The Effects of Virtual Immersive Gaming to Optimize Recovery (VIGOR) in Low Back Pain: A Phase II Randomized Controlled Trial.

Background: Chronic low back pain (cLBP) with kinesiophobia is difficult to treat, and traditional graded activity approaches often show limited adherence and short-term effects. Virtual reality (VR) may enhance treatment engagement by providing immersive game-based environments that encourage therapeutic movement. This randomized controlled trial aimed to examine the effects of VR interventions designed to promote lumbar spine flexion in individuals with cLBP and elevated movement-related fear. Methods: Participants were randomized to one of two nine-week VR game conditions that differed only

Testing a Personalized Approach to Chronic Low Back Pain: A Randomized Controlled Trial in Older Veterans.

OBJECTIVE: We aimed to test the efficacy of personalized treatment of older veterans with chronic low back pain (CLBP) delivered by Aging Back Clinics (ABCs) as compared with usual care (UC). METHODS: Two hundred ninety-nine veterans aged 65 to 89 with CLBP from three Veterans Affairs (VA) medical centers underwent baseline testing, randomization to ABC or UC, and 12 months of follow-up. ABC care was guided by trained physicians and published algorithms targeting key conditions contributing to CLBP (eg, hip osteoarthritis, depression, fibromyalgia). UC was guided by the participant's primary

Diagnosis and Treatment of Thelazia callipaeda Infection: A Case Report.

This study reports a 50-year-old male case of Thelazia callipaeda infection without a history of pet contact. The patient exhibited chronic ocular surface inflammation after exposure to wild insects and was misdiagnosed multiple times with bacterial conjunctivitis before definitive diagnosis by an ophthalmic specialist. This report emphasizes the core steps of the T. callipaeda extraction procedure. These steps include preoperative preparation, topical ocular anesthesia using 0.4% oxybuprocaine hydrochloride eye drops, sufficient exposure of the surgical field with an eyelid speculum, precise

Review of Peptic Ulcer Disease: Insights from Pathophysiology, Mechanisms, Pharmacotherapy, Phytotherapy, and Case Reports.

Peptic ulcer disease (PUD) remains a significant global health concern associated with substantial morbidity. The primary etiologies include Helicobacter pylori infection and the use of nonsteroidal anti-inflammatory drugs (NSAIDs), both of which impair gastric mucosal defense mechanisms. This review aims to provide an updated overview of the pathogenesis, clinical manifestations, diagnostic approaches, and current management strategies of PUD, including emerging therapeutic developments. Advances in diagnostic modalities, such as endoscopy and imaging, have improved early detection and clinic

A novel clustering of down syndrome and coeliac disease complicated by enteritis and small intestinal strictures: a case series.

Down syndrome (DS) increases risk for autoimmune conditions, including coeliac disease (CeD). Here, we report five adults with DS and biopsy-proven CeD who developed enteritis and symptomatic small bowel stricturing, a clustering not previously described in this population. All five patients had confirmed DS and CeD, and despite apparent adherence to a gluten-free diet, all developed small bowel strictures requiring endoscopic intervention, most commonly at the duodenal D1/D2 junction. Stricture histology demonstrated active CeD with varying degrees of inflammation and fibrosis. Three patients

Effectiveness of disease management across healthcare settings: the Multiple Sclerosis Center is a pillar for multi-disciplinary practices that promote quality of life for people with Multiple Sclerosis.

INTRODUCTION: Multiple Sclerosis (MS) is a chronic neurological disorder, prevalent in young adults. MS leads in disability accrual, thus affecting overall patients' quality of life. Moreover, the management of MS poses significant burden on health systems worldwide. The present study delves into the impact of different health providing settings (e.g. private office/Clinic vs. specialized MS Center) on the effectiveness of MS management, as well as on patient-reported outcomes related to the quality of life (Axis A). Moreover, the study addresses their relative effectiveness in a health crisis

Enhancing the Therapeutic Efficacy of Berberine and Quercetin Through Salt Formulation for Liver Fibrosis Treatment.

Liver fibrosis, caused by chronic hepatic injury, is a major threat to human health worldwide, as there are no specific drugs available for its treatment. Natural compounds, such as berberine (BBR) and quercetin (QR), have shown the ability to regulate energy metabolism and protect the liver without significant adverse effects. Additionally, combination therapy (the cocktail therapy approach), using multiple drugs, has shown promise in treating complicated conditions, including liver injury. In this study, we prepared a salt formulation of BBR and QR (BQS) to enhance their combined effect on l

Rationale and design of the IMProving Adherence to medications for breast Cancer and cardiovascular disease equiTably (IMPACT) randomized clinical trial.

BACKGROUND: Cardiovascular disease (CVD) is a leading cause of mortality in patients with early-stage breast cancer (BC), many of whom are prescribed endocrine therapy (ET) to prevent BC recurrence. Yet, nonadherence to ET and CVD medications is common among BC survivors, particularly in underserved populations at high risk for adverse outcomes. Little is known about how to equitably improve medication adherence for multiple chronic conditions. METHODS: The IMProving Adherence to Medications for Breast Cancer and Cardiovascular Disease EquiTably (IMPACT) trial is an investigator-initiated, si…

Health Outcomes in Childhood Cancer Survivors with Congenital Anomalies in the Childhood Cancer Survivor Study.

BACKGROUND: Congenital anomalies are associated with an increased risk of childhood cancer. However, there is a knowledge gap about health outcomes for childhood cancer survivors with congenital anomalies. METHODS: We included childhood cancer survivors from the Childhood Cancer Survivor Study (n = 22,247), comparing survivors with and without self-reported anomalies. Using Cox regression, we estimated HR and 95% confidence intervals (CI) of chronic health conditions (CHC) classified per the Common Terminology Criteria for Adverse Events from 1 (mild) to 5 (fatal) and subsequent malignant neo

Air quality and health risks in chemical supply shops in southeastern Nigeria.

This study assessed the concentrations of sulfur dioxide (SO2), nitrogen dioxide (NO2), and hydrogen sulfide (H2S) in chemical stores at Ogbete Market, Enugu, and Ogbo-ogwu Market, Onitsha, southeastern Nigeria, during the wet season (June to August 2024). Air quality measurements, taken from six randomly selected stores in each market and a control site (clothing store) located at about 900 m away, revealed that while concentrations of SO2 and NO2 far exceeded public health guidelines (WHO), they remained below Occupational Safety and Health Administration (OSHA) limits. However, chronic low-

Antiarrhythmic potential of SGLT2 inhibitors: Mechanistic insights and clinical evidence.

Sodium-glucose cotransporter 2 (SGLT2) inhibitors were originally developed to treat type 2 diabetes by promoting glycosuria through inhibition of renal glucose reabsorption. However, their clinical utility has expanded rapidly following the demonstration of consistent cardiovascular and renal benefits in large-scale randomized controlled trials. These trials have shown that SGLT2 inhibitors significantly reduce adverse cardiovascular outcomes-including sudden cardiac death-in patients with diabetes, heart failure, or chronic kidney disease, irrespective of glycemic control. As clinical experi

Retinal detachment in a neonate with congenital chylothorax and purpura fulminans associated with the PAK2 genetic variant: A case report.

INTRODUCTION AND IMPORTANCE: A potential relationship between bilateral retinal detachment, chylothorax, and purpura fulminans in a female neonate with a PAK2 gene variant is not commonly reported. This emphasizes the significance of early ophthalmologic assessment in neonates with congenital chylothorax. CASE PRESENTATION: A full-term female infant weighing 2.775 kg was delivered by cesarean section due to breech presentation. Prenatal imaging revealed fetal bilateral pleural effusion, suggestive of chylothorax. The neonate developed respiratory distress and purpura fulminans after birth. Th

Shear Wave Elastography for measuring the elastic properties of the Psoas Major muscle: An intra- and inter-examiner reliability study.

BACKGROUND: The psoas major (PM) plays an important role in population with low back pain (LBP). Its evaluation considers clinical history, which can be confused with other lumbar and lower limb conditions, physical examination, with an inconsistent palpation, and imaging tests that provide inconclusive parameters in various studies. Therefore, developing reliable assessment procedures to evaluate PM elastic properties is necessary to improve diagnosis and follow-up. This study aimed to evaluate the intra and inter-examiner reliability of shear wave elastography (SWE) for calculating the PM st

3,4-Dihydroxyphenylglycol levels separate multiple system atrophy from Parkinson disease with orthostatic hypotension.

BACKGROUND: The autonomic synucleinopathy multiple system atrophy (MSA) can be difficult to distinguish clinically from Parkinson disease with orthostatic hypotension (PD+OH). 18F-Dopamine positron emission tomography separates these conditions based on cardiac noradrenergic deficiency in PD+OH and not in MSA but is available only at the NIH Clinical Center. 3,4-Dihydroxyphenylglycol (DHPG) is the main neuronal metabolite of norepinephrine. This retrospective observational study examined whether DHPG levels in cerebrospinal fluid (CSF) or plasma differentiate MSA from PD+OH. METHODS: We revie

Bullous Wells' Syndrome: Case Report and Systematic Review.

Background/Objectives: Wells' syndrome (WS) is an uncommon cutaneous disease with unknown etiology. Itchy bullous lesions and erythematous plaques characterize the bullous WS (BWS), a rare subtype of the syndrome. We describe the case of a woman affected by chronic lymphocytic leukemia who developed BWS and responded to the classic corticosteroid treatment. We also systematically reviewed the literature, analyzing the clinical, laboratory, and histological features and treatments of this rare disease. Methods: We used the databases Ovid MEDLINE, PubMed, and EMBASE with the following search ter

The Overlooked Burden of Atopic Comorbidities in Eosinophilic Esophagitis: Insights from a Real-Life Comprehensive Multidisciplinary Evaluation.

Background/Objectives: Eosinophilic esophagitis (EoE) is a T2-mediated disease characterized by dysphagia and food impaction. It is often associated with other atopic disorders and is considered a late manifestation of the "atopic march". In clinical practice, allergic comorbidities are frequently underdiagnosed and primarily based on self-reporting, potentially underestimating the true burden of T2-related pathology. To address this, a multidisciplinary task force was established at our tertiary center to systematically evaluate newly diagnosed patients with EoE. Methods: This cross-sectional

Wearable devices in neurological disorders: a narrative review of status quo and perspectives.

BACKGROUND AND OBJECTIVE: Neurological disorders are a group of diseases involving motor, sensory, cognitive, and autonomic functions, among which stroke, Alzheimer's disease (AD), and Parkinson's disease (PD) are prevalent. Their management, especially in conditions with chronic courses or long-term sequelae, remains a substantial unmet need. With the growing comprehension of neuroscience, the development of digital technology, and the rising demand for quality of life, wearable devices offer a promising solution for disease management. The review aimed to evaluate the application and prospec
