACKGROUND: Although there is a growing interest in the role of attentional biases in depression, there are no studies assessing changes in these biases after psychotherapeutic interventions. METHODS: We used a validated eye-tracking procedure to assess pre-post therapy changes in attentional biases toward emotional information (i.e., happy, sad, and angry faces) when presented with neutral information (i.e., neutral faces). The sample consisted of 75 participants with major depression or dysthymia. Participants were blindly assigned to one of two 10 weekly sessions of group therapy: a cognit

Motor imagery in bipolar depression with slowed movement.

We hypothesized that motor retardation in bipolar depression is mediated by disruption of the pre-executive stages of motor production. We used functional magnetic resonance imaging to investigate neural activity during motor imagery and motor execution to elucidate whether brain regions that mediate planning, preparation, and control of movement are activated differently in subjects with bipolar depression (n = 9) compared with healthy controls (n = 12). We found significant between-group differences. During motor imagery, the patients activated the posterior medial parietal cortex, the poste

Evaluation of temporomandibular joint disc displacement via MRI and computerized axiography: a case report.

BACKGROUND: Magnetic resonance imaging (MRI) is commonly used as a reference imaging modality for evaluating the temporomandibular joint (TMJ) disc-condyle relationship because it provides direct visualization of the articular disc in closed- and open-mouth positions. However, routine MRI primarily provides structural information at static mandibular positions and may not adequately reflect dynamic mandibular function. Consequently, discrepancies between imaging findings and clinical symptoms or treatment response may create diagnostic uncertainty in selected patients. Computerized axiography
